Experimental Study On Comprehensive Treatment Of Steroid - Induced Avascular Necrosis Of Femoral Head With Integrated Traditional Chinese And Western Medicine

Objective:To explore the mechanism of steroid-induced ischemic necrosis of femoral head(SINFH)of rabbits.Methods:28 rabbits were randomly divided into model group(n=14) and blank group(n=14).The rabbits of the model group were injected lipopolysaccharide(10ug/kg) and dexamethasone sodium phosphate(10mg/kg).The rabbits of the blank group were injected same dose normal saline. After 6 weeks, in the model group, Osteonecrosis of the femoral head was confirmed with magnetic resonance imaging (MRI).The levels of TNF-α、IL-2、IFN-γ in serum of the rabbits of the two groups were examined with ELISA method. The cholesterol(TC) and triglyceride(TG) were measured in the machine of HITACHI7600-020. Pathological changes of femoral head were observed with light microscope.Results:the rabbits of model group were in low spirits poor appitate、gait decayed when started work in the first week. After 6 weeks,we found the femoral head in low and hight signal change within 12 rabbits(12/14,85.7%) with MRI, but there were nothing in blank group. Bone trabeculars of the model group were observed become thinner, disruption, a lot of empty lacunae of bone cells and fat cells. The numbers of empty bone lacunae and levels of TNF-α、IL-2、IFN-γ、TC、TG in model group were significantly higher than that in blank group (P<0.05).Conclusion:the rise of TNF-α、IL-2、IFN-γ TC、TG may be the important factors in SINFH.Objective:To study the mechanism of western medicine combined with ShuXuetong injection Ossotide injection^ Methy lenediphosphonate injection for treatment of the femoral head necrosis induced by dexamethasone sodium phosphate.Methods:80 rabbits were divided randomly into A group(n=8),B group(n=72). The rabbits of the B group were injected lipopolysaccharide (lOug/kg) and dexamethasone sodium phosphate(10mg/kg).After 6 weeks, Osteonecrosis of the Femoral Heads were confirmed with magnetic resonance imaging (MRI).And then B group were randomly divided into B1,B2,B3,B4,B5,B6 groups.The level of TG,IL-2,TNF-a,IFN-y and the number of empty lacuna in the femoral heads were observed on the rabbits before treated and after treated in 2,8,14 days.Results:1.thigh bone trabecular bone and bone cell structure of A Group were the normal form, femoral head bones of B1 group become thinner and sparse, subchondral trabecular bone atrophy, interruption,nucleus pycnosis, accumulated a large number of fat cells in the bone marrow. A lot of the normal bone cells in the bone trabecular were disappeared. Degree of pathological changes of B2, B3 and B4 group slightly lighter compared with B1 group, but there were still a lot of fat cells in medullary cavity.B5 and B6 group observed bone absorption,trabecular bone repair, adipose cell diameter decreases, and the empty pit rate decreased significantly compared with group B1.Empty pit rate of A group compared with group B1, B2, B3, B4, B5 difference, compared with B6 group no difference.2.The level of TG and TNF-a in A group was close to G group, but significantly lower than those in the B1,B2,B3,B4,B5groups. The level of IFN-y in B1 group was closely to B2,B3,B4groups, but higher than those in the A,B5,B6groups. The level of IL-2 in B1 group was close to B2,B3,B4,B5,B6 groups, but significantly higher than A group.Conclusion:The three medicines intervention or combined therapy can reduce TG, IL-2,TNF-a,IFN-y,repair the femoral head necrosis and make the active ostogenic taking place.
