| ObjectiveBy applying randomized-allocation study way,via opposing needling group and routine needling group as control groups;through clinical observation of acupoint Zhongfeng opposing needling combined with routine needling to treat on periarthritis humeroscapularis in order to explore its effectiveness and superiority.Methods90 patients suffered from periarthritis humeroscapularis that meets the inclusion criteria for the study of periarthritis.They were randomly allocated to one of three groups,routine needling group(A group),acupoint Zhongfeng opposing needling group(B group)and acupoint Zhongfeng opposing needling combined with routine needling group(C group).Each group has 30 patients.Regarding the diagnostic criteria of shoulder periarthritis,western medicine diagnostic criteria refers to "Clinical Pain Therapeutics".The dialectical classification of Chinese medicine is based on the important syndrome differentiation of "Shoulder Bi",which refer to "Diagnosis and treatment of rheumatic disease".Three groups based on treatment under the same conditions.A group therapy the main point:the LI 15,SI 9,SJ 9,SJ 14.Distribution point:Small Intestine Meridian of Hand-Taiyang,with SI 3 and B 60;Large Intestine Meridian of Hand Yangming,with He Gu and S 38;Intestine Meridian of Hand-Shaoyang,with SJ 5 and GB 34;Blood stasis resistance persons,with He Gu and CV 6;Windchill persons,with G 20 and He Gu;Cold-damp Stagnation persons,with ST 36 and SP 9;Deficiency of qi and blood,with ST 36 and CV 6.B group the main point:the contralateral lower limbs of the affected side of point LR 4.Group A and Group B acupoints are the same body acuounctue,and joint the contralateral lower limbs of the affected side of point LR 4.The acupuncture treatment by a professional acupuncturist was carried out the operation,before the trial operation of the uniform operator norm.All groups were taken treatment for one time every day,which were needed 4 courses the three course treatment,the score of shoulder joint function of three groups was significantly increased,it showed all the patients of the three groups have been greatly improved in the shoulder procedure of acupoint zhongfeng opposing needling therapy is convenient,it needs to be combined with routine needling for better curative effect.Therefore,the curative effect of acupoint Zhongfeng opposing needling combined with routine needling therapy for perarthritis of shoulder is worthy of affirmation and promotion.2.The scores of McGill questionnaireIn terms of PRI,The rating index of shoulder joint pain in the three groups was improved before treatment,and the changes of pain scores of sensory items and emotional items were statistically different before and after treatment.The PRI scores of three groups were reduced after the course of treatment,it is indicated that three groups have the effect of relieving pain.After treatment,comparisons were made among the three groups.In terms of the improvement of PRI,the acupoint Zhongfeng opposing needling combined with routine needling may have a superposition effect,in which the treatment advantage is more obvious in the pain scores of sensory items and emotional items.The PRI scores of the acupoint Zhongfeng opposing needling combined with routine needling were lower than the routine needling group and the acupoint Zhongfeng opposing needling group.There was no difference in PRI scores between two groups.Although the procedure of acupoint Zhongfeng opposing needling is convenient,it needs to be combined with routine needling for better reduction of PRI.In terms of VAS,it was improved after treatment in all three groups,and there was a statistically significant difference in patients' pain degree before and after treatment.The VAS scores of three groups were reduced after the course of treatment,it is indicated that three groups have the effect of relieving pain.After treatment,comparison was made among the three groups.In the three groups of treatment methods,in the improvement of VAS,the acupoint Zhongfeng opposing needling combined with routine needling may have a superposition effect,and the pain degree of patients gradually alleviated from severe pain to mild pain,and the treatment advantage in VAS was more obvious.The VAS scores of the acupoint Zhongfeng opposing needling combined with routine needling were lower than the routine needling group and the acupoint Zhongfeng opposing needling group.There was no difference in VAS scores between the routine needling group and the acupoint Zhongfeng opposing needling group.In terms of PPI,PPI of the three groups was improved after treatment,and there was a statistically significant difference in the change of pain degree of the patients before and after treatment.The PPI scores of three groups were reduced after the course of treatment,it is indicated that three groups have the effect of relieving pain.After treatment,three groups were compared.Among the three groups,in terms of PPI improvement,the acupoint Zhongfeng opposing needling combined with routine needling may have a superposition effect,and patients' pain degree gradually alleviated from extreme