Part One Comparison of the efficacy of phase Ⅱ home-based cardiac rehabilitation and traditional center-based cardiac rehabilitation in patients with acute myocardial infarction after percutaneous coronary interventionObjective: The objective of this study was to compare the efficacy of phase Ⅱ home-based cardiac rehabilitation with traditional center-based rehabilitation in patients with Acute myocardial infarction(AMI)after Percutaneous coronary intervention(PCI)treatment.To provide it is a safe,effective and convenient cardiac rehabilitation treatment program for AMI patients after PCI,and it can promote the development of rehabilitation treatment methods and improve the prognosis of the disease.Methods: A total of 80 patients with acute myocardial infarction after PCI in Cangzhou Central Hospital from September 2019 to March 2020 were selected as subjects and divided into observation group and control group by random method,with 40 cases in each group.The control group received traditional center-based rehabilitation therapy,while the observation group received remote home-based rehabilitation therapy under the guidance of remote ECG monitoring system.The treatment intervention lasted for 3months in both groups.LVEF,LDL-C,treatment compliance,cardiovascular event rate,quality of life score,and 6-minute walking test were compared between the two groups.Results: There were no deaths or cardiovascular events in either group.There were no statistically significant differences in patients with re-infarction,heart failure,unstable angina,unplanned readmissions,medication adherence,left ventricular ejection fraction,and the initial6-minute walk test within 6 months of discharge(P>0.05).There were statistically differences in exercise adherence,LDL-C cholesterol and quality of life(P< 0.05);There was a significant difference in the 6-minute walk test at the third and 6th months after discharge between the two groups(P< 0.05).Conclusion: Home-based rehabilitation is a new cardiac rehabilitation mode that is efficient,convenient,full-process monitoring and convenient follow-up.It can effectively improve the cardiac function,cardio-respiratory fitness and quality of life among AMI patients,improve their self-management awareness and rehabilitation compliance,reduce the risk of cardiovascular events,and have a positive impact on the survival prognosis and rapid recovery of AMI patients.Part Two Comparison of the efficacy of phase Ⅱ home-based cardiac rehabilitation and traditional center-based cardiac rehabili- tation in patients with heart failure after percutaneous coronary stent surgery for acute myocardial infarctionObjective: To analyze the application effect of phase Ⅱ remote home cardiac rehabilitation in improving cognitive ability and satisfaction of patients with heart failure complicated by percutaneous coronary stenting(PCI)after myocardial infarction.Methods: The study subjects selected 100 patients with myocardial infarction who came to our hospital from September 2020 to October 2022,and divided them into observation group(n=50)and control group(n=50)according to the random number table method,all of whom underwent PCI and were complicated by heart failure with preserved LVEF after surgery.We implemented stage-Ⅱ-remote-home-based cardiac rehabilitation in the observation group,and traditional center-based cardiac rehabilitation in the control group,and the interventions last for 3 months in both groups.Then we compared the improvement of cardiac function,cognitive ability and satisfaction before and after cardiac rehabilitation between the two groups.Results: A comparison of baseline data and BNP between the two groups before the intervention showed that the difference between the two groups was not statistically significant(P>0.05).However,after the intervention,the level of LVEF was significantly higher than that before the intervention,while the LVESD and LVEDD of the observation group were significantly lower than that before rehabilitation,and the level of the observation group was also significantly lower than that of the control group,the differences were statistically significant(P<0.05).The total improvement rate of cognitive ability in the observation group was 80.00%,with statistical significance(P<0.05).There was no significant difference in the 6-minute walking distance between the two groups of participants at the first month after recovery(P>0.05),but the difference was statistically significant at the third and sixth months(P<0.05).There were significant statistical differences in the quality of life of the two groups within 6 months after discharge(P<0.05).Conclusion: Compared with traditional center-based cardiac rehabilitation,phase Ⅱ remote home-based cardiac rehabilitation can increase the cardio-respiratory fitness of patients compared with center-based rehabilitation,thereby effectively improving the cardiac function of patients with heart failure after PCI,and improving the quality of life and cognitive ability of patients. |
