| With the change of the way of life, the number of patients with colon cancer in our country is continuous growing at an average annual rate of rapid growth of 4.2% per year,there were more than 170000 new cases of colon cancer were found every year,higher than the global average increasing speed.It has become a serious threat to our people the health as the fourth leading cause of death in all the malignant tumor. The diagnosis and treatment of colon cancer is not optimistic nowadays,early diagnosis rate is low,and survival period is short,so it is important to take effective measures to control the occurrence and development of tumor,and prolong the survival time of patients.In Western medicine,diagnosis and treatment of colon cancer are based on the guideline that is draw up by USA Joint Committee on cancer and the International Union against cancer. Surgery has been the important means,and the preferred solution,of western medicine treatment of colon cancer,and then adjuvant therapies,such as chemotherapy radiotherapy and targeted therapy were given after the operation.But there are lots of problems at present western medicine treatment of colon cancer, such as excessive and inadequate treatment, inadequate of multidisciplinary collaboration mechanism on the treatment,and the specific cases of specific period cannot be put forward reasonable and accepted diagnosis and treatment plan, a "blank" period of no diagnosis, treatment and clinical observation will appear. Traditional Chinese medicine plays an increasingly important role in treatment of colon cancer. This disease is deficiency in origin and excess in superficiality, integrity is the fundamental cause of the tumor. Therapeutic method can be adjusted according to the dynamic evolution law of syndromes,such as fortify the spleen and replenish qi,tonify and enrich blood,replenish qi and enrich yin,soothe the liver and regulate qi,clear heat and eliminate dampness,active blood and resolve stasis,resolve stasis and promoting defecation,expelling pathogen and detoxify,which can effectively improve the prognosis and quality of life of patients,whether in the perioperative phase, radiation and chemotherapy, end-stage of tumor.Professor Hua Baojin is engageding on clinical oncology more than 20 years.He is knowledgeable and eclecticism.He has formed his own unique diagnostic features,and he has accumulated rich clinical experience in preventing the recurrence and metastasis of colon cancer patients after surgery,reducing chemotherapy toxicity and increasing efficiency,prolonging life and improving the quality of life,etc.This topic is going to analyze the subjects of Professor Hua Baojin in treatment of colon cancer, and further summarize treatment experience,at the same time,to observe the effect of Banxia Xiexin Decoction "in the treatment of colon cancer using prospective clinical research method,and based on experiments to verify the clinical effect,which lays the foundation for the further transformation,in order to provide new ideas for clinical treatment of colon cancer.First, Summary of experienceObjective:To summarize the clinical experience of drug laws and Huabaojin professor of treatment of colon cancer, and provide new ideas for the treatment of colon cancer with integrated traditional Chinese and Western medicineMethods:The diagnosis and treatment of colon cancer cases were collected and the application of traditional Chinese medicine in the treatment of colon cancer patients with typical cases were summarized in this paper, to analyze the one’s own knack in how to use the theory of traditional Chinese medicine treatment of this disease; At the same time, sorting, analyzing the papers and books of the teachers and his students, finishing my experience of learning with teacher notes, clinical experience and knowledge.Results:The characteristics of Huabaojin professor on clinical diagnosis and treatment are following:(1) Adhering to the thought of "Preventive Treatment of Disease", promoting the health management mode, advocating the early diagnosis and treatment. (2) Regarding the tumor as a chronic disease, againsting the excessive treatment, puting forward surviving with tumor. Tonifying action by discharging method is the basic therapeutic principle;using the principel of soothing the liver and regulating Qi, adjusting the movement of Qi;practicing the Acrid-Release and Bitter-Downhearing Therapy,adjusting the lifting and falling movement of Qi;practicing the therapy of fortify the spleen and harmonize the stomach,operating the center of Qi.Conclusion:The treatment of integration of traditional Chinese and Western medicine therapy of Dr.Hua Baojin on the colon cancer has a remarkable effect, and the unique clinical feature is using the Rising and Falling of Qi Therapy on clinical diagnosis and treatment.Second, clinical researchObjective:To prove the clinical curative effect on the treatment of colon cancer of Decoction of Pinellia for Purging Stomach-fire, one of the representative prescriptions of Acrid-Release and Bitter-Downhearing Therapy.Methods:This clinical study choose the postoperation, cold-heat complex type of stage III colon cancer patients as the research objects. Using the nonrandomized concurrent control methods, these patients were divided into treatment group (TCM +chemotherapy group) and control group (chemotherapy group), Respectively, using the Decoction of Pinellia for Purging Stomach-fire Combined with western medicine and simple Western medicine chemotherapy.To observe the efficacy indicators