| Purpose: Evaluate the clinical effect of blood stasis type PHN scientific and syntheticly through clinical research with puncturing the disease. Give more guidelines to the manipulation of fire needles, promote the developing the fire needle and promote the development of qcupuncture.Method: 1. Divide the selected 68 cases into 2 groups, 34 cases for the fire needle group and 34 for the philiform needle group. Stop any other treatment for PHN 1 week before the experiment. 2. Therapeutic principle of the treatment group: soothe the muscular region and dredge collaterals, resolve blood stasis and relieve pain. Acupoints: A shi point, corresponding Jia Ji point. Manipulations: Let the patient stay in the comfortable position, sterilize the skin, heat the fire needle in the outer flame of lighted alcohol cotton and prick the points multiple-superficially, 1.0 cm or so from the other needle, 2~3 pricks for each points with the depth of 2~3mm. The pricking should be fast enough within 0.1second. After pricking, sterilize the needle hole and the around skin with 75% alcohol cotton, one treatment every other day. Observe the effect after 2 weeks. The therapeutic principle and acupoints of the control group is the same as the treatment group. The acupuncture of A shi points: Make sure of the location of pain, sterilize the skin with 75% alcohol cotton (or An Er Dian for patient with hypersensitive skin) and needle the points subcutaneously 0.8~1.0 cun with a 1.5 cun needle toward the centre. After the arrival of the qi response, perform fast even twisting method for 1 minute. The acupuncture of Jia Ji points: Make sure of the Jia Ji points according to the location of the pain. After sterilization, puncture the Jia Ji points obliquely toward the spinal column 1.0~1.2 cun with 1.5 cun needles. Stop the manipulation when there is distending or radiating sensation along the intercostal space. Treat once every other day, retain the needle for 30 minutes and manipulate the needle every 10 minutes. Observe the effect 2 weeks later.Result: Fire needle has marked effect for blood stasis type PHN which can obviously relieve the pain and promote the sleeping efficiency. The total effective rate is 100%, while curative and marked effective rate 85.3%. It is also effective for the cell parameters which substitutes for the blood stasis, especially for RDW (P<0.01) and PDW (P<0.05). The fire needle therapy is not superior to the philiform one in improving the VAS score and blood cell parameters, but it has the obvious superiority in instant relieving pain, P<0.05. Otherwise, we have also observed that fire needle therapy is effective for patient in all of the observed stage of life, and there is no difference among the different stages. There is also no difference among the difference square of pain in blood stasis type of PHN patients. The effect of fire needle for the disease course below 6 months is better than that of over 6 months, P<0.05. In all, fire needle is markedly effective in treating the blood stasis type of PHN. It can obviously relieve the pain of the patients, and improve the cell parameters especially the two parameters of RDW and PDW. In contrast with the philiform needle, fire needle is faster in relieving pain. Otherwise, fire needle is also good in low relapse rate, safety and no side effect, etc. It should be widely used in clinic.
