Objective:MR-proANP may have a good value of diagnostic and prognostic in HFpEF compared with N-terminal pro-B-type natriuretic peptide(NT-proBNP).Method:In basic research,the relationship between atrial pressure and MR-proANP secretion was studied by establishing a canine atrial hypertension model.In clinic study,participants with cardiovascular disease who were enrolled in this study were divided into the non-HF group(n=75),HFpEF group(n=65)and heart.failure with reduced ejection fraction(HFrEF)group(n=50).The MR-proANP and NT-proBNP levels in the plasma from all patients were.measure,d by enzyme-linked immunosorbent assay.Baseline data,biochemical indicators and cardiac echocardiography data were also collected for the three groups every 6 mouths to analyze the diagnostic and long-term prognostic value of MR proANP in patients with chronic HFpEFResult:The basic research confirmed that in the canine atrial hypertension model,the secretion of the MR-proANP in the plasma increased gradually with the increase of atrial pressure,and the secretion rate increased rapidly.In the clinic study,the plasma MR-proANP and NT-proBNP levels in the HFpEF and HFrEF groups were higher than those in the non-HF group(P<0.05).There were significantly different(P<0.05)of MR-proANP levels in different Classification patients with HFpEF.In the diagnostic analysis,the area under the curve of MR-proANP(0.844)was higher than the area under the curve of NT-proBNP(0.518,P<0.001).Compared with NT-proBNP,MR-proANP had a better advantage in the diagnostic value of HFpEF patients(AUC:0844 vs 0.518,P<0.001).NT-proBNP hard a good predictive value for the 1-year rehospitalization risk of HFrEF patients and HCY was an independent risk factor for patients with HFpEF in 1-year rehospitalization risk.Conclusion:The present study indicates that MR-proANP may be more sensitive and specific than NT-proBNP in diagnostic and prognostic in chronic HFpEF.This parameter may be used as a potential diagnostic biomarker in patients with chronic HFpEF.
