As of 2013, there are over 382 million people worldwide suffer from diabetes, of which China ranks first in the number of patients globally, among which type 2 diabetes accounts for more than 90%. Diabetes has become a major threat to the health of people. Prevention and treatment of diabetes is very urgent. The theory of constitution, as an important part of " preventive treatment of disease " theory of Chinese medicine, progressively valued by the people. Some scholars have proposed that constitution plays an important role in the occurrence and progression of diabetes and its complications. Therefore, the research of diabetes and its complications from a constitutional point of view is valuable. The constitutional classification of three yin and three yang was put forward by Professor Zhao Jinxi who researched the theory in classics and combined with the clinical practice. It will be very helpful to guide the clinical therapy if we could combine the differentiation of constitutions, diseases and syndromes well.Objective:To investigate the relationship between the constitution before illness and characteristics of TCM Syndrome in illness by patient questionnaire. To preliminarily explore the relevance between Jueyin constitution and its subtypes and TCM syndrome characteristics on the basis of preliminary studies on constitution, thus revealing the clinical features of type 2 diabetes patients with specific constitution. To provide the clinical research data for treating diabetes through the diagnosis and treatment model of the "trinity" including constitution, disease and syndrome differentiation.Method:On the basis of preliminary studies on constitution, a correlation analysis of type 2 diabetes patients with constitution before illness and TCM syndrome characteristics in illness is made. 71 cases of patients with Jueyin constitution and 22 cases of patients with Yangming constitution were received through filling in "Preliminary scale based on three yin and three yang theory". The general information was collected and the syndromes were judged by "TCM Syndrome Differentiation Standard of diabetes". Compared the syndromes frequency of the two groups, and then made a correlation analysis on the items with statistical differences. The data were analyzed by SPSS 17.0. Count material with the chi-square test, non-normal measurement data with rank sum test, and Logistic regression analysis was used in correlation analysis.Results:(1) In this study, TCM syndromes of Type 2 diabetic patients with Jueyin constitution occurs as a percentage of descending order of frequency are:wind yang (62.0%), yin deficiency (57.7%), qi deficiency (50.7%), damp-heat (33.8%), blood stasis (33.8%), accumulated heat (29.6%), stagnant heat (28.2%), yang deficiency (25.4%), qi stagnation (18.3%) and phlegm-dampness syndrome (16.9%).Those with Yangming constitution occurs as a percentage of descending order of frequency are:accumulated heat (63.6%), qi deficiency 54.5%), blood stasis (45.5%), yin deficiency (40.9%), yang deficiency (31.8%), stagnant heat (22.7%), damp-heat (18.2), phlegm-dampness (9.1%), air stagnation (9.1%) and wind yang syndrome (9.1%). Wind yang syndrome and accumulated heat syndrome have a statistically significant difference between the two groups of different constitutions. Wind yang syndrome and Jueyin constitution was positively correlated (B= 2.791, OR= 16.296). Accumulated heat syndrome and Jueyin constitution was negatively correlated (B=-1.427, OR= 0.240).(2) In this study, TCM syndromes of Type 2 diabetic patients with Jueyin A constitution occurs as a percentage of descending order of frequency are:wind yang (53.1%), yin deficiency (49.0%), qi deficiency (44.9%), blood stasis (36.7%), accumulated heat (32.7%), damp-heat (32.7%), yang deficiency (28.6%), stagnant heat (26.5%), phlegm-dampness (18.4%) and qi stagnation syndrome (16.3%). Those with Jueyin B constitution occurs as a percentage of descending order of frequency are:wind yang (81.0%), yin deficiency (76.2%), qi deficiency (61.9%), damp-heat (33.3%), stagnant heat (28.6%), qi stagnation (23.8%), blood stasis (23.8%), accumulated heat (19.0%), yang deficiency (14.3%) and phlegm-dampness syndrome (14.3%). Wind yang syndrome and yin deficiency syndrome have a statistically significant difference between the two groups of different subtypes. Yin deficiency syndrome (B=-1.204, OR= 0.300), wind yang syndrome (B= 1.324, OR= 0.266) and Jueyin A constitution was negatively correlated.(3) There was no statistical difference on general information between two groups of patients with Jueyin and Yangming constitution. The duration of diabetic patients with Jueyin A and B constitution have a statistically significant difference.Conclusion:(1) Type 2 diabetic patients included appear more qi deficiency and yin deficiency syndrome in deficiency syndromes. Besides, accumulated heat and blood stasis syndrome is common in excess syndromes. (2)In this study, Type 2 diabetic patients with Jueyin constitution exhibit wind yang, yin deficiency and qi deficiency syndromes often. Patients with Yangming constitution express not only as yin deficiency and qi deficiency syndrome, but also accumulated heat and blood stasis syndromes. Type 2 diabetic patients with Jueyin constitution appear more wind yang syndrome and less accumulated heat syndrome than other people. It is proved that there is a convergence between TCM syndrome in illness and constitution before illness, namely the existence of constitutional "Conghua" phenomena. (3) Type 2 diabetic patients with Jueyin B constitution appear more wind yang and yin deficiency syndrome than Jueyin B constitution. It may be related to the characteristic of the subtype itself, which can be summarized as yin deficiency and yang excess. This could be considered as a more specific manifestation of the constitutional "Conghua" phenomena.
