ObjectiveOn patients with dilated cardiomyopathy (DCM), after the merger subclinical hypothyroidism (SCH), the changes of its heart function and blood lipids、platelet parameter’s changes, understand the correlation of subclinical hypothyroidism with heart function、blood lipids、platelet parameters in patients with dilated cardiomyopathy; To further investigate whether the subclinical hypothyroidism for dilated cardiomyopathy patients have certain clinical evaluation function or predictive value, which can provide some new clues for clinicians in further probe into the pathophysiology of DCM, the control of risk factors, prevention and treatment.MethodsAll cases were inpatients during august of2012to february of2014in the department of Cardiology of Northern Jiangsu People’s Hospital,126case DCM patients. According to match different subclinical hypothyroidism is divided into the combining subclinical hypothyroidism group (hereinafter called the combining SCH group)19cases, Don’t combining subclinical hypothyroidism group (hereinafter referred to as the control group)107cases. All the cases accord with DCM and subclinical hypothyroidism diagnosis standard. Record to be included in the patient’s general situation, the New York heart association (NYHA) classification, thyroid function, blood lipids platelet parameters and clinical data such as ultrasonic cardiogram compare each index, combination of SCH group compared with control group statistical analysis of the above indexes, the corresponding conclusions.Results1.The morbidity of NYHA class Ⅳ patients in combining SCH group was obviously higher than that of control group (63.2%VS43%, P=0.005), ultrasonic cardiogram compare each index between the two groups have no statistical differences (P>0.05)2.Merger of SCH group compared with control group, the High density lipoprotein cholesterol (HDL) significantly decreased (P=0.045), individuation cardiac function classification, control the factors affecting of cardiac function, only in the NYHA class IV level, HDL significantly decreased (P=0.00)3.Merger of SCH group compared with control group, Platelet distribution width (PDW) significantly increased (P=0.021), Mean platelet volume (MPV) significantly increased (P=0.031).After an individuation cardiac function classification, only in the NYHA class IV level, MPV in group SCH was higher than control group obviously (P=0.024)Conclusion1. In patients with dilated cardiomyopathy, subclinical hypothyroidism easier found in NYHA class IV patients.2. Subclinical hypothyroidism has obviously effects to HDL in patients with DCM, And which is associated with patients’ heart failure degree,with the heavier as the degree of heart failure,the HDL level lower trend.3. Subclinical hypothyroidism has certain effects to PDW and MPV in patients with dilated cardiomyopathy, after an individuation cardiac function classification(NYHA), in the NYHA class IV level, SCH is associated with the rise of MPV.
