| BackgroundAcne Vulgaris is a common chronic inflammatory skin disease in dermatology, which is generally related to the gonadal endocrine disorder by blocking hair follicles and sebaceous gland. It is a very popular clinical occurring disease, recently the incidence rate of adolescents and teenagers are getting higher to70-80%, and the number of adult acne patients is increasing.It is typically present on adolescents and teenagers on their facial skin, which is readily apparent to others and can cause harmful effects externally, affects the psychological aspect, social life and quality, or even worst can cause anxiety, depression and other psychological problems. However, the problems of anxiety and depression can also contribute to the occurrence of acne. Therefore, the investigation of the relationship between the occurrence of acne and acne patients’psychological condition and quality of life, has a significant meaning in studying the cause of acne, and its therapy and prevention.Purpose1. To understand acne patients in Guangzhou and Hong Kong of their psychological condition and quality of life.2. To observe the Integrated Zheyin Qingre Prescription TCM Treatment for the improvement of psychological condition and quality of life to acne patients.Method1. Select120qualified acne patients from Guangdong Provincial Hospital as observed patients, conduct a three-time observations in8weeks by using Integrated Zheyin Qingre Prescription TCM Treatment, and record acne patients’ personal data and other scoring index in different observed stages.Select patients need to fill up2sets of questionnaires (QoL-Acne and Cardiff Acne Index CADI) in each consultation observation period (1st week,4th week and8th week). And analyze reliability, validity and responsivity of the2sets of questionnaires used.Finally analyze the co-relation between the severity of acne to the psychological condition and quality of life, and acne patients’therapeutic improvement after the treatment and the improvement on their psychological condition and quality of life.2. Select60qualified acne patients from Hong Kong to conduct a one-time basis observation in general data and fill up2sets of questionnaires (QoL-Acne and Cardiff Acne Index CADI), and analyze Hong Kong acne patients’ psychological condition and quality of life.Compare the difference of Guangzhou acne patients and Hong Kong acne patients in their psychological condition and quality of life.Result1. After8weeks of Integrated Zheyin Qingre Prescription TCM Treatment, Guangzhou acne patients’scores of skin lesions decreased from52.72±22.5to13.97±7.06, the difference has statistical significance (P<0.05), without side effect, it proved to be an effective and safe treatment method to acne vulgaris.2. From120cases of observed Guangzhou acne patients,118cases (98.3%) had influences in their quality of life, acne patients who suffered from the aspects of Self-Perception, Role-Emotional and Role-Social were103cases (85.8%),117cases (97.5%) and88cases (73.3%) respectively. After8weeks of treatment, acne patients’scores on their quality of life increased from15.07±8.10to27.35±8.11, the differences had statistical significance (P<0.05), it has shown that acne patients have obvious improvement in their quality of life after the Integrated Zheyin Qingre Prescription TCM Treatment.3. The research data of psychological condition had shown that120cases of Guangzhou acne patients had different level influences of their psychological condition. Mild quality of life impairment21cases (17.5%), moderate impairment68cases (56.7%) and severe impairment31cases (25.8%) After8weeks of treatment, acne patients’scores of Cardiff Acne Disability Index (CADI) decreased from15.07±8.10to27.35±8.11, the differences had statistical significance (P<0.05), it had shown that acne patients also have obvious improvement in their psychological condition after the Integrated Zheyin Qingre Prescription TCM Treatment.4. From60cases of observed Hong Kong acne patients,35cases (58.3%) had influences from quality of life, acne patients who suffered from the aspects of Self-Perception, Role-Emotional and Role-Social are20cases(33.3%),34cases(56.7%)and13cases(21.7%)respectively. Totally50cases (83.33%) had influences from psychological condition, mild impairment h30cases(50%), moderate impairment20cases(33.3%) and severe impairment0case(0%). Score of quality of life (QoL-Acne)in first time consultation was29.38±9.28, has higher score when compared to Guangzhou acne patients (15.07±8.10); CADI score was4.03±2.88, lower than Guangzhou acne patients (8.53±2.96), it has shown that Hong Kong acne patients when compared to Guangzhou acne patients, had less influences neither in quality of life nor in psychological condition.5. Guangzhou acne patients had positive co-relation between their score of psychological condition and score of skin lesions. Guangzhou acne patients had negative co-relation between their score of quality of life and score of skin lesions, and Hong Kong acne patients had no obvious co-relation between their score of psychological condition/quality of life and score of skin lesions, it is possibly related to the complicated factors such as career, age and basic psychological condition of acne patients. But two cities had obvious negative co-relation between psychological condition and quality of life. It explained that to acne patients’psychological condition is an important factor to influence the quality of life.Conelusion1. Acne vulgaris had different degree of influences on acne patients’ psychological condition and quality of life. The Integrated Zheyin Qingre Prescription TCM Treatment can improve effectively acne patients’ psychological condition, and can improve obviously their quality of life.2. Both Chinese versions of Quality of Life Questionnaires (QoL-Acne) and Cardiff Acne Disability Index (CADI) had shown very good reliability, validity and responsivity. With some adjustments made which can be the measurement and assessment tools for evaluating psychological condition and quality of life, and become the valuable supplementary to the evaluation system of acne treatment results.3. Guangzhou and Hong Kong acne patients had varying degree of differences in severity of diseases, psychological condition and quality of life, it may be related to the different living environment and different living habits. |
