| Objective:To summarize pathogenesis of postmenopausal osteoporosis from a theoretical point of view, and observe the changes of bone mineral density, blood biochemistry, cytokine of ovariectomized rats with osteoporos through experimental study. To observe therapeutical effect and mechanism of Bushen Huatan formula on postmenopausal osteoporosis from the angle of adjusting bone metabolism of rats, and provide a scientific basis for the clinical application of compound recipt.Methods:1. Theoretical InvestigationFor providing basis to experimental study, we understand pathogenetic factor and mechanism, therapeutical principle and method of osteoporosis pathogenesis in traditional Chinese medicine, as well as modern medicine study on mechanism of TCM treating osteoporosis through reviewing systematically ancient and modern literature. 2 Experimental StudyPostmenopausal osteoporosis rat model was established. Rats were randomly divided into model group, sham operation group, western medicine group (0.23 mg/kg·b·w), high dosage (18.8g/kg·b·w), medium dosage (9.4g/kg·b·w) and low dosage TCM group.3 months after operation,rats in treatment groups were intragastrically administrated for 8 weeks, rats in normal control group and sham operation group were administrated equivalent saline solution for 8 weeks. After rats were sacrificed, femur of left hindlimb was got, then bone mineral density and levels of TNF-αand IL-6 in femur were detected; blood in heart was draw and centrifuged, then contents of ADP, TRACP5b andβ-CTX in serum were detected by ELISA assay; total RNA were extracted from bone marrow cells of femur, then mRNA expression of PPARγ2 was detected by RT-PCR.Statistical analysis:SPSS 13.0 statistical software were used for analysis of variance, all data were represented by mean±standard deviation, P values of< 0.05 were considered statistically significant.Results:1 Theoretical InvestigationRenal deficiency was regarded as key point to onset of osteoporosis in ancient and modern literature about pathogenetic factor and mechanism, therapeutical principle and method of osteoporosis. However, knowledge of other pathogenic factors is not uniform.The topic raised hypothesis that abnormal lipid metabolism maybe related to osteoporosis, and put out new therapeutical method about osteoporosis; Bushen Huatan formula was clinical empirical formula of Professor Xiang Nan, it had better therapeutical effect on osteoporosis, and its effect on osteoporosis has been confirmed in animal experiments.2 Experimental ResearchCompared with sham operation group, bone mineral density of femur was significantly decreased in model group. Compared with model group, bone mineral density of femur was significantly increased in western medicine group and TCM group. There was no significantly difference between western medicine group and high dosage TCM group in bone mineral density of femur.Compared with sham operation group, the levels of ADP, TRACP5b andβ-CTX were significantly increased in model group. Compared with model group, above indices were significantly decreased in western medicine group and high dosage TCM group. There was no significantly difference between western medicine group and high dosage TCM group in above indices. There was significantly difference between medium dosage TCM group and high dosage TCM group inβ-CTX level.Compared with sham rats, ovariectomized osteoporosis model rats HE staining bone marrow cavity expansion, bone thinning, see the fracture phenomenon, and the total area of reduced vision, decreased the number of distributed sparse, widened the gap between the trabecular bone, arranged irregularly. Phlegm western side group and high dose bone marrow cavity smaller quantity of trabecular bone, fracture less, arranged in neat rows, the smaller the gap between the trabecular bone; medicine in larger doses of bone marrow cavity, bone small beam thinning, trabecular bone fracture can be seen clearly arranged in slightly disordered, but better than the untreated group.Immunohistochemical observation shows TNF-αand IL-6 in bone tissue expression mainly in the surrounding trabecular bone osteoclasts, osteoblasts and bone marrow stromal cell cytoplasm observed under light microscope, brownish yellow granules. and was significantly fewer positive cells in the untreated group.Compared with sham operation group, positive cells in bone tissue were significantly increased in model group. Compared with model group, positive cells in bone tissue were significantly decreased in high dosage TCM group and Western medicine group. The levels ofTNF-αand IL-6 levels in high dosage TCM group were lower than those in medium dosage TCM group, but there was no significantly difference between two groups.Compared with sham operation group, mRNA expression of PPARγ2 of bone marrow cells were significantly increased in model group. Compared with model group, above indices were significantly decreased in Western medicine group and high dosage TCM group. Above indices in high dosage TCM group were lower than those in medium dosage TCM group, but there was no significantly difference between two groups.Conclusions:Under the guidance of TCM theory, the application phlegm Treatment of postmenopausal osteoporosis theory, but also experimental research support. The experiment indicated that bone resorption was enhanced and lipid metabolism was abnormal in ovariectomized rats. Bushen Huatan formula could increase bone density, decline serum biochemical indicator and cell factor related to bone resorption, decrease mRNA expressions of PPARγ2.A marked in activity and. Experimental research can only increase with phlegm bone mineral density in ovariectomized rats, lower levels of serum biochemical parameters of bone resorption and reduce bone resorption-related cytokines and PPAR y 2 mRNA expression in bone marrow of ovariectomized rats better treatment of disease. The mechanism may be multi-channel, multi-target to promote bone formation and inhibit bone resorption. |
