Currently, laparoscopic radical prostatectomy (LRP) has become a routine procedure for the treatment of clinically localized prostate cancer in selected urological departments in Europe and the United States, its tendency is to replace the standard open radical prostatectomy. In China, LRP is now currently performed in initial phase. LRP is a new surgical technology successfully combined minimal invasiveness with the advantages of traditional open surgical procedures. It is advantages for patients who undergo LRP that include minimal invasive nature, the possibility for early catheter removal, potential reduced blood loss nearly zero percent transfusion rate, a shorter hospital stay, shorter postoperative re-convalescence, less intra-operative and postoperative morbidity. It is also advantages for surgeons who perform LRP that include a superior visualization of the operative field and an exact and watertight anastomosis. Recent data show that oncological outcome in LRP is not compromised by the minimal invasive nature of the procedure, LRP is able to provide similar functional results as its open counterpart.But LRP requires significant demanding technique and a considerable learning curve, the operative time in initial development of LRP is too long.Recently, the functional and oncological outcome in LRP are far from being excellent although LRP has emerged. Some uncertainty remain on the neuroanatomy of the neurovasular bundle and cavernasal nerves, urethral striated sphincters and its innervation. How to improve and refine the techniques of LRP reduces the positive surgical margin rates, especially the iatrogenic positive surgical margin rates.
