Objective:Gallstone is a worldwide common disease which have a strong impact on our health, the morbidity connected with racial inheritance, environment, internal secretion, metabolism, epidemiologic study show that obesity is one independent,high risk factor of cholesterol gallstone,but still lack of molecular mechanism research, possibly, may be adipokine is the crux factor of gallstone which cause by obesity.XINJIANG is the high ocurrance area of gallstone and obesity, years ago, our research team have been find there is regularity relevance between adiponectin and gallstone. Therefore, our study is based on the research past we have done, proceeding further research by gene expression regulation:(1)analyse the relationship between adiponectin gene polymorphism and expression level of adiponectin detected from omentum and subcutaneous fat which obtain from gallstone patient and non-gallstone patient of Uyghur and Han nationality.(2)Discuss the relationship between gene expressionion regulation and morbidity of cholesterol gallstone and obesity, and how genetic factor, race, environment effect the expression level of adiponectin mRNA and morbidity of gallstone, to provide clue and theoretical foundation for XINJIANG’s high morbidity molecular mechanism of gallstone. Methods:(1) Randomized controlled trials were applied for this research.Collect omentum and subcutaneous fat from gallstone patients and non-gallstone patients of Uyghur and Han nationality in people’s hospital of Xinjiang Uyghur Autonomous region. adopt RT-PCR to identify expression level of the adiponectin mRNA contain in each omentum and subcutaneous fat of different patients. Then discuss the difference of expression level of the adiponectin mRNA between uyghur and han nationality obesity and non obesity patients, and the relationship with gallstone.(2)adopt Sequenom MassARRAY SNP to quantitative analysis the mutation site of he adiponectin, than combine with result of serum adiponectin level determined by ELISA to estimate the relationship between mutation site ⅣS2+712A/G and serum adiponectin level, disscus the relevance between mutation site IVS2+712A/G and serum adiponectin level of gallstone, provide a molecular mechanism basis for high morbidity of gallstone. Results:the result of RT-PCR shows that whether the expression level of adiponectin mRNA detected from omentum or subcutaneous fat of Uyghur gallstone group is lower than the non-gallstone group, and the difference of expression level of adiponectin mRNA between two group have statistical significance (P<0.05).compare clinical index such as age,fasting GLU,CHO, TG,HDL-C,LDL-C and average expression level of adiponectin mRNA detected from omentum and subcutaneous fat between Uyghur obese and non-obese group,the fasting CHO and fasting TG of Uyghur obese group is magnificently increased (P<0.01), while the adiponectin mRNA expression level of greater omentum and subcutaneous fat is magnificently decreased (P<0.01). compare with non-obese group,despite of sex, adiponectin mRNA expression level detected from greater omentum is magnificently decreased, and the adiponectin mRNA expression level of subcutaneous fat from female obese patient is also decreased.pairwise comparison among four subgroup of han nationaily group,fasting GLU,CHO, TG of gallstone obese group are higher than the other two non-obese group, have statistical differences (P<0.05), may related to obese patient easily develop glucose and lipid metabolism disorder. Adiponectin mRNA level of greater omentum and subcutaneous of han nationality obese group are lower than the other two non-obese group, have statistical differences (P<0.05). Adiponectin mRNA level of greater omentum and subcutaneous of han nationality gallstone group are lower the non-gallstone group. but,only the Adiponectin mRNA level of subcutaneous of han nationality obese group is lower than non-obese group (P<0.05), the difference of Adiponectin mRNA level of greater omentum between obese and non-obese group have no statistical significance, Pa=0.05, different from Uyghur patient at this point, this may caused by deficiency of obese han nationality patient(Uyghur obese:non-obese=98:56, han nationality obese:non-obese=35:44).(2)the result of Sequenom MassARRAY mass spectroscopy shows that, among uyghur case group the percentage of AA, AG, GG genotype of gallstone group is30.2%,48.9%and20.9%, the percentage of AA, AG, GG genotype of non-gallstone group is30.2%,48.9%and20.9%, thus, AA, AG,GG genotype have no significant difference between this two group (x2=1.77,P=0.413).A and G allele frequency of gallstone group is52.4%and47.6%,but, A and G allele frequency of non-gallstone group is54.65%'45.35%, so have no significant difference between this two group (x2=0.123,P=0.798). among han nationality case group the percentage of AA, AG, GG genotype of gallstone group is33.3%、41.7%、25%,the percentage of AA, AG, GG genotype of non-gallstone group is37.5%、48.95%、17.5%. A and G allele frequency of gallstone group is54.15%and45.85%,A and G allele frequency of non-gallstone group is60%and40%, so have no significant difference between this two group (x1=0.437,P=0.266).(3)the result of ELISA shows that among Uyghur case group,serum adiponectin level of gallstone group is lower than non-gallstone group,have statistical significance (P<0.05);among han nationality, serum adiponectin level of gallstone group is lower than non-gallstone group,have statistical significance (P<0.05), the result indicate that decreasing of serum adipnectin level related to the development of gallstone. Conclusions:(1)The average expressionion level of adiponectin mRNA of XINJIANG’s Uyghur and Han nationality who suffer gallstone and obesity is lower than the nationlity who haven’t suffer gallstone and obesity, furthermore, have negative correlation with body weight; during the research, we find out the subcutaneous fat’s expressionion level of adiponectin mRNA is obviously higher than omentum majus’s. we still find out there is relevance between nationality,sex,BMI,the research result show that female of uyghur people is more easily gain weight and suffer cholesterol gallstone.(2) Possibly there is no relationship between polymorphism of adiponectin gene+712A/G and morbidity of gallstone in Uyghur and Han people who living in XINJIANG(3)The reducing of serum adiponectin level of XINJIANG’s Uyghur and Han nationality possibly related to the morbidity of gallstone.
