ObjectiveThis paper is aimed to summarize the study of Acupuncture of "dredging the governor meridian and nourishing the spirit, and drawing qi to its origin" (It’s called Tong Yuan Acupuncture in brief), which is created by Prof. Lai’s clinical experience over 40 years. To set up a preliminary frame system of the study of Tong Yuan Acupuncture by induction and analysis, summary and refinement, and systematic arrangement on the basis of collecting Prof. Lai’s literature and data thoroughly. To conduct clinical researches on Primary Dysmenorrhea, a disease that Tong Yuan Acupuncure is expert in with the help of modern scientific research methods. And to observe its effectiveness and security.Methods1.With the help of notes, records, videos and other technical means, collect original data through following clinics, attending lectures, learning classic theories, collecting clinical cases, tutorial explanation, analysis, demonstration and other ways. Systematically induce and summarize the clinical experience and the academic theory of Tong Yuan Acupuncture created by Prof. Lai, and set up a preliminary frame system of the study of Lai’s Tong Yuan Acupuncture.2.Divide 60 patients with primary dysmenorrhea who meet inclusion criteria into treatment group and control group at random. Keep 30 cases in each group. One group use Tong Yuan Acupuncture in treatment (the Tong Yuan Acupuncture group). Select Guanyuan, Qihai, Guilai, Tianshu, Yintang, the hand three-needle, the leg three-needle as main points, and the luo-connecting point combined with the xi-cleft point as adjunct points:(1) Select Ligou and Zhongdu in the liver meridian; (2) Select Gongsun and Diji in the spleen meridian; (3) Select Dazhong and Shuiquan in the kidney meridian, the five organs acupoints (both sides) in accordance with signs, plus Shenmen, Laogong, Xingjian, Zhongfeng, Baihui etc. Another group use acupuncture prescription in control (the routine acupuncture group). Select Shimen, Yinjiao, Daheng, Zigong, Shousanli, Neiguan, Hegu, Sanyinjiao, Xingjian, Xuehai as main points. The adjunct points and selection of points according to signs are same with those in treatment. Start the treatment 14 days before the patients’ menstrual period. And Finish it one day before the period ends. Treat the patients once every two days,6 times in total. One menstrual period as a course,3 courses in all. Use VAS pain rating scale combined with menstrual symptom scale to evaluate the efficacy of this research. The VAS pain rating scale is mainly used to evaluate the efficacy of acupuncture analgesia. And the menstrual symptom scale is mainly used to evaluate the improvement of symptom and recovery of illness. Conduct the effect evaluation before the therapy, right after the first course, after the second one, and the third one. Input the evaluation results in a statistical software to do some relevant analysis. Mark the efficacy of dysmenorrhea therapy after the first treatment as instant efficacy, and that after all the treatments as clinical efficacy.Resultsl.Has preliminarily formed the concept of Lai’s Tong Yuan Acupuncture. Lai’s Tong Yuan Acupuncture is the short form of Acupuncture of "dredging the governor meridian and nourishing the spirit, and drawing qi to its origin" which is created by Prof. Lai. This method seeks cause from sign, determines treatment according to cause, takes spirit and mind as foundation, regards zang and fu organs as core, considers ren and governor meridians as superclass, views yin and yang regulation as general principle. It emphasizes on the close links of principle-method-recipe-acupoint-technique. It contains Lai’s special academic theory of strengthening body resistance and eliminating evil, regulating spirit and mind generally, reinforcing and reducing moderately, and treating disease from the root.2.Has summarized the following theoretical basis of Lai’s Tong Yuan Acupuncture. Vitality is the foundation of the whole body. It plays an important role in the genesis, development, recovery and prognosis of disease. Select channel from sign. Select acupoint along channel. Acupuncture should exert the advantage of acupoints and special acupoints. Acupuncture should establish holism and yin-yang concept. The key to acupuncture is regulating spirit, and the crux to spirit regulation is activating vitality.3.Has preliminarily enriched the connotation of Lai’s Tong Yuan Acupuncture. It has been summarized in five aspects:principle, method, recipe, acupoint, and technique. As to the principle aspect, Tong Yuan Acupuncture stands for the TCM theory of nature-human integration and yin-yang combination; As to the method aspect, Prof. Lai believes that Acupuncture happens to have the same view with Taoism, especially when it refers to the ren, governor, and eight extra meridians. As to the recipe aspect, Tong Yuan Acupuncture follows the idea that acupuncture treats disease from the root. It is aimed at regulating yin and yang, so it takes ren and governor meridians as general principle, and considers five transport points and point selection as support; As to the acupoint aspect, Tong Yuan Acupuncture still selects the acupoints, but it prefers those of the two meridians with the exterior-interior relationship. The ren and governor meridians can be regarded as the two meridians with the exterior-interior relationship besides the three-yin and three-yang meridians. Tong Yuan Acupuncture has the effect to " unite yin with yang and integrate the five organs (integrate the five elements) ", which is also one of the theory evidences of Tong Yuan Acupuncture’s