Objective Hepatitis C virus(HCV) is characterized by remarkable heterogenousness and advanced changeability.All of the HCV lines with known genome had been analyzed and the results showed that the sequences of nucleotide and aminoacid had significant differences among them.In recent years,peoples have paid more and more attention to HCV gene types.It is found that the patients with different gene types have different clinical manifestations.The course of the disease is related to the HCV gene type.The patients with different gene types have different sensitivities to interferon and Ribvirin.The risk agents of the transmission of HCV also have relations with HCV gene type.HCV gene types have influences on the effects of vaccine.Therefore,HCV gene types have important significance in the affection,transmission,diagnosis,treatment and prevention of HCV.This study was designed to analyze the gene types of hepatitis C and liver cirrhosis with core region type- specific primers and Restriction Fragment Length Polymorphism(RFLP) .Our aim was to explore the gene types of hepatitis C distribution features in Shandong province,thesignificance of gene types in hepatitis C transmission,being chronic,treatment responses and fatty denaturalization.Methods The patients blood samples were obtained at different times.The sera were obtained by centrifuge at 15000rpm and kept in the tubes at -70. C.HCV gene types were analyzed with core region type- specific primers and Restriction Fragment Length Polymorphism (RFLP) .At the same time,the general data of the patients,the transmission approach, liver functions,HCVRNA quantitative analysis,treatment responses and fatty denaturalization under B ultrasound were registered.Results 1. core region type- specific primers and Restriction Fragment Length Polymorphism (RFLP) led to the same results.2.The ratios of blood transmission in HCV RNA gene type Ⅱ,Ⅲ,Ⅱ/Ⅲ were 71.70% (38/) ,16.98% (9/) and 11.32% (6/) respectively,while that of scattered affections were 75.00% (24/32) ,15.63% (5/32) and 9.37% (3/32) .There had no significant difference between these two groups. 3. The distributions of gene type II,III,II/III in chronic hepatitis C were 41/56 (73.21%) ,11/56 (19.64%) and 4/56(7.14%),while that in acute hepatitis were 8/11 (72.72%),2/11 (18.18%) and 1/11 (9.09%) .There had no significant difference between these two groups ( P>0.05 ) .4.The HCV-RNA levels were different remarkably in different HCV gene type.The HCV-RNA quantities of gene type Ⅱ was 107.4 ±3.2,that of gene type Ⅲ was 106.5 ± 2.1 and that of gene type Ⅱ/Ⅲ was 106.6±3.1.The virus level in gene type Ⅱ was higher than gene type Ⅲ and Ⅱ/Ⅲ (P<0.01).5.The interferon reponses of different gene types had remarkable difference.The effective and uneffective distributions of gene typeII,III and II/III were 13,6,2 and 30,5,3.The distribution was correlated to HCV-RNA level. 6.The occurrence ratios of fatty liver in HCVRNA gene type II ,111,II/III were 40.3% (25/) ,21.4% (3/) and 22.2% (2/) with P<0.05.7.The ALT/GGT ratios in the interferon responses and nonresponses group were 134.7 + 36.4,0.23 + 0.04 and 227.4 + 85.3,0.87 + 0.42 with P<0.001.8.The distributions of gene type 11,111 and II/III in the mild,moderate,severe and liver cirrhosis were 8 (66.67%) ,18 (66.67%) ,15 (88.24%) ,13 (72.22%) and 3(25.00%),8(29.63%),0(0.00),l (5.56%)and 1 ( 8.33%) ,1 (3.70%) ,2 (11.76%) ,4 (22.22%) .9.The high dose of interferon and the combination of interferon and ribavilin could improve the effection of hepatitis C with gene type II.Conclusions l.The bases in high reservative area were stable relatively,though the HCVRNA had high grade viability, core region type-specific primers and Restriction Fragment Length Polymorphism (RFLP) led to the same results.2.The had no significant difference between HCV gene type and transmission route.The gene type distribution between acute and chronic hepatitis C had no significant difference.The persistence of HCV affection had no definite relationship with virus gene type.3.The virus level of gene type II was higher than that of gene type III and II/III.4.The different gene types had different responses to interferon.The gene type II had the worst response to interferon.5.The gene type II had much influence on fat metabolism.The patients...
