Part One:ERCC1Protein Expression and Correlation With Survivin and Ki-67Protein Expression in Esophageal Squamous Cell CarcinomaOBJECTIVE:Occurrence and development of tumor is a complex pathological processes with variety of cellular gene expression changes and cumulative interaction, the change of DNA repair genes is also an important part. ERCC1gene (excision repair cross complementation group1) encodes a single-stranded DNA endonuclease, the enzyme is a key role in nucleotide excision repair (NER) pathway. ERCC1mRNA and protein expression levels of genes may reflect the DNA repair activity of NER pathway,ERCCl is closely related to tumor occurrence and development, the effect of chemotherapy and prognosis.The aim of this study was to investigate the differences of ERCC1protein expression between esophageal squamous cell carcinoma(ESCC) and adjacent normal esophageal mucosa,the correlation of ERCC1expression with clinicopathological parameters,Survivin and Ki-67expression.METHOD:Tumor specimens were collected from70patients with ESCC who underwent complete resection. Forty-one cases of normal squamous mucosa were included for comparison parameters.We analyzed the ERCC1,Survivin and Ki-67expression levels in ESCC by Immunohistochemistry.The expressions of ERCC1were observed and correlated with size of tumor,tumor cell differentiation,invasion depth, lymph node metastasis,clinical stage.The correlation of ERCCl expression and Survivin,Ki-67expression were investigated. The prognostic significance of ERCC1was analysed by Cox regressionRESULTS:ERCC1expression was found in65.7%(46/70) of the ESCC patients in test group, and9.8%(4/41)positive expression was found in the normal squamous epithelium in control group (P<0.01).Survivin expression was found in60%(42/70) of the ESCC patients in test group, and19.5%(8/41)positive expression was found in the normal squamous epithelium in control group (P<0.01).ERCC1expression was not associated with tumor size,depth of invasion,differentiation,lymph node metastasis and clinical stage(P>0.05). There was a positive correlation between the expression of ERCC1and Survivn (r=0.40,p<0.01) but no positive correlation between the expression of ERCC1and Ki-67(r=0.062,P>0.05).Radio-chemotherapy was significant predictors of Survival in esophageal squamous cell carcinoma (hazard ratio:0.379,95%CI:0.15-0.955, P<0.05. Expression of ERCC1-negative patients with an average survival time of31months, the average Survival time of patients with positive expression of25months, ERCC1is not an independent prognostic factor (P>0.05).CONCLUSION:The over-expression of ERCC1was one of the important phenotypes and characteristics in ESCC carcinomatous change, but was not significantly associated with invasion depth, tumor differentiation degrees, lymph node metastasis and clinical stage in ESCC.ERCC1may play a role in apoptosis inhibition during the development of esophageal squamous cell carcinoma, it was not a prognostic factor in resected esophageal squamous cell carcinoma patient.Part Two:ERCC1mRNA Expression and Correlation With Survivin mRNA Expression in Esophageal Squamous Cell CarcinomaOBJECTIVE:The aim of this study was to investigate the differences of ERCC1mRNA expression between esophageal squamous cell carcinoma(ESCC) and adjacent normal esophageal mucosa, the correlation of ERCC1mRNA and clinicopathological parameters,Survivin mRNA expression in esophageal squamous cell carcinoma(ESCC).METHOD:Tumor specimens were collected from72patients with ESCC who underwent complete resection without preoperative chemoradiotherapy.72cases of normal squamous mucosa of the same patients were included for comparison parameters.We analyzed the ERCC1and Survivin mRNA expression levels in ESCC by Real-time fluorescent quantitative reverse transscription polymerase chain reaction. The expressions of ERCC1mRNA were observed and correlated with clinicopathological and Survivin mRNA.RESULTS:1There was significant differences between ERCC1mRNA expression of ESCC and normal squamous mucosa (P<0.01).There was significant differences between Survivin mRNA expression of ESCC and normal squamous mucosa (P<0.01)。2ERCC1mRNA expression was not associated with tumor size,depth of invasion,tumor cell differentiation,lymph node metastasis and clinical stage(P>0.05).3There was a positive correlation between the expression of ERCC1mRNA and Survivn mRNA(r=0.618,P<0.01)CONCLUSION:There was significant differences between ERCC1mRNA expression of ESCC and paraneoplastic normal squamous mucosa.The ERCC1mRNA expression was not significantly associated with invasion depth, tumor cell differentiation degrees, lymph node metastasis and clinical stage in ESCC.ERCC1may play a role in apoptosis inhibition during the development of esophageal squamous cell carcinoma.