include:the improvement of symptoms, quality of life score, tumor marker (CEA), body weight; late indicators include:the chemotherapy completion rate, recurrence time, location, disease free survival, survival; including the safety index:the adverse reaction of renal and hematopoietic system evaluationResults:1 In the treatment group, the total efficiency of the syndrome of cold-heat complex type is 94.3% and total efficiency of the control group the Western medicine group is 62.2%, the difference between the two groups has statistical significance (P< 0.05). The quality of life score of the treatment group decreased only 5.7%, the quality of life score decreased the proportion reached 40.5% in the control group, and the difference has statistical significance (P< 0.05).2 The study of folfox4 and mfolfox6 scheme for colon cancer treatment are on moderate intensity, the timely completion of the treatment group (88.6%) is higher than control group (67.6%), the difference has statistical significance (P< 0.05).3 Common adverse reaction of chemotherapy were bone marrow suppression, liver and kidney dysfunction, the difference was statistically significant in comparison with the control group, there was no statistically significant differences in red blood cell and platelet, renal function, CEA and other indicators of Decoction of Pinellia for Purging Stomach-fire to improve the tolerance of the adverse reaction of chemotherapy patients, alleviate the impact of bone marrow depression and liver damage with Decoction of Pinellia for Purging Stomach-fire on immune function, and may be related to upper regulation.4 The treatment group of patients was statistically significant changes in body weight compared with the control group.31.4% patients from the treatment group loss the weight, and only 67.6% in the control group. There was significant difference between those two groups (P>0.05).5 Although some patients did not appear endpoint at the deadline (December 31, 2014), and there will be some censored data, but the treatment group patients were disease-free survival compared with the control group the difference has statistical significance. Because of the medication time and the observation time is short; more positive results have not yet been aquared.Conclusion:Cold-heat complex symptoms can be effectively improved by using the Decoction of Pinellia for Purging Stomach-fire combining with chemotherapy among the colorectal cancer patients with syndrome of cold-heat complex type, at the same time, patients can keep stable weight, live a better life, achieved the chemotherapy plan successfully, and it also can protect the Ieukopenia and liver function from the injury of chemotherapy, prolong the disease-free survival period.Third, the basic researchObjective:Verify the effect of Decoction of Pinellia for Purging Stomach-fire on proliferation and apoptosis of colon cancer cells, and provide an experimental basis for the effective treatment of colon cancer with Decoction of Pinellia for Purging Stomach-fire.Methods:Preparation of Decoction of Pinellia for Purging Stomach-fire containing serum, which were designed to blank control group, Chinese medicine group (Decoction of Pinellia for Purging Stomach-fire group),5-Fu group and Chinese medicine group +5-Fu, the effect on human colon cancer HCT116 were evaluated with MTT for the proliferation of tumor cells, flow cytometry detection for the apoptosis. Western Blot for the detection of BCL-2 and BAX protein expression.Results:1 The MTT experimental results show that, compared with the control group, each group containing serum can inhibit the proliferation of HCT116 cells, they were significantly different (P<0.05), and the effect is time-dependent, the inhibition rate of each group containing serum of Decoction of Pinellia for Purging Stomach-fire +5-Fu>5-Fu> Decoction of Pinellia for Purging Stomach-fire.2 Flow cytometry results showed that, compared with the control group, the experimental groups containing serum can induce apoptosis of HCT116 cells in different degrees (P＜0.05). The Decoction of Pinellia for Purging Stomach-fire group, the apoptosis rate was 14.93%, compared with the control group, the difference was statistically significant (P<0.05); the apoptosis rate of 5-Fu group was 23.45%, compared with the control group, the difference was statistically significant (P＜0.01); Decoction of Pinellia for Purging Stomach-fire+5-Fu group, the apoptosis rate was 23.45%, compared with the control group, the difference was statistically significant (P＜0.001).3 Western Blot experimental results show that, compared with the control group, the experimental groups containing serum could inhibit BCL-2 protein expression and increase BAX protein expression, the difference was statistically significant (P＜0.05).Conclusion:Decoction of Pinellia for Purging Stomach-fire containing serum can inhibit the proliferation of HCT116 cells, and the combination of 5-Fu and Decoction of Pinellia for Purging Stomach-fire can significantly inhibit the proliferation of HCT116 cells; Decoction of Pinellia for Purging Stomach-fire containing serum can induce the apoptosis of HCT116 cells,the effective rate comes to a head when it combines with 5-Fu.Decoction of Pinellia for Purging Stomach-fire group can inhibit the expression of BCL-2 protein and its combination with 5-Fu showed a synergistic effect, which can increase the expression of BAX protein and the combination of Decoction of Pinellia for Purging Stomach-fire and 5-Fu also showed the best effect. |
