| Objective To introduce 2 surgical approaches for radical esophagectomy and discuss advantages and disadvantages of each surgery.Methods Clinical data of 52 patients of left transthoracic incision and 1085 of transhiatal incision with squamous cell carcinoma of the lower esophagus, who underwent radical esophagectomy from Jan 2009 to July 2009, and from Jan 1978 to July 2009 in my hospital in Mongolia, were analyzed retrospectively. Compared was made on number of lymph node resected, regional lymph node metastasis ratio, perioperative mortality, operation time, bleeding volume, surgical comp lication rate, postoperative hosp ital stay between the 2 groups.Results The operation with left transthoracic incision has advantages in the perioperative mortality, operation time, bleeding volume, surgical comp lication rate and postoperative hospital days.Conclusion Left transthoracic incision is more effective on lymphadenectomy of superior mediastinum, whereas transhiatal incision showsmore effective on lymphadenectomy of abdominal region. |
