Object:Chronic soft tissue injury is a kind of disease with the characteristic of chronic pain,hard to curing and easy recidivism.Because of the fast paced modern society,the incidence of chronic soft tissue injury is tending to increase in the past years.There are no very efficient ways(including Chinese medicine and west medicine)in healing this disease.Transverse process of third lumbar vertebra syndrome is one kind of the chronic soft tissue injury,and is popular in clinic.The low back pain resulting from this syndrome is severe and hard to cure, affects the normal work,degrades quality of life and spends a great quantity of medical resources.Acupotomy initiated by ZHU Han-zhang is a new Chinese medical treatment.It is under the guide of Chinese medical theory and also absorbs surgical intervention theory.It is a new invention result with intellectual property rights.Many clinic reports had confirmed that acupotomy dissolution had a special effect to treat the transverse process of third lumbar vertebra syndrome by disconnecting the conglutination and scraping the scar.It can decrease the patient's pain,increase their movement and regain dynamic equilibrium.The patient accepted this medical way for its low expense,predominant curative effect and convenient.In the past years,most attention had been paid on clinic treatment and few on the mechanism of acupotomy dissolution.In our study we has founded the model of the transverse process of third lumbar vertebra syndrome.We will investigate the analgesia effect of acupotomy dissolution and its mechanism through the model rat.Our research will provide a reliable evidence to illustrate the scientific nature of aupotomology,consummate the theory system of aupotomology and push the further development of aupotomology.Methods:1 The health male SD rats were randomly divided into four groups:normal group,model group,electroacupunture(EA)group and acupotomy dissolution group.The model in rats with the third lumber vertebrae transverse process syndrome was established by inserting absorbable gelatin sponge into the deep subfascial part of the 3rd lumbar transverse process.EA group and acupotomy dissolution group were used EA therapy and lysis of acupotomology respectively.The rats lived 28days and 56days after the operation.2 The analgesia effect of acupotomy dissolution in transverse process of third lumbar vertebra syndrome model rats was studied through recording the Tail flick latency(TFL)in radiation heat stimulation test.3 The effet of acupotomy dissolution on eliminating the partial inflammation and promoting muscle regeneration and repair in transverse process of third lumbar vertebra syndrome model rats were studied through observing the model rats' pathological change by the light microscope and the electron microscope.4 The effect of acupotomy dissolution on regulating the neurobiochemical metabolic disorder in transverse process of third lumbar vertebra syndrome model rats was studied through detecting the contents of SP,ENK,β-EP and CCK-8 in spinal cord and hypothalamus by ELISA.5 The effect of acupotomy dissolution on algesic transmission in transverse process of third lumbar vertebra syndrome model rats was studied through analysising the express and IOD of SP and ENK protein in spinal dorsal horn,hypothalarnus,hippocampus and cerebral cortex by immunohistochemistry method.Results:1 Between the 2end 14th day after the operation,comparied with normal group,the pain threshold values of last three groups decreases significantly(P<0.05).In the 28 and 56th day, pathology picture showed that the inflammatory cell infiltration,desmoplasia,and exomysium thickening in the muscle tissue.The ultrastructural organization with electron microscope showed myofilament fragmentation,mitochondria tumescence and collagen fibers increasing.The model was successful to simulate the chronic aseprtic inflammation whih was the clinical pathology characteristic of the syndrome. So the model can be used in the experiment to study the third lumber vertebrae transverse process syndrome.2 Acupotomy dissolution can prolong the TFL of rats whih were induced by the nociceptive stimulus.This effect can keep a long time.The TFL of rats in acupotomy dissolution group was longer than model group in 28th day and 56th day.3 Acupotomy dissolution can decrease the inflammatory cell infiltration in local tissue and increase the regeneration muscle fiber quantity.In addition,the proliferation collagenous fibre arranged orderly and paralleled with the muscle fibers.4 Acupotomy dissolution can cut down the heightened contents of SP,ENK,β-EP and CCK-8 in spinal cord and hypothalamus of the model rats(P<0.05,P<0.01).Acupotomy dissolution can regulate them to normal level.The results in 28th day and 56th day were same.5 Acupotomy dissolution can derease the heightened SP protein expression and IOD in spinal dorsal horn of the model rats.Accordly it can weaken the heightened ENK protein expression and IOD in spinal dorsal horn,hypothalamus,hippocampus and cerebral cortex of the model rats(P<0.05,P<0.01).The results in 28th day and 56th day were same.Conclusion:Acupotomy dissolution has the analgesia function and can have a long time effect. Acupotomy dissolution can promote the injury muscle's regeneration and repair,reduce the inflammation response in local tissue.Acupotomy dissolution can adjust the disorder of neuropeptide SP,ENK,β-EP,CCK-8 of the model rats.It can not only down regulation the abnormally increased contents of pain-producing substance(SP and CCK-8),inhibit the algesic transmission.But also reduce the high level analgesia material(ENK,β-EP)correspondingly,to avoid their adverse effect.Acupotomy dissolution can make the pain and analgesia tend to normal level,regulate the disorder of neuro-biochemistry level,help organism restore the balanced stable state.
