To observe the attention and working memory changes and modafinil effects after total sleep deprivation(SD) in healthy young male volunteers and to explore the relative mechanisms by BOLD-fMRI and hydrogen proton magnetic resonance.The results suggested that the attention performance decreased significantly and working memory performance did not change in early SD and decreased significantly in later SD.Modafinil and placebo improved the performance notably and the effect of modafinil was more obvious.The parietal lobe activation decreased and frontal and thalamus activation increased after SD.Modafinil inceased the parietal lobe activation and decreased the frontal and thalamus activation.Modafinil increased significantly the decreased Cho/Cr and Lac/Cr value of anterior cingutate after SD. We conclude that modafinil and placebo can improve significantly the cognitive performances damaged after SD and the effect of modafinil was more obvious. The performances are the worse, the modafinil effects are the more obvious.Maybe modafinil can improve the damaged cognition by ameliorating the brain activation model and substance metabolism.
