Clinical Study On Treatment Of Chronic Hand Eczema With Blood Deficiency And Deficiency Syndrome By Qufeng Huoxue Lotion Combined With Jianpi Yangxue Decoction

Chronic Hand Eczema (CHE) is a common dermatology disease which featuring locolized atopic disease, with that female has higher tendency of onset than male. CHE is also known as "WO Chuang" and "E Zhang Feng" in traditional Chinese medicine. CHE, has clinical characteristic of infiltration, hypertrophy, thickening, desquamation. prolong course of disease, irritation and reoccurrence which is difficult to treat. CHE often needs a long time of conventional treatments in clinical practice, while long term intake of anti-allergic drugs makes the patients unresponsive to the treatment or even rebound after withdraw, and keratolytics application sometimes stimulate the inflammation of skin and induce the symptoms. Besides, steroid and emollient application has limited effect due to the thickening skin of the lesioa TCM has advantage in the treatment for CHE. external application is a common therapeutic option---especially herbal decoction immersion and wash therapy. This research focus on treatment of "Blood deficiency and wind dryness syndrome" CHE with Qu Feng Huo Xue Fang (dispel wind and promote blood flow) herbal decoction immersion wash therapy incorporate with Jian Pi Yang Xue granules (strengthening spleen and nourish blood) prescription oral intake combine with mixture of Qingpeng ointment and sicorten external application. The goal is to find a therapeutic method that could rise the effectiveness of CHE treatment.ObjectiveClinical observation of Qu Feng Huo Xue wash combine with Jian Pi Huo Xue granules i n treating chronical hand eczema of blood deficiency and wind dryness syndrome.MethodA randomized controlled study divided 46 patients into therapeutic group (22 patients) and control group (24 patients). Control group patients were given herbal decoction oral treatment along with ointment external application:Jian Pi Yang Xue granules. (1 pack/use, bid); ointment for external application with 1:1 mix ratio of Qing Peng ointment with sicorten ointment (2/day). Therapeutic group has additional Qu Feng Huo Xue Fang herbal decoction immersion wash therapy for 30 minutes before ointment application. Both groups has treatment schedule set for 4 weeks---at the time point of before treatment.2 weeks into treatment and treatment completion, HESCI score, degree of irritation and pain evaluation were conducted along with patients feedback of side-effect.ResultAmong 46 cases of CHE patients,65.22% of total patients were between age 40 to 69. and female patients occupied 67.39% of total specimens. The course of disease were between 2 months to 20 years, with average course of disease of 5.05±5.10 yr. Patients with family history of allergic disease occupied 28.3% of total specimen, and patients with personal allergic disease history or food/drug allergy occupied 22% of total specimen. The result shows therapeutic group shows 77.27% total effectiveness compare to that of the control group’s 58.33%. Therapeutic group shows better treatment effect than control group (P<0.05). In HECSI. among all symptom factors (spiloplaxia. keratinization. rhagadia and area of lesion) and subjective symptoms (irritation, pain) evaluation shows that at 2 weeks into treatment, therapeutic group shows better recovery on rhagadia. irritation and pain than the control group (P<0.01 or P<0.05) whereas spiloplaxia. keratinization and area of lesion shows no significant statistical differences between the two groups (P>0.05). At the completion of treatment the rhagadia factor shows better recovery on therapeutic group (P<0.05) whereas spiloplaxia, keratinization, area of lesioa irradiation and pain factors show no significant statistical differences (P>0.05). No obvious side-effect reported on both groups.ConclusionQu Feng Huo Xue wash has the function of dispel wind to soothe irritation, promoting blood flow and nourishing blood; and the effect of removing dampness and dispel stagnation. Clinical study shows that Qu Feng Huo Xue Fang herbal decoction immersion wash therapy has satisfying result in treating CHE of blood deficiency and wind dryness syndrome with no obvious side-effect. It resolves rhagadia. irritation and pain relatively faster and has even more satisfying result on treating rhagadia.
