Free muscle graft to reconstruct facial paralysis has been widely used in clinicin recent 10 years. Although the result is obvious, not a single method can restore all the deformity. The history review, anatomy and animal experiment about facial paralysis have been discussed in this article, so do the operation study.The anatomy is managed in the nerve and blood vessel distribution of temporalisand masseter to select proper temporal flap for transferring and masseter tunnel position,which help to reconstruct the deformity of eyes and oral commissure.The animal experiment is done to verify the free muscle surviving and nerve regeneration after being transplanted, rats are taken to receive free transplantation of EDB and EHB after being denerved. We make the muscle biopsy to do histollogical, hischemical and electromicro- scopical examinations at. different period, which prove the metabolism change and formation change. It has concluded that the muscle regenerates obviously after 4 weeks post-operatively, nerve control almost has consummated after 12 weeks post-operation. This give a good basic fondation to clinical work. At the same time we compared the survival of transplantated muscle denerved using traditional way and BTXA injection , the result is that the denerved muscle survival is more effective using BTXA injection than that of traditional way.In clinical operation study,we improve the method of Thompson, discuss the indication of operation, theory of muscle survival and nerve regeneration, suspending position and other advantages.The method to free transplanted muscle after being denerved 3 weeks stimulates the nerve to regenerate rapidly and possibly,shorten the contracture time of muscle and nerve.The muscle can survive before atrophy, and nerve can grow into graft...
