Clinical Study On Treatment Of Back Pain And Functional Rehabilitation With Chinese Herb Tongluo Shujing Yin In Post-operative Scoliosis

Obiective:To compare the clinical outcome of chinese herbs (Tong luo shu Jin Yin) versus lornoxicam in the treatment of back pain and functional rehabilitation after operation of scoliosis;To investigate the mechanism of chinese herbs and provide the basis of the treatment of back pian in postoperative scoliotic patients.Methods:From the Jun in 2007 to Apr in 2009,a consecutive series of 42 scoliotic patients were operated.Etiologies of the 42 deformites were 22 idiopathic scoliosis,14 congenital scoliosis,2 neuroscolisis,1 marfan's syndrome,1 traumatic scoliosis,1 cicatricial scoliosis and 1 midgestism scoliosis.The mean age at surgery was 16.76±3.484 years (from 8 to 24 years),8 males (19%) and 34 females (81%),6 cases (ASF),27 cases(PSF) and 9 cases (APSF) were undergone spinal fusion.The random and placebo control clinical expeimental methods were adopted according to the order of hospitalization in succession,42 cases after operation were divided into three groups:after operation of scolilsas in 24 hours western medicine group,14 cases were intervenously infused lornoxicam (16mg/d) until the 14th day; Chinese medicine group,14 cases were orally administrated Tong Luo Shu Jin,until the 14th day;Placebo Control group,14 cases were intervenously infused Vit C 1.0g/d Until 14th day. All cases were measured the degree of pain through visual pain scale (Visual analog scale,VAS) in 2d,3d,5d,7d,9d,14d,20d,investigated the information and incidence of gastrointestinal adverse reaction and the chang of hepative function of prior treatment and post-treatment.According to the Scale of RDQ and LBOS,the functional rehabilitation were evaluated in 20 days in all patients.Results:Clinical parameters of all the three groups (major curves of preoperation and postoperation,aptical vertebrae translation,coronal and sagittal balance,operation time,average operation blood loss,surgical methods,gender,age,fixed segment were no significant difference (P>0.05).The pain score measured by VAS was gradually degraded according to the time of postoperation.The rest score of VAS was quickly degraded in 3d after operation and the motion score of VAS was quickly degraded in 5d after operation.The pain score of Chinese Herbs group was lower than lornoxicam group and Vit C contorl group(p<0.001).The pain score of lornoxicam group was lower than placebo control group(p<0.001). The VAS of motion score of Chinese Herbs group was lower than lornoxicam group and Vit C contorl group(p<0.05).The score of LBOS and RDQ of Chinese Herbs group was 56.36±7.490,8.57±2.563, lornoxicam group was 33.07±7.098, 14.64±3.177; control group was 30.36±13.838,16.86±3.880. F1=28.794 and F2=24.367(P<0.05). The score of functional rehibilitation after operation of scoliosis in Chinese Herbs group was excellent and good,genaral in the lonoxicam group and in the placebo control group.The effect of medicine on the hepatic function in the three groups was no significant (p<0.05).The incidence of emergency uncler of lornoxicam group was higher than the other two groups (p<0.05),and the incidence of the other gastrointestinal adverse reaction was no siginificant(p>0.05).Conclusion:Chinese Herbs (Tong Luo Shu Jin Yin) can effectly treat the back pain of postoperative scoliosis,and help the functional rehabilitation.In contrast to lornoxicam,The incidence of gastrointestinal adverse reaction of Tong Luo Sun Jin Yin is lower.
