| Part1:Short-term Clinical Effect of Dynamic Cervical Implant in Treatment of Cervical SpondylosisObjective:To observe the short-term clinical effect of DCI in the treatment of cervical spondylopathy caused by lesions of the cervical intervertebral disc.Discuss its security and effectiveness.Methods:From02,2012to12,2013, in Torigji hospital Orthopedic,12cases with cervical discectomy and DCI implantation (a total of17intervertebral disc), use the imaging and clinical methods to evaluate the effect of surgery. To use neck VAS score, JOA score, NDI score, Odom rating to evaluate the clinical effect.To observe prosthesis position, Heterotopic Ossification. To measure the preoperative and postoperative Range of Motion of the operative and adjacent segment, intervertebral Disc Height, FSUCobb’s Angle and Overall cervical curvature (C2-7Cobb’ s Angle).To evaluate the clinical effect of patients using DCI.Results:All operations were successful, without the injury of spinal cord, nerve root and important vessels. The average operation time of single segment DCI was70min, double segments DCI was150min.The intraoperative blood loss was100ml in single segment,and was160ml in double segments.In the follow-up period JOA score increased significantly comparing with the preoperative period, the result was statistically significant (P<0.05).In postoperative period the VAS score and the NDI score decreased comparing with the preoperative period, the result was statistically significant (P<0.05). The ROM of DCI implanted segment increased, the difference was statistically significant (P<0.05). The intervertebral Disc Height of DCI implanted segment (DH) increased significantly, the difference was statistically significant (P<0.05).FSU Cobb’s Angle increased, the difference was statistically significant (P<0.05).Overall cervical curvature (C2~7cobb’s Angle) increased slightly, but there was no statistically significant difference (P>0.05).Conclusion:The short-term effect of applicating DCI to cure Cervical Spondylosis is satisfactory.The activity of the segment was retained, and the rate of Heterotopic Ossification was reduced.The degeneration of the small joint caused by the excessive movement is forbiden. The long-term curative effect needs to do more research. Part2:The short-term clinical curative effect of Dynamic Cervical Implant combined with ACDF in the treatment of Cervical SpondylosisObjective:To study the short-term clinical curative effect of Dynamic Cervical Implant combined with ACDF in the treatment of Cervical SpondylosisMethods:From May2012to November2013,10cases of patients with cervical spondylosis recived the hybird operation.3cases involved two intervertebral discs,7cases involved three intervertebral discs. Using JOA score, NDI score, VAS score, and Odom rating to evaluate the therapeutic effect. According to the preoperative and the postoperative cervical lateral X ray film to observe the effect of DCI.Results:To research10patients with cervical spondylosis.7patients were male, and3cases were female.Age was from33to58years old, with an average age of46.50years.All patients had successful operations, without the injury of the spinal cord, nerve root and important vessels.Postoperatie JOA score was significantly higher than preoperative JOA. Postoperatie NDI and VAS scores were significantly lower than preoperative score.Postoperative Odom ratings shew that the fine rate was90%. Postoperative radiographic follow-up of10cases shew that the postoperative cervical curvature was restored. There were not found that the Heterotopic Ossification around the DCI prosthesis. The fusion prosthesis were not found shift, the fusion results were satisfactory.Conclusion:DCI combined with ACDF can integrated the advantages of two kinds of technology.It could reach the thorough decompression at the same time avoid the complications of long segmental fusion. |
