ObjectiveThe system inflamatory reaction will activate coagulation system,which cause coagulation,microcirculation disorder,and microthrombosis in sepsis.By using traditional medicine XUBIJING intervention,the reaserch approach the intervention and prophylactic effect of XUBIJING to the patho-changing in sepsis.The aims including:.1.To establish sepsis models in rats by CLP,observe the effect of the XUBIJING intervention to system inflamatory reaction and coagulation disorder in sepsis rats.2.In vivo in sepsis rats,observe the effect of the XUBIJING intervention to the alteration of mesentery microcirculation,the changing of capillary haemodynamics and microthrombosis.3.Survey the effect of the XUBIJING intervention to the intestinal mucosal barrier function and immune function.Methods1.150 female and male Sprague-Dawley rats(280-320g)were randomly divided into 3 groups:SEPSIS(A),XUBIJING(B)and SHAM(C).Every group 50 rats.A group:undertake CLP.C group:open abdomen,expose cecum and undo CLP.B group:done same to A group,after CLP,intraperitoneal injection XUBIJIN 4 ml/kg,one time per 12h,3d.A and C group,intraperitoneal injection the same volume physiological saline.Collect blood sample through arteria carotis and detect inflammatory factors and coagulation function after establishing models at once and injection XUBINING 12h,24h,48h,72h,then kill the rats. Every time point kill ten rats effectively.2.Before collecting samples at zhe same point,the mesentery near to the ileum was exteriorized and the microcirculation blood flow as well as PMN-endothelial cell interaction and microthrombosis was evaluated by means of intravital microscopy.3.After collecting samples at zhe same point,quickly collect one small intestinal samples 10cm part from cecum and fix by 10%formalin,then check by pathoanatomy.Slice after paraffin imbedding and observe intestinal mucoma by microscope.Detect intestinal mucoma Macrophage antibody by immunohistochemistry(Ed-1)。Results1 SHAM group rats coagulation function are normal and compare to SEPSIS and XUBIJING group,the differences are obvious.SEPSIS group rats appear coagulation disorder 24h after CLP,maintain high level 48h and 72h after CLP,difference is obvious;XUBIJING group rats appear coagulation disorder 24h after CLP,reach peak level 48h and 72h after CLP,compare to SEPSIS group the differences are obvious. 2 The levels of TNF,IL-1,IL-6 and IL-8 in serum of SHAM group rats are lower than those of SEPSIS and XUBIJING group,and the differences are obvious.The levels of TNF,IL-1,IL-6 and IL-8 in serum of SEPSIS group rats begin to step up 12h after CLP and to significantly higher levels 24h after CLP,keep high level 48h and 72h after CLP;XUBIJING group the levels of TNF,IL-1,IL-6,IL-8 same to SEPSIS group 12h after CLP,and reach peak 48h and 72h after CLP,the differences contrast to SEPSIS group are obvious;3 SEPSIS group rats mesentery microcirculation blood flow rate speed up 12h after CLP and slow down 24h after CLP and the number of PMN adhering to the wall of the blood vessel increase obviously.Stasis of blood stream appear 48h and 72h after CLP and blood flow rate slow down and down and microthrombosis,then blood flow rate begin to speed up.XUBIJING group rats mesentery microcirculation blood flow rate speed up 12h after CLP and slow down 24h after CLP and the number of PMN adhering to the wall of the blood vessel increase obviously.But compare to SEPSIS group,the differences are obvious.Contrast to SHAM group,these changes of SEPSIS and XUBIJING group,the differences are obvious.4 SEPSIS group rats intestinal tract mucous hyperemia and dropsy 12h after CLP,but intestinal tract mucous mostly are normal,I.e class 0 and class 1,class 2 and calss3 24h after CLP,class4 and class 5 48h and 72h after CLP;intestinal mucosa ED-1 express increase gradually,to the peak 24h after CLP,began to decrease 48h after CLP,and the differences are obvious.XUBIJING group rats intestinal tract mucous hyperemia and dropsy 12h after CLP,but intestinal tract mucous mostly are normal,I.e class 0 and class 1,class 1 and calss 2 24h after CLP,class3 and class 4 and class 5 by chance 48h and 72h after CLP,and contrast to SEPSIS group,zhe differences are obvious;intestinal mucosa ED-1 express increase more than SEPSIS group and the difference are obvious.Compare to SHAM group,the differences are obvious.Conclusion1.XUBIJING can inhibit the activation of inflammatory factors, lessen coagulation disorder and relieve sepsis.2.XUEBIJING can improve microcirculation function in sepsis and decrease microthrombus.3.XUBIJING can protect the intestinal tract mucosa mechanical and immunologic barrier disfunction in sepsis at some degree.
