| Bronchial asthma is a chronic airway inflammation with reversible air flow that is involved in a variety of cellular and cellular components.Clinical manifestations of recurrent wheezing,shortness of breath,chest tightness and/or cough,etc.,at night or early morning attack,exacerbation,seriously affecting the quality of life of patients.At present,the treatment of asthma is still not curable,and its main purpose is to control and relieve its onset.In recent years,the incidence of asthma increased year by year,has become a global medical problem.This study through a large number of literature and theoretical discussion,that the pathogenesis of asthma is deficiency in origin and excess in superficiality,spleen and lung Qi throughout the entire course of asthma,the treatment should be timely replenishing spleen and lung Qi.On the basis of clinical experience using the side "Yi Qi Ping Chuan" for clinical research.At the same time,focusing on the TSLP/NF-κB signaling pathway,the mechanism of action is explored through experimental studies.Purpose:To observe the clinical efficacy of Yi Qi Ping Chuan Decoction in treating asthma with spleen-lung-qi deficiency and to further explore the molecular mechanism of Yiqi Pingchuan decoction in treating asthma.Method:1.Fifty-six patients with spleen-lung-qi deficiency were selected from chronic asthma exacerbation and/or remission asthma patients,and randomly divided into TCM treatment group and control group.The control group received budesonide formoterol powder inhalation;TCM treatment group with both Yi Qi Ping Chuan Decoction and budesonide formoterol powder inhalation,the observation time was 2 months.The symptom scores,ACT scores,TCM syndrome scores and quality of life scores of asthmatic patients were observed before treatment,4 weeks and 8 weeks after treatment,respectively.The differences between the two groups were analyzed and compared,and the effectiveness of two groups in treating asthma and safety.2.The high expression of TSLP was induced by LPS and Poly(I:C)in human bronchial epithelial cell line to simulate the asthma airway model in vitro.The asthma models were induced by different concentrations(2 ug/ml,3 ug/ml,4 ug/ml).The effect of Yi Qi Ping Chuan Decoction on the protein expression of TSLP and NF-κB signal transduction pathway was detected by western blot,Q-PCR,immunofluorescence and ELISA.Result:1.After 4 weeks of treatment,the TCM treatment group and the control group had similar efficacy in alleviating the day and night-related symptoms of asthma patients,controlling the condition of asthma and improving the quality of life of the patients.There was no significant difference between the two groups(P>0.05);After 8 weeks of treatment,the TCM treatment group was better than the control group(P<0.05).TCM treatment group at various time points after administration can effectively improve the symptoms of "spleen-lung-qi deficiency" in asthma patients(compared with before treatment,P<0.05 after 4 and 8 weeks of treatment),while the control group improved TCM syndrome had no significant effect,the difference between the two groups was statistically significant(P<0.05).2.Different concentrations of Yi Qi Ping Chuan Decoction can effectively reduce the expression of TSLP in asthmatic airway models(all P<0.05),and inhibit the expression of P-P65,P-IκBa and P-Iκκα The degree,of inhibition is proportional to the concentration of Chinese medicine intervention(P<0.05).At the same time,Yi Qi Ping Chuan Decoction can significantly reduce the expression of TNF-α,IL-1β,IL-6 and IL-8 and down-regulate the expression of TLR4 and MyD88 in the upstream of NF-κB(all P<0.05),but had no effect on the expression of TLR3 and TRIF(P>0.05).Conclusion:1.Yi Qi Ping Chuan Decoction can significantly improve the clinical efficacy of western medicine in the treatment of asthma.2.Yi Qi Ping Chuan Decoction can alleviate the occurrence of allergic inflammation by inhibiting the expression of TSLP by regulating the activation of TLR4/MYD88/NF-KB signaling pathway. |
