Objective:1 Literature research:Analyzed and summarized ancient Chinese literature discussed the commonly used drugs, compatibility rules, syndrome differentiation theory and treatment methods of headache. Analysis the prescription and origin of the treatment on headache. Support and provide guidance on the clinical study and headache treatment.2 Clinical observation:To observe the clinical curative effect of the traditional Chinese medicine decoction of master Han in the treatment of primary headache. Hope to evaluate the effectiveness of traditional Chinese medicine decoction through this study. And do the preliminary foundation work for the future research and development toward the hospital preparation.Methods:1 Literature research:The study collected literatures from the Han Dynasty to the Qing Dynasty (1911). Using the computer retrieval method with the combination of manual retrieval method. Computer retrieval source is the Encyclopaedia of traditional Chinese medicine database. Artificial retrieval and manual re-examination from Beijing University of Chinese Medicine library books. Establish database and ues statistical methods for data processing. Use TCM theory to analysis statistical results.2 Clinical observation:The study collected 50 patients with primary headache. Use the traditional Chinese medicine decoction of master Han and add or subtract permits according to the symptoms. Observe the headache-related symptom scores, TCM syndrome scores before treatment,3 days after treatment,7 days after treatment and after 1 months follow-up. Establish database and use statistical methods for data processing and statistical results analysis.Results:1 Literature research:The research received 652 prescriptions,4272 times of drug use frequency in total.(1) There are12 prescriptions before and including the Han Dynasty, accounting for 1.84%, frequency of drug use is 57 times, accounting for 1.33%.Ginger, licorice (sunburn) is the most commonly used drugs. Warmth and level properties, pungency and sweetness tastes are most commonly used. (2) The study collected 54 prescriptions from the Wei Jin Southern and Northern Dynasties, accounting for 8.28%, frequency of drug use was 281 times, accounting for 6.58%. Ginger, licorice (fried) is still the most commonly used drugs in this period, followed by Ramulus Cinnamomi, Radix Angelicae Pubescentis. Prescription for cold and heat property is balanced. Pungency and Warmth medicinal is most commonly used, followed by bitterness and level, sweetness and level, sweetness and warmth medicinal. (3) The study collected 54 prescriptions from the Sui and Tang Dynasties, the Five Dynasties and Ten Kingdoms period, accounting for 16.26%, frequency of drug use was 782 times, accounting for 18.31%. Cinnamon Twig, licorice, ginger (fried) still occupy the most commonly used drug in the top three. Pungency and Warmth medicinal is most commonly used, followed by sweetness and level, sweetness and warmth, bitterness and level medicinal. (4) Prescriptions of Song Jin yuan Ming and Qing Dynasties are 480, accounting for 73.62%, frequency of drug use was 3152 times, accounting for 73.78%. Licorice as the assistant medicinal is most commonly used, followed by Ligusticum wallichii, Plaster, Radix sileris and Notopterygium. Pungency and Warmth medicinal is still most commonly used. Compared with the previous period, the medicinal is more abundant, animal drugs also have more use.(5) From the aggregated data, ancient physicians most often use pungency and warmth medicinal in treating headache in every period, such as Cassia twig, Ginger, Ligusticum wallichii, Radix sileris and Notopterygium. Besides, Pungency and cold medicinal such as Plaster, Licorice as the assistant medicinal use a lot, too.2 Clinical observation:(1) The study group of patients with primary headache was more commonly seen in women, the ratio is 1:1.63. The age of onset interval concentrated in the 20-59 years old, with 40-49 year old was the largest, the number of cases decreased gradually over the age of 60. There is a family history of headache accounted for 22% of the total; Smoking and alcohol history accounted for 20% and 16%.(2) The study group of patients with primary headache average course for (3.20+36.77) years, The largest proportion is 1-6 months between. There is a clear basic diseases accounted for 66%, with cervical vertebra disease sees more most(44%), followed by Hypertension(38%), Stroke(12%), Hyperlipidemia(18%), Anxiety and depression(8%), Diabetes mellitus(14%).(3) The research group of primary headache patients with migraine to most Western Medicine Branch type(36%)/followed by tension-type headache(52%), cluster headache(8%),other primary headache(4%).(4)The research group of headache in TCM Syndromes of patients with the most common to wind pattern(46%), followed by fire-heat pattern(12%),dampness-phlegm pattern(8%),blood stasis pattern(10%),depressed pattern(4%),qi deficiency pattern(8%),blood deficiency pattern(2%),yin deficiency pattern(4%),yang deficiency pattern(6%).The effective rate 3 days after treatment,7 days after treatment and after 1 months follow-up were 22%,84% and 98%. The headache-related symptom scores, TCM syndrome scores both improved compared with scores before treatment with statistical significance (P<0.01).wind pattern, dampness-phlegm pattern, blood stasis pattern, depressed pattern,qi deficiency pattern,yang deficiency pattern improved with statistical significance 7 days after treatment; wind pattern, fire heat pattern, dampness phlegm pattern, blood stasis pattern, depressed pattern,qi deficiency pattern,yang deficiency pattern improved with statistical significance after 1 months follow-up.Conclusions:1 Literature research:Ancient Chinese medicine on prescription headache have come down in one continuous line points. Ancient physicians most often use pungency and warmth medicinal in treating headache in every period, show the theory of wind-caused headache. Licorice as the assistant medicinal is most commonly used in different age, shows the monarch system thinking in TCM.The average medicinal used in prescriptions of treating headache is 6.55. Prompt us use logical and well proportioned prescriptions to enhance clinical curative effect, reduce social medical expenses and reduce economic burden of patients.2 Clinical observation:Clinical study shows the traditional Chinese medicine decoction of master Han is useful in treating primary headache and treat TCM Syndromes and patterns. No side effects observed, the compliance of patients is fine. It is a safe and effective method of treatment, no side effect observed. Can be Widely use in clinical application.
