Background Rheumatoid arthritis(RA) is a complex hetergenous autoimmune condition with unknown etiology.More and more evidence have suggested that innate immunity play an important role in RA,and toll like receptors(TLRs) might link innate immunity and adoptive immunity.TLR2 and TLR4 recognized a wide array of bacterial partial structures including lipoproteins, peptidoglycan, lipoteichoic acid and so on ,moreover,endogenous antigen such as HSP and fragments of hyaluronic acid fibronection, (FN) could also activite them which lead to release of proinflammatory cytokines.Is has been well documented that those endogenous antigen are abundantly present in inflamed synovial joints of RA.Objective To explore the role of toll like receptors-2 and toll like receptors-4 in the pathogenesis of rheumatoid arthritis(RA).Methods A case control study was designed to entroll 32 RA patients and 10 healthy controls,The expression rate and intensity of TLR2 and TLR4 on the CD 14 positive monocytes in peripheral blood (PBMC) were determined by muti-colore immunofluorescence flow cytometry.We used T test to compare the means and Person analysis to calculate the correlation degree.Results①The expression rate and intensity of TLR2 was 96.7 ± 2.4% and 3.38 ±0.39 respectively,significantly higher than that of normal controls.②The expression rate and intensity of TLR4 was 51.8±11.9% and2.22 ± 0.26respectively,significantly higher than that of normal controls.?The co- expression rate and intensity of TLR2 and TLR4 were 49.8 + 13.7%,significantly higher than that of normal controls.? There were no defference of the expression pattern between the early RA group and the later stage RA group;(5) The intensity of TLR2,but not TLR4 was correlated with serum CRP level, (r=0.527), P (0.01.Conclusion The results indicate that innate immunomoculars such as TLR2 and TLR4 might involved both in the initiation stage and perpetuating stage of RA.
