| Research Purpose:Nutritive qi and Defensive qi are important nutrient substances of humans. There are definite discussions on the source, generation and operation of Nutritive qi and Defensive qi(Yingwei) in Huangdi neijing. Thus, they have been valued by doctors of all dynasties. Nanjing and Treatise on Cold-induced Diseases have applied and developed the theory of Yingwei. Doctors later generations,especially for numerous famous medical scientists in Ming and Qing Dynasties, have disputed the theory invariably and repeatedly, forming a conclusion that "Defensive qi has lucidity and generates yin fluid, while Ying blood has warm temperature and converts yangshen". Such the conclusion has been an important theoretical achievement in the grand dispute of Yingwei. The Professor Wang Hongdu, who is an expert apprenticed from the national fifth distinguished veteran doctors of TCM, inherits academic opinions of past scholars and further proposes an opinion that Yingwei gasification completes metabolism is the essence of Yingwei. The basic theory of previous TCM can't figure out or even confuse the process that Yingwei is generated by ingesting essence of water and grain, is gradually operated to the grain of skin, and returns to the body. Such the process from inside to outside and from outside to inside completes metabolism of Yingwei(including sympathetic separation and reunion of Yingwei, mutual transformation, qi combination and metaplastic mechanism of qi, etc.links) and plays an important physiological function in these links.The study is based on the "metaplastic mechanism of cutaneous interstices qi,of professor Wang Hongdu, settles the academic ideological origin, argument basis and main content, and combines with Professor Wang's experience in clinical treatment of surrounding nerve paralysis and remaining neuralgia of herpes zoster, showing the practical application of the theory. In order to better realize the important theory, the paper also implements clinical experiment of "encircling needling curing remaining neuralgia of herpes zoster", verifies correlation among cutaneous interstices structure, cutaneous interstices qi and remaining neuralgia of herpes zoster from the perspective of briefly discussing encircling needling and activating qi to relieve pain, and elaborates its mechanism with scientific experimental data.Research methods:1. Verification with an teacher:It is necessary to insist in outpatient of an teacher, observe it carefully when the teacher diagnoses and treats, experience verification of an teacher diligently, records clinical data collected from four ways of diagnosis(including look, listen, question and feel the pulse) in details, so as to reflect and summary after diagnosis. Moreover, it is also necessary to record experience of pharmacy, stick to look, think and ask questions, persist in summarizing typical cases of the teacher, experience and try to figure out idea of differentiation and basis of pharmacy.2. Study of Classics:Under the guidance of the teacher, it is necessary to read more classics, understand the origin of the teacher's academic thought comprehensively comprehend connotations of important articles in classics, refer to words of experts giving notes to ancient literatures in previous generations, and try to realize digestion.3. It is necessary to study and discuss academic opinions of the teacher, study academic papers published by the teacher, comprehend his thought, consult with the teacher for unclear theory and thought in daily life, and better experience academic opinions and theoretical origin of the teacher.4. The purpose of Yingwei gasification is to "abandon old things and use new substances", including substances, energy metabolism and qi metaplasia. Cutaneous interstices are important places for Yingwei gasification. Therefore, it is also the processing plants of qi metaplasia.2. Professor Wang Hongdu's academic thought connotations and clinical experience of "metaplastic mechanism of cutaneous interstices qi"The paper includes theoretical basis and argument of "metaplastic mechanism of cutaneous interstices qi", as well as main performance of qi. For painful patients, they often show stabbing pain or scorching hot pains and it often belongs to abnormality of skin conduction. Under the guidance of the teacher, the author selects encircling needling to cure the typical cases with abnormal feeling of cutaneous interstices on eh basis of "briefly introducing encircling needling and activating qi to relieve pain., conducts forward-looking randomized controlled clinical observation, measures the pain degree, emotion, sleep, P substance content in blood before and after curing remaining neuralgia of herpes zoster, and verifies effectiveness of encircling needling for remaining neuralgia of herpes zoster.Conclusions:1. Based on the theory of Yingwei gasification, Professor Wang Hongdu argues the special structure of branches of channels—grandchild vessel for cutaneous interstices of Yingwei and sympathetic gasification and summarizes the "metaplastic mechanism of cutaneous interstices qi" with the process of metaplastic qi. The mechanism supplements the theory of Yingwei gasification and also the sublimation of TCM about Yingwei's physiological cognition. On the other hand, cutaneous interstices qi is the foundation of balancing human physiology and numerous external treatment methods of TCM.2. Professor Wang Hongdu applies the theory to guide clinics, form unique characteristic clinical treatment experience in remaining neuralgia of herpes zoster, and acquires a good curative effect. The theory also enlightens the in-depth reflection and understanding of the author.3. Based on the "metaplastic mechanism of cutaneous interstices qi", the author observes the clinical experimental observation of remaining neuralgia of herpes zoster, by using encircling needling and verifies the changes of cutaneous interstices qi, generation of pain and effectiveness of percutaneous treatment objectively by regarding quantitative determination of P substance as one of observation targets. |