pain to discomfort,etc.The treatment advantage was more obvious in PPI.The PPI scores of the acupoint Zhongfeng opposing needling combined with routine needling group were lower than the routine acupuncture group and the acupoint Zhongfeng opposing needling group.There was no difference in the PPI scores between the routine needling group and the acupoint Zhongfeng opposing needling group.In the total score of McGill pain questionnaire,the total score of McGill pain questionnaire of three groups were reduced after the course of treatment,it indicated that three groups have the effect of relieving pain.The total score of McGill pain questionnaire of the acupoint Zhongfeng opposing needling combined with routine needling group were lower than the routine needling group and acupoint Zhongfeng opposing needling group.There was no difference in the total score of McGill pain questionnaire between the routine needling group and the acupoint Zhongfeng opposing needling group.3.The comparison of the quality of life(WHO QOL-BREF)All the three groups have remarkable effect on improving the quality of life,by the analysis of the World Health Organization Quality of Life-BREF score.The routine needling group only improved in the physiological domain,the psychological field,the subjective feeling of the quality of life and the health condition,and the improvements of physiological domain were highest.There was no significant change in environmental areas and social relations.In the acupoint Zhongfeng opposing needling group,there was no significant change in both environmental areas and social relations,and the other four areas were significantly improved after treatment.In the acupoint Zhongfeng opposing needling combined with routine needling group,there was no significant change in environmental areas,and the other five areas were significantly improved after treatment.4.Short form 36 health surveyBefore treatment,the quality of life in all three groups was low,and there was no significant difference between groups.The three groups' s nice-area level of physical function,role-physical,bodily pain,general health,vitality,social function,role-emotional and mental health are all higher than that of pre-treatment.5.Self-rating anxiety scalesThere was no statistically significant difference in SAS scores between the three groups of patients before treatment.The anxiety level of the three groups was consistent.After different acupuncture treatment,all three groups of SAS scores decreased,and the decrease was more significant than before treatment.It is indicated that the three acupuncture therapy can significantly relieve the anxiety of patients with periarthritis of shoulder.6.The comparison of curative efficacyTreated by four courses,the routine acupuncture group cured 2 examples(6.67%),and 7 examples had excellent effect(23.3%),12 examples had been better(40%),and 9 examples had no effect(30%).The balance acupuncture group cured 3 examples(10%),and 8 examples had excellent effect(26.7%),11 examples had been better(36.7%),and 8 examples had no effect(26.7%).The acupoint Zhongfeng opposing needling combined with routine needling group cured 9 examples(30%),and 13 examples had excellent effect(43.3%),5 examples had been better(16.7%),and 3 examples had no effect(10%).All the three methods of treatment had good effect on perarthritis of shoulder.And the acupoint Zhongfeng opposing needling combined with routine needling group had better than the routine needling group and the acupoint Zhongfeng opposing needling group.There was no difference in efficacy between the routine needling group and the acupoint Zhongfeng opposing needling.Therefore,the acupoint Zhongfeng opposing needling combined with routine needling shows the effect of additive and intensive treatment.Conclusion1.The routine needling group,acupoint Zhongfeng opposing needling group and acupoint Zhongfeng opposing needling combined with routine needling group had obvious curative effect on periarthritis of shoulder.The three treatments can not only relieve the pain in patients with periarthritis of shoulder,but also improved the function of shoulder joint activity,Therefore,it can improve the quality of life of patients with periarthritis of shoulder,improve the quality of life and alleviate the anxiety of patients.2.On the base of the treatment of routine acupuncture,the acupoint Zhongfeng opposing needling enhanced curative effect.The acupoint Zhongfeng had obvious curative effect on periarthritis of shoulder,the effect of its superposition can be achieved by acupoint Zhongfeng opposing needling combined with routine needling.3.Both the acupoint Zhongfeng and routine needling can treat periarthritis of shoulder,and the clinical effect was significant.The clinical effect of the acupoint Zhongfeng opposing needling and routine needling on shoulder joint function and pain level of the patients with periarthritis of shoulder was similar.4.The acupoint Zhongfeng opposing needling combined with routine needling had special therapeutic effect on periarthritis of shoulder and should be popularized and applied in clinical treatment. |
