Objective:Lower respiratory infections,such as acute pneumonia,peribronchitis,acute bronchitis are common diseases in clinic,which belong to the Wind-warm Lung-heat disease in Chinese medicine with high morbidity rates in children.This study intended to explore the etiology relationships between YangMing internal heat and Wind-warm Lung-heat disease by processing theory exploration and prospective cohort study methodology,for providing a theoretical basis as the prevention and treatment for Wind-warm Lung-heat disease.Methods:1?Theory exploration.Through study of the Chinese medicine classics,and analyzing the location and nature of Wind-warm Lung-heat disease and YangMing internal heat,we clarified the concepts between two of them,further combining with the traditional Chinese medicine theories of "integration of internal and external" and "corresponding of homogeneous qi",exploring how internal injury affect external infection.YangMing internal heat localized in stomach and intestine,Wind-warm Lung-heat disease localized in lung,both are closely correlated between internal injury and external infection,viscera and belly.With this theory exploration,it is proved that YangMing internal heat does have impacts on the onset of Wing-warm Lung-heat disease,and this provides a theory basis for the development of clinical cohort study.2?Prospective cohort study.Cases were collected from Pediatric of Oriental Hospital of Beijing University of Chinese Medicine,Beijing University of Chinese Medicine Dongzhimen Hospital Department of Pediatrics,Guo Yi Tang outpatient department of Beijing University of Chinese Medicine,selecting children of 1 to 18 years of ages who suffered from respiratory tract infection more than 3 times a year and cured,between April to June of 2013.According to YangMing internal heat diagnostic criteria,we divided the patients with Exposed factor(YangMing internal heat)as Exposed group,and the ones with Non-exposed factor as Non-exposed group.We followed up each patient for six months,and every three months for a period,total two followed-up within 6 months.Details were documented regarding information about Wind-warm Lung-heat disease onset during this period of timeAfter gathered all the information,we used Epidata 3.1 to record,and used SAS Enterprise Guide 5.1 version statistics software to analyze the data.Results:1?334 cases were collected,320 cases of them were followed up,14 cases were withdrawn(4.19%).In which 227 cases were in Exposed group(70.9%),93 cases were in Non-exposed group(29.1%).During six months of this study,the onset frequency of Exposed group was 2.9339±1.70113,but Non-exposed group was 2.2473±1.76719.2?The onset frequency of exposed cohort group in six months was 95.15%,and that of Non-exposed group was 84.95%,the former is higher than the latterbout the onset frequency of Wind-warm Lung-heat disease(P<0.05),RR>1..3?Statistically analysis showed that the positive correlation between YangMing internal heat and onset frequency of Wind-warm Lung-heat disease had statistically significance(P<0.01)differenence.The higher the YangMing internal heat,so is the onset frequency of Wind-warm Lung-heat disease,it was basically a linear tendency.4?A positive correlation between poor diet structure and YangMing internal heat,has a statistically significance.However,poor diet structure shows no relation to the onset frequency of Wind-warm Lung-heat disease.5?With correlation analysis to selecting the high risk factor of Wind-warm Lung-heat disease,we have discovered that age,eye discharge,erythra,sleep-talking,night molars,redden tongue,thick fur,and so on has positive correlation to the onset frequency of Wind-warm Lung-heat disease,Using linear regression to screen the risk factor of onset frequency of Wind-warm Lung-heat disease,it appeared nasal scab,redden tongue,allergic history,has positive correlation,Linear regression formula is Y=-0.060+0.103X1+0.106X2+0.238X3-0.082X4.(Factor variable Y is the onset frequency of Wind-warm Lung-heat disease in six months,X1 is nasal scab,X2 is redden tongue,X3 is allergic history,X4 is thirst)With correlation analysis to selecting the high risk factor of Wind-warm Lung-heat disease,we have discovered that dry feces,the low frequency of bowl movement,frequent sweating,abnormally increased appetite with frequent hunger,nasal scab,heat in palms and soles,smelly feces,redden tongue,thick fur and so on has positive correlation to the onset frequency of Wind-warm Lung-heat disease,Using linear regression to screen the risk factor of onset frequency of Wind-warm Lung-heat disease,it appeared dry feces,nasal scab,night molars has positive correlation,Linear regression formula is Y=3.778+0.401X1-0.113X2+0.293X3-0.292X4+0.198X5.(Factor variable Y is the onset frequency of Wind-warm Lung-heat disease in six months,X1 is dry feces,X2 is BMI,X3 is nasal scab,X4 is eye discharge,X5 is night molars.)6?In screening the high risk factor of poor diet structure to YangMing internal heat,we used linear regression formula Y=16.245+2.61X1+1.747X2+1.609X3+3.716X4+2.749X5+1.152X6+1.11X7.(Factor variable Y is the level of YangMing internal heat,X1 is greasy food,X2 is cookies,X3 is meat,X4 is allergic history,X5 is sticky food,X6 is overeating,X7 is canned juice.)Conclusion1?YangMing internal heat is the high risk factor causing onset of Wind-warm Lung-heat disease.The severity of YangMing internal heat induced he high onset frequency of Wind-warm Lung-heat disease.In which dry feces is relatively more risky.YangMing internal heat could be the internal cause of allergies.2?YangMing internal heat is the high risk factor causing onset of respiratory tract infection.The severity of YangMing internal heat induced he high onset frequency of Respiratory tract infection.3?Improper diet structure is a high risk factor of causing YangMing internal heat.The improper diet structure is positive correlated with the severer YangMing internal heat increase.4?The prevention and treatment of Wind-warm Lung-heat disease involves with paying close attention to how YangMing internal heat greatly affects on the onset of Wind-warm Lung heat disease.Also correct diet structure could reduce the degree of YangMing internal heat.eliminating or decreasing YangMing internal heat helps decreasing the onset frequency of Wind-warm Lung-heat disease.It has a very important clinical guiding significance for preventing the onset of such disease.
