[Objective]:To investigate the protective effects and mechanism of Qing Re Hua Yu Prescription-â…¡on acute cerebral ischemic injury (ACI).[Methods]:Including two parts of clinic study and experiment study.1. In the part of clinic study, Seventy-two case of Acute Cerebral infarction (ACI) chosed by strict standard of diagnosis and inclusion were divided into two groups, the treatment group and control group by the principle of randamization, blindness, comparison. The treatment group was treated with Qing Re Hua Yu Prescription-â…¡, and control group was treated by TianDanTongLuo Capsules,14 days was one course of treatment, one course was used in succession for both two groups. Before and after therapy, ICAM-1, CD62P, WBC, Platelet in blood and S100B protein in serum were examined by the methods with FCM and ELISA. At the same time, neurological defective mark and the score of symptom of chinese medicine were done.2. In the part of experiment study, (1)Experiment one:Adopt the models of cerebral ischemia-reperfusion (CIR), The rats with CIR were randamly divided into six groups, including sham-operated group, CIR model group, control group and treated group with different dosage of QRHY Prescription-â…¡. The changs of following parameters including the contents with tumor necrosis factor-α(TNF-α), interleukin-8 (IL-8) and pathological changes in field CA1 of hippocampus were observed respectively at reperfusion 3h,6h,12h,24h,48h after ischemia 2h. Meanwhile, the water in brain tissue was detected and ICAM-1, L-selectin in blood were examined by FCM at those time points respectively. (2)Experiment Two:Adopt the models with CIR, The rats with CIR were randamly divided into severn groups, including blank group, sham-operated group, CIR model group, control group and treated group with different dosage of QRHY Prescription-â…¡. The protein changs NF-KBp65 and IκBαexpressed in cerebral ischemia tissue were detected by western-blot technology respectively at reperfusion 3h,6h,12h,24h, 48h after ischemia 2h.[Results]:1. Clinical study:(1) The total effective rate of treated CIR(88.24%) and the symptom of traditional chinese medicine (91.18%) in treatment group were markedly higher than that of the control group(74.28% and 71.43%). (2) After treating, ICAM-1, CD62P, S100B and the count of WBC and Platelet in treatment group were markedly decreased than that of the control group (P< 0.01~0.05). (3) Tthe mark of neurological defective and the score of symptom with chinese medicine were markedly improved than that of the control group (P<0.01~0.05).2. Experiment study:(1) Experiment one:â‘ The neurone degeneration and lightly necrosis was occurred in field CA1 of rats hippocampus pretreated by drugs, but the damage was much serious in the model group.â‘¡QRHY Prescription-â…¡with middle and high dose decreased the contents of IL-8, TNF-α, ICAM-1, CD62L and the water in brain tissue were much obviously than those of model group and control group (P<0.01~0.05). (2)Experiment two:â‘ Compared with sham-operated group and blank group, IκBαexpressed is much lower in model group But in the groups pretreated with QRHY Prescription-â…¡is not obviously become lower.â‘¡NF-κBp65 expressed was much higher in model group than that in sham-operated group and blank group at the same reperfusion time point. QRHY Prescription-â…¡decreased the contents of NF-KBp65 is obviously than control group.[Conclusions]:1. QRHY Prescription-â…¡can obviously improve the extent of nervous function defect on acute ischemic stroke and the score of symptom of chinese medicine. The main mechanism may be that it eliminates inflammation by notably restrain the degradation of IκBαafter ICR and reduce the liberation of NF-KBp65. Consequently, decreasing the transcription and expression of NF-KBp65’s key target-factor as cytokine(IL-8, TNF-α) and adhesion molecule(ICAM-1, CD62L), abating phlogosis.2. QRHY Prescription-â…¡break down the vicious circle of ischemiaâ†'inflammationâ†'thrombusâ†'ischemia by blocking the vious chain-reacting of haemostasisâ†'phlegmâ†'pyretic-toxicityâ†'thrombus. 3. Haemostasis, Phlegm and Pyretic-toxicity are all have intimate connection with inflammation or thrombus, So we should take into account the pathogenesis of multiple factors, intervene it by multiple ways, achieving the regulation and control in multiple links in Actue Cerebral Ischemia.
