Recently, psoriasis is recognized as a chronic inflammatory systemic disorder, which is associated with several systemic diseases. Many epidemiologic studies with varied designs link psoriasis to systemic metabolic comorbidities such as obesity, dyslipidemia, hypertension and metabolic syndrome. Erythrodermic psoriasis is a kind of rare and special types of psoriasis which can seriously affected the patient’s life quality. The pathogenesis of erythrodermic psoriasis is unclear, the relationship with other types of psoriasis and outcome have no unified agreement. Rare research focus on the clinic and laboratory features of metabolic syndrome.ObjectiveTo understand and analyze clinic and laboratory features of metabolic syndrome in erythrodermic psoriasis, found that the correlation of erythrodermic psoriasis and clinic and laboratory features of metabolic syndrome, to provide clinicalguidance and the research basis of pathogenesis.MethodsWe performed a case-control study including patients with erythrodermic psoriasis hospitalized in Anhui Provincial Hospital from Jan 2001 to Dec 2014 and sex and age matched patients with psoriasis vulgaris and controls. And then we compared the blood pressure, fasting blood glucose, serum lipid and uric acid between three groups.ResultsThis study recruited 55 patients with erythrodermic psoriasis, age and sex matched 62 patients with psoriasis vulgaris and 55 healthy controls. Erythrodermic psoriasis compared with control, BMI, systolic pressure, diastolic pressure, fasting blood glucose and triglycerides were no obvious difference. The uric acid of erythrodermic psoriasis patients is higher than the control group, the blood calcium and blood cholesterol of erythrodermic psoriasis is lower than the control group, the difference is statistically significant (p< 0.01). Total number of white blood cells, the number of neutrophil and monocyte in erythrodermic psoriasis group is higher than the control group, the difference has statistical significance (p< 0.01).The results of binary logistic regression analysis indicated that the uric acid, the blood calcium were significatnely correlation with the erythrodermic psoriasis (P< 0.05).Systolic pressure, total number of white blood cells, the number of neutrophil, the number of monocyte and blood cholesterol were significantly correlation with of the psoriasis vulgaris (p<0.05).ConclusionThe clinic and laboratory features of metabolic syndrome and the distribution of blood cell in erythrodermic psoriasis patients were abnormal. The distribution of blood cell and some laboratory features of metabolic syndrome were correlation with the erythrodermic psoriasis and psoriasis vulgaris.
