| BPH (benign prostatic hyperplasia, BPH) is common in middle-aged or old men, which usually occurs at the age of 40. The incidence of the disease among people of 50 years old reaches 40％, and increases gradually with age, up to 70％-80％at the age of 80. The caus e of the di s eas e is not so clear, but it is closely related to increase in age and changes in testicular function. BPH, as a progressive disease, seriously affects the quality of life.With the arrival of an aging population, there has been an increase in life span,but the incidence of benign prostatic hyperplasia in our country is rising.Since BPH has become the maj or disease of international concern, it requires our more in-depth understanding of its etiology and pathology and our further research into its diagnosis and treatment methods.The instructor, Professor He Juqiao,has studied many literatures, and after so many years of clinical experience, according to the principle of combination of disease identification and pattern differentiation in Chinese Medicine, based on pattern differentiation, combined with disease identification, has created the treatment of boosting qi, inhibiting water, quickening blood and dissipating binds.Based on this treatment,he invented Qian Long Tong capsule. It has been proved clinically that thi s capsule can reduce prostate volume in some patients and inhibit the further proliferation of the prostate. Its effect is stable, with no obvious side effects. However, previous studies were limited to the limitations of in vivo experiments, and can not accurately research the effect of Qian Long Tong capsule on cell proliferation of different types and Kinetics of apoptosis cells. It has been accepted in principle that prostatic hyperplasia is mainly due to interstitial cell proliferation.This study,based on the research into the mechanism of this Chinese medicine on cultured human BPH interstitial cells and its clinical effect, will do further research into the treatment characteristics and mechanism of BPH on the level of cell and molecular biology.Part One:Experimental S tudyIn this study, some related technologies are used, such as human prostate interstitial cells cultured in vitro,pharmacology of Chinese medicine in plasma,RT-PCR, western blotting and so on,to explore the effect of Qian Long Tong capsule, which is composed of Chinese herbs of boosting qi and quickening blood,on interstitial cells Fibronectin,CollagenⅣ,Smad4 gene,Smad4 protein.From the gene expression level and molecular level and cell signal transduction at different levels, the effect of the treatment of boosting qi and quickening blood on the benign prostatic hyperplasia the molecular mechanism of cell kinetics is discussed.The results of this study are as follows:(1) Traditional Chinese medicine, Qian Long Tong capsule, is effective to BPH by inhibiting the proliferation and promoting apoptosis of interstitial cell s.(2) Traditional Chinese medicine, Qian Long Tong capsule, is effective in inhibiting the expression and secretion of interstitial cells CollagenⅣ, and thus affecting the process of prostatic interstitial hyperplasia, thereby inhibiting the expression of genes of prostate interstitial cells Fibronectin, but the difference is not statistically significant.(3) Traditional Chinese medicine, Qian Long Tong capsule, strengthens the expression of gene and protein of interstitial cells Smad4, thus contributing to both TGF-βand its receptors and Smad4 together to form the active complex, promoting the complex trans ferring from the cytosol to the nucleus, hindering cell cycle G 1 moving to S,and inhibiting cell proliferation.Smad4 is a factor to transmit information that TGF-βneeds. It inhibits cell proliferation and matrix synthesis by involving and regulating TGF-βsignal transduction,thus affecting the occurrence and development of prostate hyperplasia.Part Two Clinical ResearchIn this study, through the clinical observation of 68 BPH patients for 3 months, it is confirmed that the treatment of BPH with Qian Long Tong Capsule, which is composed of Chinese herbs of boosting qi,quickening blood and dissipating binds is effective.This study shows that this capsule can significantly improve symptoms of BPH, decrease I-PSS score and BS scores, improve prostate symptoms, reduce prostate volume, and improve Qmax,Qave and so on. Compared with the control group, this Chinese medicine is superior in reducing I-PSS score and BS scores, and improving Qmax and Qave. This study shows that this capsule, composed of Chinese herbs of boosting qi, quickening blood and dissipating binds, can treat BPH through improving symptoms, decreasing prostate volume, and reducing obstruction, which provides a use ful inspiration for the treatment of BPH with traditional Chinese compound formula.This part summarizes the academic thoughts of the instructor, Professor He Juqiao, who has studied BPH by means of Integrated traditional Chinese and Western Medicine.(1) The instructor, Professor He Juqiao, from the theory of Integrated traditional Chinese and Western Medicine research and clinical practice, believes that BPH can be classified as the patterns of "Dribbling Urinary Block" or "Seminal Dribbling Block" from the point of Chinese medicine symptom. It should be noted that the etiology and pathogenesis of this disease, combined with cutting-edge of contemporary science studies, is conducive to the development of the study on BPH with Integrated traditional Chinese and Western Medicine, and helps to understand the nature of pathogenesis of BPH from the higher level(2)The instructor believes that to treat BPH,disease identification should come before syndrome differentiat-ion,and the disease and the syndrome should be combined. Apply modern medical methods and means of identi fying diseases to find the cause and different physiological and pathological changes, di fferentiate the syndrome based on TCM theory, and combine the disease and the syndrome, which can achieve better clinical effect.(3)Combining pathomechanism of Chinese Medicine with modern medicine, the instructor advocates (kidney) both qi deficiency and blood stasis should be paid attention to.Based on the pathomechanism of qi deficiency and blood stasis of BPH and deficiency-excess complex, the treatment is to choose Chinese herbs of boosting qi and quickening blood. Take into account both the adj ustment of the local and the regulation of body functions, which reflects the overall concept of Chinese Medicine.(4) Promote clinical with scientific research and attach importance to the mechanism of treating BPH with Chinese herbs.(5) Pay attention to the factors that affect treatment and relapse, and health guidance. Reasonable and timely health guidance helps BPH patients understand the relevant knowledge, ease the psychological pressure on patients to eliminate negative emotions, and enhance the confidence to overcome the disease.
