Objective: Ideal vermilion reconstruction should be accomplished with satisfactory outcome with respect to aesthetics and function. The buccal musculomucosal flap pedicled anteriorly is used for a wide vermilion and orbicularis oris muscle reconstruction with innervation of sensative nerve of the mucosa and motion nerve of the muscle in the flap.Methods: The buccal musculomucosal flap based on the anterior buccal branches of the facial artery and the branches of the infraorbital nerves is designed in a shuttle. The mucosal sensation and muscle motion in the flap were evaluated.Results: The flap was successfully used for wide vermilion and orbicularis oris reconstruction in 9 patients. Seven of 9 of the patients in this study had no postoperative complications. Two patients had some mucosal sloughing on the tip of the flap, but the underlying muscle remained viable and mucosalized normally. The sensation (the feeling of cold, heat and touch) of the mucosa in the flap exists was on the first postoperative day.
