| Object:The chronic soft tissue injury is a big difficult problem with cancer and cardio,cerebrovascular disease as the three important difficult and complicated cases,and the dispute over of the mechanism is still existed. At present,the incidence of chronic soft tissue injury is tending to rises year by year.As a popular disease of the chronic soft tissue injury in clinical,the transverse process of third lumbar vertebra syndrome is the common reason to cause the low back pain that severe and hard to cure,and has a seriously influence on people's health and normal life quality.Acupotomy initiated by ZHU Han-zhang is a new Chinese medical treatment, which is under the guide of Chinese medical theory and also absorbs surgical intervention theory.NOW,it has developed to a new science with intellectual property rights.A lot of clinical reports confirm that,according to the acupotomy theory of chronic soft tissue injuries;acupotomy to treat the third lumbar vertebra syndrome has the advantage of rapid,short and efficient.However,the focus of the current research is still on the summary of clinical efficacy,but lack of in-depth study on the mechanism of acupotom's effect.In our study we investigate the analgesia effect of acupotomy dissolution and its mechanism by observing the change of the pain threshold,CCK-8 and NOS in the CNS of the model rat s of the transverse process of third lumbar vertebra syndrome after acupotomy,and provide a reliable evidence to support the function of acupotomy in the clinical,and illustrate the scientific nature of aupotomology,consummate the theory system of aupotomology and push the further development of aupotomology.Methods:1 The health male SD rats were randomly divided into four groups:normal group,model group,electroacupunture(EA) group and acupotomy dissolution group.The model in rats with the third lumber vertebrae transverse process syndrome was established by inserting absorbable gelatin sponge into the deep subfascial part of the 3rd lumbar transverse process.EA group and acupotomy dissolution group were used EA therapy and lysis of acupotomology respectively.The rats lived 28days and 56days after the operation.2 The analgesia effect of acupotomy dissolution in transverse process of third lumbar vertebra syndrome model rats was studied through recording the Tail flick latency(TFL) in radiation heat stimulation test by JZZ-light and heat pain measurement instrument.3 The effect of acupotomy dissolution on algesic transmission in transverse process of third lumbar vertebra syndrome model rats through analysising the express and IOD of CCK-8 and NOS in spinal dorsal horn, hypothalamus and cerebral cortex by immunohistochemistry method.Results:1 The 2nd day after the operation,comparied with normal group,the pain threshold values of three groups stimulated model are significantly lower than normal rats group(P<0.05),and there are no difference between these three groups(P>0.05).This change in pain threshold show that the model established is successful.2 After the treatment of Acupotomy dissolution(the sixteenth day),the value of pain threshold is increase,and keep for a long time.Both the the value of pain threshold of 28 days survival group and 56 days survival group has the tendon to increase.3 Acupotomy dissolution can decrease the expression of the CCK-8 in spinal dorsal horn,hypothalamus and cerebral cortex of the model rats(P<0.01,P<0.05),and there is no difference compared with the eletro-acupuncture group.The results in survived for 28 days and 56 days groups were same.4 Acupotomy dissolution can promote the NOS expression and IOD in spinal dorsal horn,hypothalamus and cerebral cortex of the model rats(P<0.01, P<.05),and there is no difference compared with the eletro-acupuncture group.The results in survived for 28 days and 56 days groups were same.Conclusion:1 The model of the third lumbar transverse process syndrome that established in our experiment can simulate the fundamental present symptoms from the Ethology and pain threshold,has some reference value.2 Acupotomy dissolution can elevate the pain threshold values and keep for a long time,which show that acupotomy has the function of analgesic like the acupuncture.3 Acupotomy dissolution can decrease the expression of the CCK-8 in CNS of the model rats,and perhaps acupotomy"s analgesic function is work through the inhabitation of the CCK-8 experission.4 Acupotomy dissolution can increase the expression of the NOS in CNS of the model rats,and perhaps acupotomy"s function analgesic is through the promotion of the NOS experission,but how it work in detail must to do more research. |
