Study On Acupoint Sensitization Phenomenon In Patients With Neck Type Of Cervical Spondylosis And Analysis Its Correlation Between Syndrome Differentiation According To Meridians

Objective:In this study,the patients with neck type of cervical spondylosis were taken as the research object and the palpation reaction,temperature and mechanical pain threshold of each acupoint were taken as the observation indexes.The manifestation,distribution and incidence of acupoint sensitization in patients with cervical spondylosis of different meridian syndrome were analyzed for exploring the regularity of acupoint sensitization phenomenon in patients with neck type of cervical spondylosis and finding its correlation between syndrome differentiation according to meridians.Finally,providing objective reference for the meridian diagnosis and treatment of patients with neck type of cervical spondylosis.Method:A cross-sectional study was conducted with 123 patients with neck type of cervical spondylosis who met the included criteria.The including patients were divided into four types according to the area of pain and meridian relationship as follows: meridian of du disease(43 cases),meridian of Taiyang disease(36 cases),meridian of Shaoyang disease(35 cases)and meridian of Yangming disease(9 cases).The detection points were choosed according to literature mining combined with common used points in treating neck type of cervical spondylosis.Observing the acupoint palpation reaction by artificial palpation,and detecting the temperature and mechanical pain threshold of each acupoint via Fotric226 platform thermography and electronic Von Frey esthesiometer at the same time.Excel was used to input the basic information and sensitization information test results of patients,and the input data were analyzed via SPSS statistical software.Result:1.Characteristics of acupoint sensitization phenomenon of patients with neck type of cervical spondylosis(1)For patients with neck type of cervical spondylosis,there was no significant difference in the sensitization of palpation,temperature and mechanical pain threshold between the healthy side and the affected side(P > 0.05).(2)The thermal sensitization rate of acupoints ranged from 8.54% to 44.72%(the average heat sensitization rate was 27.61%),and the pain sensitization rate of acupoints ranged from 45.93% to 80.89%(the average pain sensitization rate was 58.80%).(3)The top three acupoints with the highest thermal sensitization rate in patients with neck type of cervical spondylosis were Fengchi,Dazhui and Tianliao;the top three acupoints with the highest mechanical pain sensitization rate in patients with neck type of cervical spondylosis were Jianwaishu,Fengchi and Dazhui.(4)The palpation sensitization rate of meridians ranked from high to low: Yangming meridian > Shaoyang meridian > Taiyang meridian > Taiyin meridian > Du meridian(P < 0.001);The thermal sensitization rate of meridians ranked from high to low: Du meridian > Shaoyang meridian > Taiyin meridian > Yangming meridian > Taiyang meridian(P < 0.001);The pain sensitization rate of meridians ranked from high to low: Yangming meridian > Du meridian > Taiyang meridian > Shaoyang meridian > Taiyin meridian(P < 0.001).2.The correlation between acupoint sensitization phenomenon of patients with neck type of cervical spondylosis and syndrome differentiation according to meridians(1)Both of the palpation sensitization rate and the pain sensitization rate of Jianjing and Tianliao in the patients with Shaoyang meridian sick were significantly higher than those of the patients with other syndromes(P < 0.05);both of the palpation sensitization rate and the pain sensitization rate of Tianzong in the patients with Taiyang meridian sick were significantly higher than those of the patients with other syndromes(P < 0.05).(2)The top three acupoints with the highest thermal sensitization rate when Yangming meridian sick were Tianliao,Fengchi and Jianjing;the top three acupoints with the highest highest thermal sensitization rate when Du Meridian sick were Fengchi,Tianliao and Dazhui;the top three acupoints with the highest highest thermal sensitization rate when Shaoyang meridian sick were Fengchi,Dazhui and Lieque;the top three acupoints with the highest highest thermal sensitization rate when Taiyang meridian sick were Dazhui,Zhongzhu and Tianliao.(3)When Yangming meridian was sick,the top three acupoints with the highest mechanical pain sensitization rate were Jianwaishu,Shousanli and Dazhu;when Du meridian was sick,the top three acupoints with the highest mechanical pain sensitization rate were Jianwaishu,Dazhui and Fengchi;when Shaoyang meridian was sick,the top three acupoints with the highest mechanical pain sensitization rate were Jianwaishu,Fengchi and Jianjing;when Taiyang meridian was sick,the top three acupoints with the highest rate of mechanical pain sensitization were Jianwaishu,Houxi,and Fengchi.(4)No matter which meridian was sick,the thermal sensitization rate of Du meridian was significantly higher than other meridians(P < 0.05);no matter which meridian suffered from disease,the mechanical pain sensitization rate of diseased meridian was higher than non-diseased meridians(P < 0.05).Conclusion:1.Under the condition of neck type of cervical spondylosis,the healthy side of the patient would appear acupoint sensitization phenomenon either,and the sensitization degree was equivalent to the affected side.In clinical diagnosis and treatment,same attention should be payed to both the healthy side and the affected side.2.For patients with the neck type of cervical spondylosis,the pain sensitization rate of acupoints was higher than thermal sensitization rate of acupoints.The pain sensitization may be an advantaged sensitizing form of the neck type of cervical spondylosis;the acupoints with both high heat sensitization rate and high pain sensitization rate were Fengchi and Dazhui,which were the optional sensitized points for the neck type of cervical spondylosis.3.Pain sensitized points could assist the diagnosis of both disease and syndrome in patients with neck type of cervical spondylosis,Jianjing and Tianliao might be the specific reaction points for the diagnosis of Shaoyang meridian disease;Tianzong may be the specific reaction point for the diagnosis of Taiyang meridian disease.4.The acupoint sensitization phenomenon in neck type of cervical spondylosis owned conspicuous distribution characteristics along meridians.Du meridian may be the dominant meridian of thermal sensitization in patients,and the thermal sensitized points on Du meridian could be optimal choose in clinical treatment;the diseased meridian may be the dominant meridian of pain sensitization in patients,and the pain sensitized points on diseased meridian could be selected preferentially in clinical treatment based on syndrome differentiation.
