Objective:Analyse the treatment effect, complications, survival and clinical impact factors of advanced pancreatic patients who were performed High-Intensity Focused Ultrasound therapy, evaluate the safety and value of low power, long time, heat accumulated HIFU treatment to these patients.Methods:From January2008to September2012,58patients from the Second Affiliated Hospital of Zhejiang University School of Medicine Oncology Centre were included; use the FEP-BY02HIFU system to perform low power, long time, and heat accumulated HIFU treatment. Observe the change of biochemical factors before and after the treatment, adverse reaction, evaluate the response of tumour to treatment after3months, and do the follow up of each patient to analyse the survival of them. Results:Within all the58cases,5.00%(2/40) patients show serum amylase elevated,22.50%(9/40) show creatine kinase elevated,80.00%(32/40) show C-reaction protein elevated,40.54%(15/37) show tumour marker CA19-9elevated,24.14%(14/56) show transient fever,67.24%(39/58) show transient abdominal pain,8.62%(5/58) show mild skin burn,24.14%(14/58) appear ultrasound pathway subcutaneous induration,27.50%(11/40) appear mild low WBC, no one get any severe complications. For the clinical effect,3.45%(2/58) patients are evaluated as PR,17.24%(11/58) are PD,74.14%(44/58) are SD,5.17%(3/58) died within3months after treatment, so there are no data to evaluate the effect. The medium follow up time for alive patients is8months (1-27months).41patients died during follow up, the medium survival time is9months (95%CI:7.752-10.248),1-year overall survival is26.0%,2-year overall survival is1.72%,17patients are alive at the end of the research. The tumour stage and palliative treatment have significant effect to the survival.Conclusion:HIFU technology is a kind of effective and safe treatment for pancreatic cancer, it could control the growth of tumour, delay the progress of tumour, prolong the survival time of patients, while the adverse reactions are relatively mild that patients could tolerate, so HIFU treatment could be routinely used in treatment of pancreatic cancer patients.