point selection. As to the technique aspect, Tong Yuan Acupuncture underlines the arrival of qi when reinforcing and reducing in addition to spirit treating and vitality activating. So Prof. Lai has established the Mind and Spirit Theory, which is the theoretical foundation of Tong Yuan Acupuncture. He has elaborated on it from the boby-feel-amount-limit aspects.4.Has Summed up the clinical application rules of Tong Yuan Acupuncture, including drawing qi to its origin, yang guiding yin, giving priority to supplement, strengthening body resistance to eliminate evil, emphasis on blooming the spirit when activating vitality, combination and oneness of acupuncture and medicines.5.Has summarized Tong Yuan Acupuncture’s commonly-used acupuncture and traditional Chinese medicine prescriptions of 14 diseases:infertility, apoplectic sequela, chronic fatigue syndrome, vascular dementia, insomnia, vascular headache, chronic gastritis, supranuclear paralysis, cervical and lumbar spondylosis, menopathy, miscellaneous internal damage diseases, infantile cerebral palsy, chronic urticarial, allergic rhinitis.6.Has summarized Lai’s Tong Yuan Acupuncture’s academic features and proposed development forecast. Tong Yuan Acupuncture is good at reinforcing and reducing. It takes mind and spirit as the foremost. But it has its own academic features in application. Tong Yuan Acupuncture is main-symptoms oriented while it should be combined with syndrome differentiation. Since Tong Yuan Acupuncture lays stress on dredging governor meridian, regulating spirit, ren and governor meridians, and adjusting yin and yang, it makes full use of affected-channel treatment’s advantages and acupoints’clinical effects. The principles of formulation are very strict, which reflects the central application value of acupuncture’s principle, method, recipe, acupoint and technique. In clinical application, it underlines the close link with acupuncture’s principle, method, recipe, acupoint and technique. It has enlightened the clinical acupuncture formulation profoundly at home and abroad. It has also provided a brand new acupuncture treatment system. It’s repeatable and easy to operate. Thus it can be repeated by the high-levels in the same industry in domestic and foreign. It has a demonstrative effect on Guangdong province and the whole country to conduct it in various hospitals and regions by use of the research project. The research findings can be used in the acupuncture and rehabilitation departments in various hospitals in Guangdong, and the similar clinics in Hongkong, Macao, Taiwan and occident. Popularize Tong Yuan Acupuncture and strengthen Chinese acupuncture to lay a solid foundation of domestic multi-center and large-sample acupuncture experiments.7.Has generalized the academic theory and clinical experience of Lai’s Tong Yuan Acupuncture as eight methods:differentiating channels and organs, clearing heat and depleting, warming meridians to activate yang, dredging meridians and channels, strengthening body resistance and fostering vigor, regulating spirit to restrain pain, dredging governor meridian to dispel the wind, tonifying deficiency and relieving collaterals.8.The research findings of Tong Yuan Acupuncture in treating primary dysmenorrhea are as follows:The two groups have no difference in age, course and seriousness of disease before treatment (P>0.05). It illustrates that the two groups’ data after treatment are comparable.After the first, second and third course of treatment, collect three evaluation time points’ scale data in comparison to the data before treatment. And compare the second course with the first, the third with the second. All the results show that there’s statistical difference (P<0.05). It tells that both treatment have clinical efficacy of improving pain. Comparison between groups:as to the VAS and menstrual symptom scale, there’s no difference before treatment and after the first course(P>0.05),while there is difference after the second and the third course (P<0.05)After the first course, compare the groups in instant efficacy and the efficacy on different degrees of dysmenorrhea. All the results show that there’s no statistical difference (P>0.05)After the third course, compare the groups in instant efficacy and the efficacy on different degrees of dysmenorrhea. The results show that there’s significant difference (P<0.01). As to the aspect of efficacy on different degrees of dysmenorrhea:there’s no statistical difference between two groups in the efficacy on mild dysmenorrhea (P>0.05).There’s significant difference between two groups in the efficacy on medium dysmenorrhea (P<0.01). There’s statistical difference between two groups in the efficacy on severe dysmenorrhea (P<0.05)Conclusion1.Lai’s Tong Yuan Acupuncture is the fruit of his clinical, researching and teaching experience over 40 years. The creation and proposal is not a coincidence. It’s the ultimate reflection of his profound theoretical basis of traditional Chinese medicine and cultural background in acupuncture. Lai’s Tong Yuan Acupuncture is composed of clear theory, solid foundation, rich connotation, strict and explicit clinical application principles. It has preliminarily formed the formulation system of common diseases treated with Tong Yuan Acupuncture. It has vivid academic character, especially the way to relieve pain. With the significant effect, its developing foreground is vast.2.There’s significant efficacy of Tong Yuan Acupuncture on primary dysmenorrhea. No obvious adverse reactions have been found. It’s worthy of wide clinical popularization.
