Periappendiceal abscess is a kind of acute abdomens which is secondary to the acute appendicitis and the serious pathological types of it. Typical Periappendiceal abscess symptoms include a shifting pain in right lower quadrant which cannot remission, fevering, some digestive tract symptoms like nausea and abdominal distension and feeling impatient. Physical examination shows:muscular tension at right lower quadrant, tenderness and rebound tenderness at McBurney point, palpated masses in right lower quadrant, shifting dullness(-), Rovsing’s sign(+), and obturator sign(+). At the same time, in Peripheral Blood, the WBC count and the percentage of neutrophils will increase significantly. And it will find some mixed echogenic masses during B ultrasonic scanning. Conventional wisdom says that acute periappendiceal abscess should be given the conservative treatment with antibiotics unless the pyemia or acute diffuse peritonitis happened. Even if the patients need emergency operation, the best way is the drainage operation. In order to avoid the missed diagnosis of ileocecal tumor and recurrent appendicitis, patients will be required to get appendectomy 3 to 6 months later after infection is under control. However, as far as the cooperation of TCM and Western medicine, integrative medicine therapy is more and more popular in the treatment of periappendiceal abscess.Objective:To observe the different curative effect between using Ruyi Jinhuang ointments externally and conventional western medicine treatment, and discusses its possible mechanism of action for clinical promotion.Method:Since September 2011 to December 2014,74 patients with periappendiceal abscess were chosen from the general surgery of Dongzhimen Hospital, and divided into the treatment group and the control group of 37 cases randomly. The control group was treated with routine western medicine plus saline hydropathic compress, while the treatment group was treated with Ruyi Jinhuang ointments externally. A course of these medical treatments is 7 days, and the whole observation period is two courses. Observes that how long the patient body temperature returned to normal and the abdominal pain disappeared will take. Besides, record the WBC count and the percentage of neutrophils in Peripheral Blood and the size of abdominal masses. According to the above factors, find the differences between two groups.Results:1. The time of body temperature returned to normal, the time of the abdominal pain disappeared and the time of the masses dissipation in the treatment group is shorter than that is in the control group. After statistics processing, two groups has significance differences (P<0.05).2. The effect on the WBC count and the percentage of neutrophils in Peripheral Blood of two groups is Equivalent. After statistics processing, between the treatment group and the control group, there is no significance differences (P>0.05).Conclusion:Based on the conventional Western medicine, the treatment by using Ruyi Jinhuang ointments externally can quickly relieve symptoms of Periappendiceal abscess such like the abdominal pain and fever, shorten the masses dissipation and absorption. This treatment has an obvious curative effect and no obvious toxic effect for clinical promotion. Furthermore, we should do some further studies about improving the transdermal absorption of external Chinese medicine, which will lead to better clinical curative effect.
