Spontaneous coronary artery Dissection (SCAD) is a rare and lethal myocardial ischemic event, which usually causes acute coronary syndrome and sudden death. Mostly, it happens more in female than male and the ratio is2:1.Those females are often lacking classic coronary disease risk factors. Depending on the formation of the dissection, spontaneous coronary artery dissection can be devided into intimal tear type(related with atherosclerosis) or intraluminal hemorrhage type(related with non-atherosclerosi). The clinical presentation depends on the location, extent, and rate of progression of the dissection.It can present as unstable angina, acute myocardial infarction, ventricular arrhythmias, sudden cardiac death or asymptom. The diagnosis of SCAD is typically made on coronary angiography, biomarker for acute myocardial infarction and clinical presentation. Treatment of SCAD is often challenging and may include medical therapy, PCI, or coronary artery bypass graft surgery, which will be choosed by the range of.dissection. Here, I choosed a young woman (between30yr to50yr) who has no history of pregnancy recently, heart disease(including atherosclerosis) or chest trauma. The only inducement is the death of her father (mental factor). In this case, we used CAG, IUVS and PCI to diagnose Spontaneous coronary artery Dissection, and cure this patient with stents. This patient had a very good prognosis and survived at1-year follow-up. From the case, we found out that when an acute coronary syndrome is identified in an atypical patient,particularly one who is young, female, lacking classic coronary disease risk factors SCAD,should be considered.
