Part I:Effect of Tengli Anti-tumor Prescription on recurrence and carcinogenesis of human colorectal adenomatous polypsObjective:To explore the effect of "Tengli Anti-tumor Prescription" on the postoperative treatment of colorectal adenomatous polyps,and provide reliable scientific basis for the prevention and treatment of colorectal adenomatous polyps recurrence and carcinogenesis with Chinese medicine.Based on the theory of"Strengthening The Body And Removing Pathogenic Factors" and giving full play to the multi-target,multi-link and multi-approach advantages of Chinese medicine.Methods:90 patients with colorectal adenomatous polyps who were admitted to the department of proctology of Guizhou provincial hospital of traditional Chinese medicine and met the inclusion criteria were randomly divided into the treatment group(n=45)and the control group(n?45).The treatment group began to take"Tengli Anti-tumor Prescription" 2 times a day for 3 months after pathological diagnosis of adenoma 3?5 days after electrotomy.No special treatment was given to the control group after the treatment with electron microscopy.After half a year routine examination,adenomatous polyp recurrence,canceration,liver and kidney function and CD4+,CD8+,CD4+/CD8+,NK cell content changes were observed.Results:1.Tengli Anti-tumor Prescription had no significant effect on the general situation of patients with colorectal adenomatous polyps,liver and kidney function,etc.,and no significant adverse reactions were found in all patients during the treatment.2.In polyp recurrence and relapse:the treatment group 45 cases,11 cases(24.4%)had polyp recurrence(recurrence and relapse),a total of 39,27 cases in the control group 45 cases(60%),recurrence of polyp recurrence(recurrence and relapse),a total of 68,between the two groups was statistically significant(P<0.05),suggesting that Tengli Anti-tumor Prescription intervention in the adenomatous polyp recurrence and relapse have a certain effect.3.Tengli Anti-tumor Prescription treatment group and control group patients in the observation after review found that the two groups in polyp size,location and pedicle has no significant difference(P>0.05),but in the aspects of polyps intraepithelial neoplasia:treatment group in the low level of intraepithelial neoplasia,high-grade intraepithelial neoplasia,disease-free variable respectively,9(23.1%),5(12.8%),25(64.1%)and control group respectively,23(33.8%)and 19(27.9%),26(38.2%),P = 0.032(P<0.05),between the two groups have significant difference.Pathological types:treatment group tubular adenoma,villous,tubular adenoma,wool tube tubular adenoma,other polyps are:12(30.8%),7(17.9%),3(7.7%),17(43.6%)and control group respectively:31(45.6%),17(25.0%),9(13.2%),11(16.2%),P = 0.03(P<0.05),indicating that has a significant difference between the two groups.4.Clinical incidence of adenocarcinoma:One adenoma in the control group was found to have penetrated through the mucosa muscularis layer and infiltrated into the submucosa,reaching the clinical diagnosis of adenocarcinoma.The incidence rate was the total patient(case)2.2%,accounting for 1.47%of total polyps(P),P=0.45(P>0.05),the two groups were not statistically significant.5.Intervention results of Tengli Anti-tumor Prescription on immune function:after treatment,two groups of CD4 +,CD4 +/CD8 + and NK cells of the numerical have obvious difference(P<0.05),the treatment group than the control group,two groups of CD8 + numerical comparison,no statistical difference(P>0.05),but was declined,the treating group of drugs in regulating immune function in patients with better than the control group.Conclusion:6 months postoperative colonoscopy and return visit found that;"Tengli Anti-tumor Prescription”can reduce the recurrence and relapse of polyp after colorectal adenomatous polyps and intraepithelial neoplasia,reduce colorectal adenomatous polyps.The cancer rate can simultaneously increase the CD4+,CD4+/CD8+,NK cell content,enhance the body's immune function,and has no significant effect on liver and kidney function in patients with colorectal adenoma.Part ?:Establishment of a colorectal adenomatous polyp model in C57BL/6 miceObjective:To establish a colorectal adenomatous polyp model in C57BL/6 mice and provide animal models for related experimental studies.Methods:60 healthy C57BL/6 mice were randomly divided into group A(model group)35 and group B(blank group)25.AOM combined with DSS three cycles were used to establish a mouse colorectal adenoma model.The optimal dose of AOM was 10 mg/kg intraperitoneal