Diabetes mellitus(DM) is the commonest endocrinal metabolic disease. This disease is caused by the decrease in the synthesis and excretion of insulin, or the insulin receptor and post-receptor defects. Hyperglycemia is its biochemical feature, which has different degrees of metabolic disorder of sugar, protein and fat, also can it cause deformity and death. Epidemiological survey found that the incidence rate of diabetes is showing rising tendency. Our second hygienic revolution has put DM the four big chronic diseases which need focal prevention and cure.Diabetic gastroparesis (DGP) is defined as utmost retardation of gastric emptying and related gastric motion abnormality. According to abroad investigations and reports, 50%~76% diabetic patients are accompanied with gastroparesis. The investigation in Beijing Concord Hospital also shows that about 50% diabetic patients have symptoms of gastric motility disorder. In recent years, diabetic gastric motility disorder has drawn attention of internal and overseas scholars and clinical physicians, and becomes research hot spots..At present, it is thought that the dyskinetic mechanism of the utmost postpone of gastric emptying includes disorders of gastric electricity rhythm, the decrease of the tension of stomach body, the decrease of the contraction and driving force of the gastric antrum, the abnormal tightening of pylorus, and the imbalance of gastric antrum-dodecadactylon motility. And the changes of stomach smooth muscle, inside-wall nerve, interstitial cells of Cajal, and blood vessel tissue and ultrastructure, and the abnormality of the transmission of the neuro-smooth muscle signal of ICC, and that is probably the important pathological basis of the occurrence of DGP.Our motherland's medicine takes diabetes mellirus as the category of "xiaoke disease" "xiaodan". Previously, some scholars think that yin-deficiency is the root, and dryness and hotness is the branch. Recently, clinical researches show that the patient's constitution is the important factor of the occurrence of xiaoke. Clinical observation shows that besides from the deficiency of both qi and yin, many xiaoke patients can become deficiency of spleen-energy syndrome due to constant deficiency of spleen, or long-term restlessness or uncontrolled diet which hurt spleen. Most patients have the symptoms as abnormal ascending and descending of the vital energy, or accompaniedby the retention of wetness-evil. So, the cause of xiaoke disease has close relationship with deficiency of spleen-energy.The typical digestive tract symptoms of diabetic gastroparesis are early stuffness, anorexia, belching, vomiting, abdominal distention etc, the severity degree of each symptoms differ from patient to patient. That coincide with \veipi in TCM. Its pathological basis is spleen-asthenia and cannot activate, abnormal ascending and descending of the vital energy, inability of the descending of stomach-energy that causes qi stagnation, wetness-evil or stagnation of food is the branch. Besides, ^/-deficiency often causes ^/-stagnancy and blood stasis. Blood stasis promotes debility, and vice versa, which is a vicious cycle. So, diabetic gastroparesis belongs to root-deficient and branch-sufficient syndrome.So, we should use the recipe which has the function of invigorating spleen, regulating qi and activating blood circulation to dissipate blood stasis. We can also combine the manifestation of kidney yang deficiency and stomach yin deficiency which are seen in clinic, and adding some Chinese herbs which have the function of invigorating kidney and enriching stomach. The Chinese composition piweikang is an experienced prescription which is developed by "Researching and Cooperating group on Diabetic Gastrointestinal Motility Dyskinesis through the pathogenesis of DGP and TCM treating method," which has significant curing effect in clinical use.
