Objective:To make a research plan for the phenomenon and regularity of acupoint sensitization in the neck type of cervical spondylosis.To explore the characteristics and regularity of acupoint sensitization in the neck type of cervical spondylosis,and analyze the relationship between the occurrence of acupoint sensitization and disease.To provide reference for the selection of acupoints for clinical treatment of the neck type of cervical spondylosis.Methods:1.Using literature visualization analysis software CiteSpace to objectively and intuitively reveal research hotspots and development trends in the field of acupoint research.Current status of research on acupoint sensitization of cervical spondylosis using bibliometric research methods.Through the pre-test of acupoint sensitization of neck type of cervical spondylosis,the relevant information of the acupoint sensitization phenomenon of the carriers were collected,and the acupoint sensitization detection method,acupoint and detection area were determined.Preliminary make a practical and feasible research plan for the phenomenon and regularity of acupoint sensitization in the neck type of cervical spondylosis.To provide a methodological basis for the study of the phenomenon and regularity of acupoint sensitization in the neck type of cervical spondylosis and healthy people.2.A case-control study was conducted with 339 patients with neck type of cervical spondylosis and 227 healthy people as carriers.Starting from the three aspects of body surface sensation,skin and subcutaneous tissue morphological changes and biophysical properties,the sensitized acupoints and disease-related information were collected.To determine the correlation between acupoint sensitization(including sensitized performance,location,quantity,etc.)and the neck type of cervical spondylosis,and to study the incidence,type and performance characteristics of acupoint sensitization in the neck type of cervical spondylosis.Results:1.Methodological study on phenomenon and regularity of acupoint sensitization in neck type of cervical spondylosis(1)Using CiteSpace to explore the latest hot bursts in the study of acupoints: acupoint sensitization,the intensity was 71721,and the time of emergence was 2017.(2)The information labels clustered as "acupoint sensitization" include: infrared radiation(Freq: 16);pressure threshold(Freq: 13),etc.And the clustering results showed that "acupoint sensitization" was closely related to "cervical spondylosis".(3)Bibliometric studies had found that the phenomenon of sensitization of acupoints in cervical spondylosis was most common in heat(66),pain(46),and shape sensitive(15).At present,there were 28 acupoints involved in all sensitization phenomenon of cervical spondylosis,all of which were mainly acupoints in the neck.(4)The results of pre-test showed that the sensitization rate of the acupoint palpation was significantly higher than that of healthy people,the temperature was generally increased,and the mechanical pain and pressure were significantly decreased.These differences were statistically significant.Prompt that the research program was feasible.2.The case-control study of acupoint sensitization phenomenon for patients with neck type of cervical spondylosis(1)Temperature analysis of acupoint in patients with the neck type of cervical spondylosisThe highest temperature of all acupoints in patients with the neck type of cervical spondylosis was higher than that of healthy people(p<0.01).After the ROC curve was drawn,the cutoff values of acupoints such as Dazhui,Wangu,and Jianzhongshu were well defined.The area under the curve was >0.8 and the Youden index was >0.5.The sensitization rate of patients was higher than that of healthy people(p<0.01).Compared with the local acupoints of patients,the temperature cutoff value of the distal acupoints of patients was best defined,and the area under the curve was 0.741.The sensitization rate of the distal acupoints were higher than the local acupoints of patients.After logistic regression analysis,it was found that the shorter the course of the disease,the sensitive acupoints such as Dazhui,Dazhu and Fengchi were more likely to occur,the younger the age,Jianjin,Jianzhongshu,Wangu,and other points were more susceptible to be sensitized,and the lower the VAS score,Jianjin,Wangu,Fengchi,and other points were more susceptible to be sensitized,which were statistically significant.(2)Mechanical pain threshold analysis of acupoint sensitization in patients with the neck type of cervical spondylosisThe mechanical pain threshold of all acupoints in patients were lower than that in healthy people(p<0.01).After plotting the ROC curve,it was found that the cutoff values of Zhongzhu,Shousanli and Tianzong were better,the area under the curve was >0.7,and the Youden index was >0.36.The sensitization rate of patients was higher than that of healthy people(p<0.01).Compared with the local acupoint of the patient,the definition of the mechanical pain threshold cutoff value of the distal acupoint was the best,and the area under the curve was 0.731.The sensitization rate of the distal acupoint was slightly higher than the local acupoint.After logistic regression analysis,it was found that the longer the desk time,Wangu,Dazhui,and Jianjing,and other points were more susceptible to be sensitized.The longer the digital products were used,Wangu,Fengchi,and Dazhui,and other points were more susceptible to be sensitized.Women were more susceptible to sensitization in Jianzhongshu,Tianliao,and Shousanli than males,which were statistically significant(p<0.01).