injection.DSS concentration was 2.5%free to drink,3 times combined with 3 cycles,9 weeks to build the model.After each cycle,6 mice were randomly selected for dissection,Observing the occurrence of adenomas by naked eye and pathological section.Results:After the end of the first cycle of the experiment,no polyps were formed in group A(model group)and group B(blank group).After the end of the second cycle,adenoma formation was observed iin 3 of the 6 model groups,and no significant improvement was observed in the blank group.After the third cycle,there was no significant improvement in the normal group.Six mice in the model group showed different degrees of adenoma growth,and the tumor incidence rate was 6/6×100%=100%.There was a significant difference between the two groups.Conclusion:AOM/DSS three-cycle mouse colorectal adenomatous polyp model has a success rate of 100%and high reproducibility.It is one of the effective methods for establishing a mouse adenomatous polyp model,which is worthy of popularization.Part III:Effect of Tengli Anti-tumor Prescription on colorectal adenomatous polyps and related immune indexes in C57BL/6 mice.Objective:To investigate the effects of Tengli Anti-tumor Prescription on colorectal adenomatous polyps and related immune function in mice by intervening treatment of colorectal adenoma C57BL/6 mice,and to provide experimental basis for the prevention and treatment of colorectal adenomatous polyps with traditional Chinese medicine.Methods:84 male C57BL/6 mice were randomly divided into 6 groups.Group A:AOM/DSS(model group)22,group B:blank group 14,group C:Tengli Anti-tumor Prescription(low dose group)12,group D.Tengli Anti-tumor Prescription(medium-dose group)12,group E:Tengli Anti-tumor Prescription(high-dose group)12,group F:Celecoxib group(positive control gi'oup)12,mouse intestinal adenomatous polyp model was established,choose AOM combined with DSS three cycles method(specifically the first part of the experiment)intervention measures.C,D,E,F group on the basis of model group for drug intervention,that is Drug intervention was initiated while intraperitoneal injection of AOM until the end of the experiment.Method of administration and dosage:Groups C,D,and E:From 1 to the 10th week,on the basis of the model group,different concentrations of rattan anti-tumor were administered to the stomach for 0.2 ml/d/day.Time:from 1 to the 10th week.Group F:20 mg/kg/d celecoxib was intragastrically administered once a day for the first week to the 10th week.Results:1.The low-dose group of Tengli Anti-tumor Prescription had no significant improvement on the intraepithelial neoplasia of colorectal adenomatous polyps in mice,and the results were similar to those in the model group(P>0.05).Compared with the model group,the medium and high dose groups were improved,and there was no significant difference in efficacy between the two groups.However,compared with the positive control group,there was no statistical difference between the medium and high dose groups in Tengli Anti-tumor Prescription(P>0.05).2.Tengli Anti-tumor Prescription has a significant effect on the content of INF-ly,IL-2 and TNF-a in a mouse model of colorectal adenomatous polyps,which can improve the INF-y,IL-2,TNF-a content of mice in the colorectal adenomatous polyp model group.The content of INF-y was not significantly different from that of the positive control group(P>0.05).The content of IL-2 in the middle dose group of Tengli Anti-tumor Prescription group was statistically different from that of the positive control group(P<0.05).The content of TNF-a in the high-dose group was significantly different from that in the positive control group(P<0.05).3.The CD4+(%),NK cell content and CD4+/CD8T in mice with colorectal adenomatous polyp were better than that in the positive control group,and the effect on CD8+(%)was not significantly different(P>0.05),which further verified that the Tengli Anti-tumor Prescription could improve the immune function of the body.Conclusion:1.The Tengli Anti-tumor Prescription has a certain effect of preventing and resisting the occurrence and development of colorectal adenomatous polyps,which is similar to celecoxib,but significantly superior to celecoxib in enhancing the immune function of the body.2.Tengli Anti-tumor Prescription can increase the content of INF-y,IL-2,TNF-a,CD4+,CD4+/CD8+ and NK cells in the colorectal adenomatous polyp mice,and enhance the immune function,which may be one of the mechanisms of inhibiting the occurrence and development of colorectal adenomatous polyps.
