ObjectiveWe used the model rats with insulin resistance induced by high-fat diet as objects and pioglitazone as control, systematically observed acupuncture’s effect of adiponectin, FFA in serum and AMPK, ACC in liver to explore effect of acupuncture on regulating lipid metabolism disorder in insulin resistance and improving insulin sensitivity at multi-level.Methods52 male SPF SD rats were fed normal diet for 5 days for adaptation, and randomly divided into two groups, blank group(n=10) and continuing normal diet feeding, model group(n=42) with a high-fat diet feeding 3 months later. From the eighth week, FPG and FINS were tested every week. ISI and HOMA-IR were calculated. When compared the model group with the blank group, the ISI and HOMA-IR had statistically significant (P<0.05), hyperinsulinaemic euglycaemic clamp would operated to confirm the insulin resistance model. When compared the model group with the blank group, the glucose infusion rate(GIR) had statistically significant (P<0.05). After 12 weeks, the model was success. The model group was randomly divided into three groups, model group(n=11), medicine group(n=12), acupuncture group(n=13). Blank group continue normal diet while model group continue high-fat diet for two weeks. Acupuncture group was treated with electroacupuncture and continue high-fat diet for two weeks. The acupuncture was applied to bilateral Fenglong and Sanyinjiao once a day. Medicine group was treated with pioglitazone (10mg/kg) once a day. After treatment, all rats were drew blood to test FPG, FINS, ADP, FFA. Livers of rats were cut off to test AMPK, ACC. The tissues of liver were observed by transmission electron microscope.Results1.Insulin resistance model was built successfully after 12 weeks. As compared with the blank group, GIR of the model group decreased significantly (P<0.01). After treatment, comparing with the model group, GIR of acupuncture group and medicine group increased significantly (P<0.01).2. Electron microscope revealed that stem cells organelles structures were in good condition in blank group. Both mitochondria and rough endoplasmic reticulum were good, no lipid droplets and degranulation in the cytoplasm, the.structure of the mitochondrial membrane and crest were good, either of rough endoplasmic reticulum. Mitochondria from rats in the Model group were enlarged and swollen, and showed morphological changes, including larger lipid droplet, and mitochondrial membrane is unclear, even had vacuolar degeneration. The cristae on the inner membrane of mitochondria were almost disappeared. The liver HE staining of rat infected with western medicine and acupuncture shows less cell volume and fat droplet accumulation.3. As compared with the blank group, serum ADP decreased significantly (P<0.01), serum FFA increased significantly in model group (P<0.01).After treatment, comparing with the model group, serum ADP increased significantly (P<0.01) and serum FFA decreased significantly (P<0.01) in acupuncture and medicine group.4. As compared with the blank group, the expression of AMPK decreased significantly (P<0.01), the expression of ACC increased significantly in model group(P<0.01). After treatment, comparing with the model group, the expression of AMPK increased significantly (P<0.01, P<0.05) and the expression of ACC decreased significantly (P<0.01) in medicine and acupuncture group.Acupuncture may increase APN to regulate the signaling pathways to increase the expression of AMPK and decrease the expression of ACC to mediate lipid metabolic disorder and improve insulin resistance which may be the mechanism of acupuncture to treat insulin resistance.