(3)Pressure pain threshold of acupoint sensitization in patients with the neck type of cervical spondylosisThe pressure pain threshold of all acupoints in patients was lower than that in healthy subjects(p<0.01).After plotting the ROC curve,the cutoff values of Shousanli,Houxi,and Wangu,and other points were better,the area under the curve was >0.700,and the Youden index was >0.350.The sensitization rate of patients was higher than that of healthy people(p<0.01).Compared with the local acupoint of the patients,the definition of the pressure pain threshold cutoff value of the distal acupoint was the best,and the area under the curve was 0.727.The sensitization rate of the distal acupoints was higher.After logistic regression analysis,it was found that the longer the desk time,Dazhui,Jianjing,and Dazhu,and other points were more susceptible to be sensitized.The younger the age,Wangu,Tianliao,and Fengchi,and other points were more susceptible to be sensitized.The lower the VAS score,Tianzhu,Jianwaishu,Tianzong,and other points were more susceptible to be sensitized.Women were more susceptible to sensitization than men in Shousanli,Tianzong,and Tianzhu,which were statistically significant(p<0.01).(4)Analysis of sensitive points in patients with the neck type of cervical spondylosisThe sensitive points were mainly distributed within a range of 1 cm from the center of the acupoint.The number of sensitive points distributed around the Jianjing was the most.The temperature,mechanical pain threshold,and pressure threshold of the patient’s sensitive points were strongly correlated with the measured indicators from the nearest acupoints,and the correlation coefficient between the measured indicators of the sensitive points within 1 cm and the nearest acupoints was the highest(most >0.9).(5)Related meridian analysis of acupoint sensitization in patients with the neck type of cervical spondylosisThe acupoints detected in the study are assigned to the meridians.The temperature of different meridians in patients was higher than that in healthy subjects(p<0.01).After the ROC curve was drawn,the Du meridian cutoff value was the best.The area under the curve was 0.809 and the Youden index was 0.582.The temperature sensitization rate of patients in different meridians was higher than that of healthy people(p<0.01).The mechanical pain threshold of patients with different meridians was lower than that of healthy people(p<0.01).After the ROC curve was drawn,the cutoff value of Taiyin meridian was best defined.The area under the curve was up to 0.738 and the Youden index was 0.365.The mechanical pain sensitization rate of different meridians in patients was higher than that in healthy people(p<0.01).The pressure threshold of different meridians in patients was lower than that of healthy people.After the ROC curve was drawn,the cutoff value of Taiyin meridian was the best.The area under the curve was 0.720 and the Youden index was 0.338.The pressure threshold sensitization rate of patients in different meridians was greater than that of healthy people.Conclusion:1.Acupoint sensitization was a hot research direction in the study of acupoints.The techniques commonly used in the study of acupoint sensitization were the study of acupoint temperature and pain threshold.2.The temperature of acupoints in patients with neck type of cervical spondylosis was higher than that in healthy people,and the mechanical pain threshold and pressure pain threshold were lower than those in healthy people.pressure pain sensitization may be a dominant sensitizing form of neck type of cervical spondylosis.3.The acupoints with three kind of high sensitization rates of acupoint sensitization were Houxi,Dazhui,and Shousanli,which were optimal sensitized points for neck type of cervical spondylosis.4.Acupoint sensitization of neck type of cervical spondylosis may be related to the location.Through the factor analysis of the three indicators of patient temperature,mechanical pain and pressure,the acupoints can be automatically classified into three types: the upper part of neck,the acupoint located at local part,and the distal part.The sensitization rate of the temperature and pressure pain threshold of the distal acupoint was significantly higher than that of the local acupoint of the neck.It was suggested that neck type of cervical spondylosis can be treated according to the principle of distal acupoint selection.5.Acupoint sensitization of neck type of cervical spondylosis may be related to meridians.The sensitization rate of each meridian in patients was higher than that in healthy people,and the temperature sensitization of Du meridian was the best.The mechanical pain and pressure sensitization of Taiyin meridian was best defined.6.Acupoint sensitization of neck type of cervical spondylosis may be related to gender,age,the course of the disease,condition,desk time and digital product usage time.
