Neovascular Glaucoma Iris Neovascularization In Pathological Microenvironment

Purpose: To investigate the mechanism of formation and contraction of iris neovascular membrane in neovascular glaucoma (NVG) from the perspective of growth factors. , discussing the pathologic microenvironment of iris neovascular membrane.Methods: Patients were divided into 3 groups: Group A (NVG, 51 samples), Group B (ischmia central retina vein occlusion, CRVO, 7 samples) and Group C (cataract, 14 samples). The grade of ectropion uveae in Group A was divided into 3 subgroups. Enzyme-linked immunosorbent assay (ELISA) was used to quantitate the concentration of the cytokines TGF- β1, TGF- β2, VEGF and HGF. Samples from 6 NVG and 3 normal eyes were obtained. Immunohistochemistry recognizing TGF—β 1,2,3 were performed in the anterior segment and iris neovascular membrance.Results: Total TGF-β 1 (729.86 ± 471.68pg/ml), Total TGF-β 2 (9880.14 ± 6394.16pg/ml) were detected in aqueous humor samples of NVG, while VEGF in NVG samples was 8859.00 ± 3759.40pg/ml. HGF in NVG samples was 5703.00 ± 3677.19pg/ml. Ischemia CRVO aqueous humor contained 14.14 ± 14.42 pg/ml(TGF-β1), 1043.00 ± 542.95 pg/ml(TGF- β2), 1150.71 ± 1047.68 pg/ml(VEGF). TGF-β1, TGF- β2, VEGF and HGF concentration in cataract samples were 0pg/ml, 333.14 ± 222.16pg/ml, 12.60 ± 28.17pg/ml and 86.63 ± 72.60pg/ml. The concentration in NVG aqueous humor was significantly higher than those in ischemia CRVO and cataract (p<0.01). TGF—β 1,2,3 were detected in all NVG iris neovascular membrane. And NVG produced more TGF — β 1,2,3 in iris and ciliary body than those in normal eyes. In addition, inflammation cells appeared in anterior segment of NVG also produced TGF— β1,2,3.Conclusions: Patients with NVG had a significantly increased level of TGF— β1, TGF — β2, VEGF and HGF. Compared with normal eyes, the function of the iris and ciliary body , as well as infiltrating inflammatory cells, to produce TGF— β1,2,3 is strengthened in NVG. TGF — β are closely related with the formation and contraction of iris neovascular membrane. TGF —β 1 may play an important role in promoting the process of myofibroblast producing smooth muscle and contracting in iris neovascular membrance.
