Objective: This study was aimed to investigate the changes of blood plasma adiponectin concentration in the obese and non-obese patients with newly diagnosed type 2 diabetes mellitus and to explore the relationship of blood plasma adiponectin with body mass index,waist hip ratio,body fat percentage,blood glucose,blood lipid and insulin resistance.Methods:1 Subjects: One hundred and one newly diagnosed type 2 diabetes mellitus patients, aged between 40 and 65, were chosen in this study. According to the standard of the Adult Body Mass Index in Asia,all the subjects with type 2 diabetes mellitus were divided into three groups randomly, which included into normal weight group(NWG), thirty-three type 2 diabetes mellitus patients, BMI18.5—22.9kg/m2, overweight group(OWG) ,thirty-six type 2 diabetes mellitus patients, BMI23—25kg/m2, and obesity group(OBG), thirty-two type 2 diabetes mellitus patients, BMI≥25kg/m2. And thirty-three healthy subjects, BMI18.5—22.9kg/m2, without diabetes'history was chosen for normal control group(NCG). 2 Methods: Height, weight, blood pressure(BP), waistline and hipline of all the subjects were measured. Plasma adiponectin was tested with Enzyme–linked immunosorbent assay(ELISA). Blood samples of subjects were examined for blood lipid(TC,TG,LDL–C,HDL–C,APOA,APOB),glycosylated hemoglobin(HbA1c),plasma adiponectin,fasting blood glucose(FBG) and insulin(FINS). Moreover, body mass index(BMI), waist hip ratio(WHR), body fat percentage(BF%) and home-stasis model assessment of insulin resistance index(HOMA–IR) were calculated. Those indexes were compared among various test groups and the correlation between plasma adiponectin and those indexes was further evaluated in the study.3 Statistical analysis: All data were analyzed by statistical software SPSS11.5 version. All date were analyzed firstly by normality test and homogeneity of variance test. Normal distributed data were presented as mean±standard deviation (SD). Abnormal data were analyzed after log-transformed. It applied paired independent-samples t-test to compare the change of variables between two groups. Analysis of variance (ANOVA) were used to compare the change of mulitiples of groups. The correlation among plasma adiponectin with blood lipid,body mass index,waist hip ratio,body fat percentage,insulin resistance,glycosylated hemoglobin,plasma glucose and insulin applied correlation and univariate and stepwise mulitiple linear regression analysis. A value of P<0.05 was considered to be statistical significance.Results:1 The plasma adiponectin was significantly lower in normal weight group,overweight group and obesity group with T2DM group than in normal control group(P<0.01).2 The plasma adiponectin was significantly higher in normal weight group than overweihgt group and obesity group with T2DM group(P<0.01). There was no significantly difference in plasma adiponectin with T2DM between over weight group and obesity group.3 There was significantly difference in plasma adiponectin between female group and male group. Not only normal control group but also T2DM group, The plasma adiponectin in female was significantly higher than in male(P<0.05).4 Blood plasma adiponectin level correlates negtively with BMI, WC, WHR, BF%, FBG, HOMA–IR, HbA1c and TG. Blood plasma adiponectin level correlates positively with HDL–C. After adjusting for WHR, partial correlation analysis showed that BMI was not correlated with plasma adiponectin. A stepwise multiple linear regression analysis showed that WHR, HDL, BF%, HOMA–IR enter the regression equation.Conclusion:1 The plasma adiponectin was significantly lower in type T2DM group and obesity with T2DM group than normal control group. The same as the plasma adiponectin was significantly difference in normal weight group than overweight group and obesity group.2 Plasma adiponectin correlates with gender. There was significantly difference in adiponectin between female group and male group.3 Blood plasma adiponectin level correlates negtively with BMI, WC, WHR, BF%, FBG, HOMA–IR, HbA1c and TG. Blood plasma adiponectin level correlates positively with HDL–C. The analysis showes that obesity and hyperglycemia are influencial factors of plasma adiponectin. And it indicates that plasma adiponectin is one kind of index which is used to appraise the level of insulin resistance.
