| Professor Li Delin is a chief physician,doctoral supervisor and national famous old doctor of traditional Chinese medicine.He had a medical dream because of he and his father’diseases.After admitted to the Heilongjiang Institute of traditional Chinese medicine,he fully learned from the academic education of Longjiang Yipa.He studied ancient books,deeply influenced by Huangdi Neijing,Treatise on Febrile Diseases,Treatise on the Spleen and Stomach,Danxi Xinfa,the Golden Mirror of Medicine,Treatise on Epidemic Febrile Diseases,Correction of the Errors of Medical Works,and so on.He learned from Professor Ma Ji and listened to the teacher’s teaching.He consolidated the foundation of traditional Chinese medicine,accumulated clinical experiences,and gradually formed his own unique academic thoughts.Summary of the academic thoughts of Professor Li Delin:1 Consolidate the origins,pay attention to spleen and kidney.In the treatment of the internal medicine disease,it is to protect the normal physiological functions of spleen and kidney,to pay attention to the roles of them,to take targeted treatment strategies,in the process of the occurrence,development,prevention and control of the disease.2 Cold-heat intermingled syndrome is polymorphism,treatment methods according to the internal mechanism include reconciliation,forward or reverse.3 Disease and syndrome should be combined according to the main pathology.The disease of no symptom can be treated with TCM according to the pathology of western medicine.4 TCM and western medicine should be used together and pay attention to pharmacology.Summary of the clinical diagnosis and treatment experiences of Professor Li Delin with typical cases.1 Chronic atrophic gastritis:The majority belongs to distension and fullness of TCM.The pathogeny includes deficiency of spleen and stomach,improper diet,emotional hurt,aeipathia and fatigue,etc.The pathogenesis is deficiency of spleen and stomach,congestion of activities of qi,abnormity of promotion and demotio.The main syndrome is cold-heat intermingled syndrome.Professor Li has developed Yanshenjianwei Capsule.Prescription uses Banxiaxiexin Decoction,adding the drugs ofstrengthening the body resistance and driving out evil spirits.Professor Li put forward that fixed prescription is the trend of the development.He attached importance to maintaining the spleen and stomach.2 Diabetes and its complications:Xiaoke in TCM is equivalent to diabetes in western medicine.Its etiology and pathogenesis are weakness of five zang viscera,fumigation of endogenous heat,consumption of body fluid and qi,sticky and thick blood.Its basic pathologic changes are deficiency of qi and yin and stagnation of blood throughout the whole process of diabetes.The basic therapies are tonifying qi and yin,clearing heat and generating fluid,promoting blood circulation and removing blood.Professor Li has developed a variety of new drugs and prescriptions for diabetes and its complications.3 Rheumatoid arthritis:It belongs to arthromyodynia in general.The pathogenesis is that nutrient qi and defensive qi are out of harmony,then exogenous evils invade and attack in the early stage.In the late,zang-fu viscera are injured and weak,then sputum and blood stasis are mutual combination.Professor Li uses different therapies.The basic prescription is Erlongjuanbi Decoction.4 Post-traumatic epilepsy:It belongs to epileptic syndrome in TCM.The pathogeny is obstruction of the upper orifices by blood stasis and phlegm.Liver wind often causes attack.Post-traumatic epilepsy often combines with limb deformity.Its pathogenesis is blood stasis and qi stagnation in the early,and qi deficiency and blood stasis after a long time.Professor Li often uses Tongqiaohuoxue Decoction,Mengshiguntan Pill and Huangqiguizhiwuwu Decoction.5 Warm-hot diseases:This qi is pectoral qi in Weiqiyingxue.Qiphase syndrome is a series of pathological changes caused by physiological function disorder of pectoral qi.If doctors can catch qiphase and treat timely even prescribe early,it can make the disease not change.This is the key to the treatment of warm-hot diseases.Professor Li Delin is good at treating spleen and stomach diseases.In recent years,he often uses the therapies of acrid-release and bitter-downbearing and regulating spleen and stomach,and adds drugs of tonifying qi and yin and promoting blood circulation and removing blood.The clinical curative effect is better.In order to objectively evaluate the clinical curative effect used by Professor Li Delin in the treatment of chronic non atrophic gastritis with cold-heat intermingled syndrome,this project has arranged the frequently-used prescription of Professor Li and carried on the clinical research.First of all,the literature reviews are summarized as follows:Western medicine has not yet clarified the pathogenesis of CNAG,treatment is mainly symptomatic treatment.Western medicine has reached a consensus on the