Objective: Compared prostate imaging quality in perineal approach from four different ultrasonic probes,explored and identified a systematic transperineal ultrasound-guided prostate biopsy procedure.Applied this procedure on the patients undergoing abdominoperineal resection(APR)or with anal stenosis,analyzed and evaluated its feasibility,effectiveness and safety for prostate cancer.Methods: 1.Use four different ultrasonic probes in perineal approach in 14 healthy volunteers,evaluate its imaging qualities and analyze the relevant factors.2.From March 2011 to March 2015,the convex array probe in perineal approach imaging prostate was used in 8 patients with increased serum PSA levels who have undergone APR or anal stenosis and had no access to the traditional prostate biopsy,explore and identify a systematic transperineal ultrasound-guided prostate biopsy procedure.3.The transperineal ultrasound-guided 6+ x transperineal prostate biopsy by convex array probe in 8 patients as mentioned was performed.Analyze the pathologies and complications.Follow up the outcomes;discuss the effectiveness and safety of this procedure.Results: 1.Comparing prostate imaging quality in perineal approach from four different ultrasonic probes,the convex array probe can show the prostate and the surrounding tissues(such as the bladder neck,seminal vesicles)more clearly.2.The convex array probe in perineal approach imaging prostate in patients who have undergone APR or anal stenosis can show good images of prostate.A transperineal ultrasound-guided 6+ x transperineal prostate biopsy procedure was performed.3.Pathological diagnosis shows that prostate cancer was detected in 5 cases,metastatic carcinoma of the rectum in 1 case,benign prostate hyperplasia and prostate chronic interstitial inflammation each in 1 case.No severe complications were found.The follow-ups ranged from 13 months to 61 months with an average of 37 ± 18.44 months,two patients were died.The results of biopsy are correct.Conclusion: In patients who have undergone APR or anal stenosis with elevated PSA,transperineal ultrasound-guided 6+x biopsy of the prostate by convex array probe can provide an accurate method for pathological diagnosis.
