Objective: To investigate the expression of E-cadherin,APC,-catenin and MMP16 in esophageal carcinoma and their relationship with the clinicopathological characteristics of the patients,and to provide reference for early screening and diagnosis of esophageal cancer.Methods: from June 2012 to June 2015,72 patients with esophageal cancer who were hospitalized in the Affiliated Hospital of * * * University of * * * University were collected.72 specimens were obtained by surgical resection.In addition,31 cases of esophageal cancer were collected as the control group.The expression was detected by immunohistochemical method in esophageal carcinoma and normal esophageal mucosa,E-cadherin beta-catenin and MMP16,analysis of the relationship between esophageal cancer mucosa E-cadherin,beta-catenin,MMP16 expression characteristics of sex,age and pathological rate and the patients,correlation analysis of E-cadherin,-catenin,MMP16 between beta.Results: the expression of E-cadherin in the cell membrane was brown or dark brown,and the positive expression was 100%.In the esophageal cancer tissues,the positive expression rate was 19.4%.The positive expression rate of E-cadherin in esophageal cancer tissues was significantly lower than that in the adjacent tissues,and thedifference was statistically significant(P<0.05).Low differentiation,infiltration and muscle layer,the positive expression of E-cadherin in esophageal carcinoma with lymph node metastasis and TNM stage were lower than the rate of high differentiation,infiltration of muscular layer,no unexpected esophageal cancer lymph node metastasis and TNM stage I and II,significant difference(P<0.05).The expression of-catenin in the cell membrane was brownish yellow or dark brown.The positive expression rate was 93.5%.In the esophageal cancer tissues,the positive expression rate was 22.2%.The positive expression rate of-catenin in esophageal cancer tissues was significantly lower than that in the adjacent tissues,and the difference was statistically significant(P<0.05).Low differentiation,infiltration and muscle layer,beta-catenin expression in esophageal cancer lymph node metastasis and TNM stage were lower than the rate of high differentiation,infiltration of muscular layer,no unexpected esophageal cancer lymph node metastasis and TNM stage I and II,significant difference(P<0.05).Soluble E-cadherin expression level in healthy population was 1.4 ~ 3.7 mg/ml(2.4 + 0.7)mg/ml;serum soluble E-cadherin expression in esophageal cancer patients was 0.9 ~ 2.5 mg/ml(1.5 + 0.8)mg/ml.The expression level of serum soluble E-cadherin in patients with esophageal cancer was significantly lower than that in healthy people,and the difference was statistically significant(t=4.872,P < 0.05).Low differentiation,infiltration and muscle layer,a soluble E-cadherin esophageal cancer lymph node metastasis and TNM stage.The expression level was lower than high differentiation,infiltration without muscle layer,no esophageal cancer lymph node metastasis and TNM stage I and II,significant difference(P<0.05).Spearman correlation analysis showed that E-cadherin was positively correlated with beta r=0.717(-catenin,P=0.000).E-cadherin and beta-catenin were negatively correlated with r=-0.627(MMP-16,-503,P=0.000).Conclusion: E-cadherin,-catenin,MMP16 beta esophageal cancer this abnormal expression,which reduced or absent expression or mutation inactivation,the abnormal expression and carcinogenesis,invasion and metastasis are closely related.Second chapter: relationship between E-cadherin and ?-catenin expression in esophageal cancer tissues and efficacy of chemotherapyObjective: To investigate the relationship between E-cadherin and-catenin expression in esophageal carcinoma tissues and the effect of neoadjuvant chemotherapy and the survival rate of patients.Methods collected in June 2012 to June 2015 in * * hospital inpatient treatment of newly diagnosed esophageal squamous carcinoma patients 72 cases,according to preoperative whether chemotherapy were divided into preoperative chemotherapy group(n = 40)and control group(n = 32).Control group treated by surgery alone,preoperative chemotherapy group in the preoperative of chemotherapy,using immunohisto chemical method was used to detect the expression of the control group of patients with esophageal carcinoma E-cadherin and-catenin expression and serum soluble E-cadherin;using the same methods were detected before treatment in patients with chemotherapy and chemotherapy of E-cadherin and-Catenin expression and serum soluble E-cadherin expression observed the clinical efficacy of the two groups and 1 year and 3 year survival rate.Analysis of the influencing factors of 3 year survival rate in patients with esophageal cancer.Results there was no significant difference in the abnormal expression of E-cadherin and-catenin in esophageal cancer patients before chemotherapy and control group(P>0.05).The abnormal expression of E-cadherin and-catenin in esophageal carcinoma tissues was significantly lower than that before chemotherapy,and the difference was statistically significant(P<0.01).40 cases of esophageal cancer patients,CR 4 cases(10%),PR 13 cases(32.5%),SD 12 cases(30%),PD 11 cases(27.5%),ORR was 42.5%(17/40).According to the efficacy of the group,divided into effective group(CR+PR),invalid group(SD+PD).The results showed that the two group before chemotherapy in E-cadherin expression positive rate of beta catenin,abnormal expression positive rate had no significant difference,after chemotherapy group E-cadherin abnormal expression of positive rate of beta catenin,abnormal expression positive rate were significantly lower than the ineffective group.Effective group and ineffective group before chemotherapy serum soluble E-cadherin expression level of no statistical difference(P > 0.05).After effective chemotherapy group of serum soluble E-cadherin expression levels were significantly higher than that of no group,the difference was statistically significant(P < 0.05).Grouped according to the chemotherapy group survival time,survival time is divided into more than 14 months group in 27 cases,less than 14 months group in 13 cases.Survival time is equal to or more than 14 months group,< 14 months before chemotherapy group abnormal expression of E-cadherin and-Catenin abnormal expression positive rate of no statistical difference(P > 0.05).After chemotherapy,survival time is more than or equal to 14 months group the abnormal expression of E-cadherin and-Catenin abnormal expression positive rate was significantly lower than that of < 14 month group(P < 0.05).Is more than or equal to 14 months group and < 14 month group,the level of serum soluble E-cadherin expression level no statistical difference(P > 0.05),is more than or equal to 14 months in the chemotherapy group of serum soluble E-cadherin expression levels were significantly higher than that of no group(P < 0.05).The overall survival rate of 72 patients with esophageal carcinoma was 66.7%(),33.3%(48/72),21(24/72),3 months,respectively.The control group of 32 patients with 1 years,3 year survival rate was 59.4%(19/32),28.1%(9/32),the median survival was 18 months.The survival rates of 40 patients in the preoperative chemotherapy group were 72.5%(29/40),37.5%(),3(15/40),respectively,and the median survival time was 29 months.The 1 year and 3 year survival rate of patientswith preoperative chemotherapy was higher than that of the control group(P<0.05).Control group of patients with abnormal expression of E-cadherin in weak positive 1 year,3 years survival rate was significantly higher than that in the patients with strongly positive;aberrant expression of beta catenin weakly positive in 1 year,3 years survival rate were higher than that of the strong positive patients.The 1 year and 3 year survival rates of and-catenin in the patients with abnormal expression of E-cadherin and were significantly higher than those in the positive group.The abnormal expression of preoperative chemotherapy for esophageal carcinoma patients with tumor size,TNM staging,E-cadherin aberrant expression of beta catenin,and serum soluble E-cadherin expression of factors that influence the curative effect in the near future,3 years survival rate.Conclusion: the abnormal expression of E-cadherin and ?-catenin,MMP16 has a significant effect on the efficacy and prognosis of chemotherapy.
