| BackgroundNear infrared spectroscopy(NIRS)is a noninvasive,sustainable monitoring,bedside operation,can be used as an indicator of inadequate tissue perfusion and oxygenation technique.At present,this technique can be used for monitoring oxygen saturation of brain,kidney,intestines,muscles and so on.Intestinal ischemia is an important cause of intestinal mucosal injury,which can lead to necrosis of intestinal tissue and porforation.Foreign scholars have reported A-rS02 for normal term infants the first two day after birth.Some other scholars have studied the characteristics of abdominal rS02 in early preterm infants.There was few reported about NIRS on A-rS02 in China.Therefore,this paper conducted a preliminary study on the abdominal rS02 of normal preterm infants and term infants within the first five days after the birth.Objectives1、Initially know reference values of neonatal A-rS02 for 5 days after birth.2、Explore the effect of gestational age(GA)on preterm infant and term infant A-rS02.3.Explore the effect of day age after birth on preterm infant and term infant A-rS02.Patients and MethodsSubjects:From December 2016 to December 2017 in Nanfang Hospital,Infants more than 32 weeks’ gestation and dayage<5d,respiratory and circulation stability,without gastrointenstinal tract diseases were eligible for enrollement in the study.Methods:A-rS02 was measured every day until discharged or day age>5d by NIRS,3-5min per time each day,and the average of A-rS02 is calculated for every time.The neonatal were categorized by GA,preterm group and term group.The effect of the GA on A-rS02 was obtained by comparing preterm group with term group.The effect of the day age on A-rS02 was obtained by comparing A-rS02 of different day within the group individually.Results1、Within the first five days after birth,the baseline value of A-rS02 in the term infant group was fluctuated at(26.1-72.9)%,the mean(M)(standard deviation(SD)was(60.2 + 7.8)%,and the median value was 62.5%(57.4%-65.2%);The baseline values of A-rS02 in preterm infants were fluctuated in(17.8-68.5)%,and M±SD(56.9 + 10.0)%,and the median value was 59.4%(50.6%-63.6%).2.Within the first five days after birth,the A-rS02 in the preterm infant group was lower than that of the term infants group,and the difference between the two groups was statistically significant(P=0.024).A-rS02 was statistically significant(P=0.046)on the third day.3、Within the first five days after birth,The lowest A-rS02 was observed on day2(57.22±9.1)%in term infants group,comparing with day3(62.7±5.9)%、day4(62.6±5.2)%,A P value is 0.004,0.006,there are differences found between day age.the baseline of A-rS02 in preterm infants increase before decreased with day age,the highest A-rS02 was observed on day 3,Means 58.9%(53.9-63.9)%,and the lowest value on day1 and day5,there is no difference found between day age.Conclusion1.This paper show that the baseline A-rS02 change with GA the first five days of life.The baseline value of A-rS02 in the preterm group was lower than that of the term infants..4、2.This paper show that the baseline characteristics of A-rS02 is different bettween term infants and preterm infants.The baseline A-rS02 on the day2 is significantly lower than day3 and day4 in term infants during the first five days of life.The baseline A-rS02 in preterm infants increase before decreased of day age during the first five days of life. |