evolution model of CNAG→CAG→PLGC→gastric cancer,but there is no effective treatment.Traditional Chinese medicine has the same understanding of the etiology of CNAG,pathogenesis and syndrome differentiation are not unified,the overall effect of treatment is better than western medicine.Traditional Chinese medicine began to pay attention to the research on the evolution model,but has not yet reached a consensus.The current treatment is more limited to the GANG stage with the syndrome differentiation,the therapies of prevention and treatment is rare for CNAG→gastric cance.Secondly,Summary and report on the clinical research.Objective:To observe the clinical curative effect of Lianxiaqimai Decoction drafted by Professor Li Delinand used in the treatment of chronic non atrophic gastritis in patients with cold-heat intermingled syndrome,and to dig deeper into his academic thought.Methods:According to the random principle of 1:1,88 patients were divided into the treatment group and the control group,44 cases in each group.At the end of the experiment,there were 4 shedding cases in each group,each group completed a total of 40 cases.The treatment group was treated with Lianxiaqimai Decoction,the control group was treated by Liuweianxiao Capsule.The course of treatment was 8 weeks.The observation indexes were distention and fullness symptoms,gastroscopy examination grading,Hp.Results:In the treatment group,the total effective rate of TCM syndrome is 92.5%,the control group is 75%,which is statistically significant.In the two groups after treatment,the scores of syndrome are significantly decreased and statistically significant,but the treatment group is significantly better than the control group.In the treatment group,the total effective rate of chronic non atrophic gastritis is 80%,and the control group is 62.5%,which has statistical significance.After treatment,the gastroscopy examination grades in two groups have statistically significant,and the treatment group is better than the control group.The positive rates of Hp in the two groups are not statistically significant.Conclusions:Lianxiaqimai Decoction of Professor Li Delin not only can significantly improve the symptoms of chronic non atrophic gastritis with cold-heat intermingled syndrome,and get a very good curative effect of TCM syndrome,but also can improve the gastric mucosal lesions under gastroscopy,and can cure or improve the disease.But the research has not yet shown that Lianxiaqimai Decoction has a killing effect on Hp.This paper excavated the academic thought of Professor Li on treatment of CNAG with cold-heat intermingled syndrome.The root cause of the happen and development on CNAG is qi and yin deficiency of the spleen and stomach.It is all the time and caused more cold-heat intermingled syndrome,eventually lead to qi deficiency,blood stasis,even canceration.The core pathogenesis of CNAG is deficiency of qi and yin,intermingling of cold and heat.The main therapy is acrid-release and bitter-downbearing and tonifying qi and yin,and the adjuvant therapy is promoting blood circulation and removing blood.It provides a new way for the clinical treatment of CNAG,and may also provide a new way for the prevention and treatment of CNAG-gastric cancer.This paper has confirmed that cold-heat reconciliation and preventive treatment of diseases used by Professor Li Delin in treatment of spleen and stomach diseases is effective and practical.Innovative ideas:This paper for the first time has excavated and refined the academic cognition of Professor Li Delin about CNAG in patients with cold-heat intermingled syndrome.He applied the idea of "disease prevention" to CNAG.This paper put forward that qi and yin deficiency of the spleen and stomach is the root reason for occurrence and development of CNAG.It runs throughout pathological changes,its gradual aggravation can lead to yin deficiency and fluid loss,blood stasis,toxin canceration.Cold-heat intermingled syndrome is the form of manifestation caused by it,and the most common in clinical practice.This paper put forward that the core pathogenesis of CNAG is deficiency of qi and yin,intermingling of cold and heat.The treatment and prevention strategies are that the main principle of treatment is acrid-releasing and bitter-downbearing,nourishing qi and yin for CNAG,and the adjuvant therapy is promoting blood circulation and removing blood for preventing development.This paper analyzed the characteristics of composition on Lianxiaqimai Decoction.lt put forward that the treatment of CNAG should attache importance to the whole process of occurrence,development and evolution of CNAG.Acrid-release and bitter-downbearing,tonifying qi and yin,promoting blood circulation and removing blood should be used simultaneously.This study not only provides a new idea for the clinical treatment of CNAG,but also may provide a new way for the prevention and treatment of CNAG→gastric cancer. |
