Knowledge Discovery Of Fuyang School Medical Case Based On Attribute Partial Order Structure Theory

ObjectiveThe inheritance of the traditional Chinese medicine school has great significance for the development of Chinese medicine.As a carrier of clinical experience,medical records are an important entry point for studying academic schools.Fuyang School is a young academic school in Chinese medicine.It is famous for its use of aconite for the treatment of difficult and dangerous syndromes.Its clinical curative effect is remarkable,which fully reflects the advantages of traditional Chinese medicine and deserves in-depth study.The previous research is mainly based on frequency analysis,cluster analysis and association rule analysis.It is still difficult to explore the tacit knowledge of Fuyang school doctors comprehensively and completely.The formal concept analysis method is rigorous and scientific.After years of exploration,the knowledge discovery method combined with the attribute partial order structure theory has become increasingly mature.In order to make the Fuyang school's academic thoughts and clinical experience deeply understood and get better inheritance,this study attempts to combine the attribute partial order structure theory to carry out data mining for the information of Fuyang medical case,and transform the medical case data into more scientific,Intuitive knowledge for easy inheritance,research and use.I hope to this thesis could plays an inspirational and beneficiary role in the clinical dialectical treatment and prescription drug use,and on this basis,provide a new way to explore the data mining work of academic genre.MethodThis research is based on medical research.The collected medical cases are mainly from three books edited by Zhang Cunxin,including 34 medical cases of 34 Fuyang medical doctors.According to the inclusion and exclusion criteria,597 medical cases were finally screened out.The information of the medical case is treated according to the name of the doctor,the medical practice area,the sex of the patient,the age of the patient,the symptoms,the signs,the tongue,the name of the Chinese medicine,the type of TCM syndrome,the TCM treatment,the prescription,the Chinese medicine,and the dose.The attributes are standardized and then typed into an Excel spreadsheet to form the original medical knowledge database.Taking the database as the object,using the knowledge discovery method of the attribute partial order structure diagram theory,the formal background optimization is carried out,and the hierarchical complex concept network diagrams of Fuyang medical case are formed"medical family-region-disease-symptom-pattern of syndrome-treatment-presciption-drug-dosage",then interpret and analyze attribute partial order structure diagrams from the perspective of hierarchies,clusters,and branch lines.According to "spirit,color,shape,state,diet,sleep,stool,chills and fever,effluent,tongue and pulse" this thesis sums up the "negative syndrome" of Fuyang School;summarizes the law that "Yin-fire" and" Yin-syndrome”must coexist in Yang-deficiency patients from the symptoms of fever and its coexistence,which provides a factual basis for accurately identifying "Yin-fire" and”Yin-syndrome".According to the attribute partial order structure diagram,the disease-prescription is found from the perspective of disease and prescription.From the perspective of prescriptions,symptoms and diseases,we can find prescriptions-symPtoms and prescriptions-diseases;from the perspective of different doses of aconite,we can find the dose-effect relationship of aconite;from the perspective of different regions,we can find the characteristics of regional medication;and from the perspective of representative physicians,we can study the experience of physicians in syndrome differentiation and medication compatibility.ResultsAccording to the inclusion exclusion criteria,597 Fuyang medical cases were collected,and the Fuyang medical case knowledge database was established.A total of 1621 attributes were obtained,and the attribute partial order structure map was constructed from different angles.According to the attribute partial order structure diagram,the following results were obtained:the most common symptoms and signs of the Fuyang medical case;the highest priority attributes of“spirit,color,shape,state,diet,sleep,stool,chills and fever,effluent,tongue and pulse"in symptoms and signs;the correspondence relation between seven fevers and accompanying symptoms and drugs;the corresponding relationship between the most comnon symptom pain in the medical case and accompanying symptoms,syndromes,treatment,prescriptions and drugs;diseases-prescriptions of 10 common diseases;common syndrome types,treatment methods,prescriptions of medical cases;the prescription-symptom and prescription-disease of Sini Decoction,Mahuang Fuzi Xixin Decoction,Fuzi Lizhong Decoction,Qianyang Jieshen Dan,Fang-Dise,Fang-Dise;the dose-effect relationship of commonly used drugs,drug pairs,drug groups,doses and aconites;the correspondence relation between medical cases and diseases,drugs and doses in different regions;analysis of the experience of four representative doctors in symptoms and signs,diseases,drugs and dosage compatibility.ConclusionCombined with the research results,it is enough to prove that the Fuyang medical doctors have a yin and yang as the key point to judge the disease,and has a detailed method in identifying the "yin syndrome".Besides,they also have concluded an original theory of "Yin-fire" on the evidence of deficient yang with upper manifestation and"genuine qi" with upper manifestation.The "Yin-fire" of people with"Yang-deficiency" often appears at the same time as "Yin-syndrome".Doctors pay more attention to Yang Qi.Kidney Yang is the basis of treatment.In the treatment of syndrome of yang deficiency due to yin excess,the first thing to use medicine is to support Yang.Ginger and Aconitum are commonly used in treatment,and Sini prescriptions are used adequately.They have a unique combination of drugs.For the treatment of critically ill patients,high-dose aconites are the mainstays.But there are also a few doctors who use small and medium doses of aconite to treat serious illness and yield good results,which is usually related to the experience of doctors and drug compatibility.Fuyang doctors come from all over the country,patients as small as newborns,infants and young children.Fuyang pays little attention to region or age,they believes"have evidence,use drugs",as long as the syndrome differentiation is appropriate that it can receive good results.The difference in prescription medication is closely related to the doctor,s comprehension of Fuyang thought,personal clinical experience,the era in which it is located,the region in which it is located,and the patient,s condition.In summary,the characteristics of Fuyang medical doctors are as follows:Yin and Yang are the key to differentiate all diseases,and the kidney yang is the main pacifier;genuine qi with upper manifestation is key to identify "yin fire";the yin and yang syndrome must be co-occurring;best use of four adversaries,compatibility with unique curative effect;syndrome of yang deficiency due to yin excess need to be treated with grand nourishment;large dosage of aconite is abnormal;regional age need not be taboo,prescription medication depends on doctors.
