Clinical Observation On Effect Of Sangqi Qingxuan Granule On Primary Hypertension And Vascular Endothelial Function

This article includes two parts:literature survey and clinical investigation.First, literature surveyDiscusses the understanding and study of western medicine and traditional Chinese medicine on hypertension pathogenesis, progress of vascular endothelial dysfunction, and the basis of compound prescription Sang-qi granule and previous research results. Hypertension, possibly caused by pathological damage and the function of endothelial cell structure disorder would further lead to the damage of endothelial function through shear stress, inflammation and stress mechanism, which forms a vicious spiral.Previous studies have shown that the mechanism of Sang-qi granule may increase the NO levels,reduce endothelin-1 levels, increase the calcitonin gene related peptide levels, lower blood pressure and improve the function of vascular endothelium; reduce blood fat, especially cholesterol and low density lipoprotein cholesterol, elevate SOD, decrease hsCRP, anti lipid peroxidation of certain effect, which could improve the lipid metabolism, prevent hardening of the arteries, reduce the damage of vascular endothelial.The research results provide a theoretical basis of Sang-qi granule lowering the blood pressure, improving the vascular endothelium. This part summarizes the research on the pathogenesis of hypertension and vascular endothelial function injury, as well as the etiology and pathogenesis, syndrome differentiation, treatment and other aspects of researches on TCM,the basis form of prescription and the former study of Sang Qi Qingxuan granule.Second, clinical investigationObjective:To investigate the effect of Sang-qi granula in decreasing blood pressure and avascular endothelial function.Method:Collect 102 patients with hypertension,and privide free Sang-qi granula for 8 weeks.Ask the patient come to get the Sang-qi granula every 2 weeks,and give them the blood pressure table in order to write down the blood pressure, heart rate and time.Ask the patient to measure blood pressure bid,each 2 or 3 times,and calculate the average.When the patient bring back the table,the doctor measure the his FMD,and rate scale of traditional Chinese medicine.At the very beginning and the end of the investigation,check the blood routine、urine routine、liver function、renal function、blood lipid、blood coagulation function、 hey、hscrp、sod、ECG、24h blood pressure monitoring、FMD、rate scale of traditional Chinese medicine.Put the index into the Excel database, SPSS version 20 software was used for statistical analysis.Results:①Systolic pressure and diastole pressure decreased significantly P<0.01.② The average SBP and the average DBP of the whole day,the average SBP and the average DBP of the day,the average SBP and the average DBP of the nignt,and the load level of all, decreased significantly P<0.01.③The PP of the whole day decreased significantly P<0.01,the PP of the day decreased P<0.05.④The MAP of the whole day、the MAP of the day、the MAP of the night decreased significantly P<0.01.⑤Systolic pressure SD and diastole pressure SD throughout the whole day decreased P<0.05, diastole pressure SD throughout the day decreased significantly P<0.01.⑥The DBP of the morning blood pressure peak decreased P<.05.⑦ T/P decreased P<0.05.⑧Symptom count decreased significantly P<0.01⑨The FMD increased significantly P<0.01. ⑩ The cholesterol and triglycerides decreased significantly after treatment P<0.01,the HDL increased significantly P<0.01.Conclusion:As a compound prescription Sang-qi granula has good effect at therapy hypertension in clinic. It can decrease blood pressure,both the SBP and DBP;decrease the PP and MAP; decrease the SBP SD and DBP SD;decrease the DBP of the morning peak.Sang-qi gualuna can decrease symptom, raise the quality of life. Sang-qi gualuna can raise the FMD.Sang-qi gualuna can decrease cholesterol and triglycerides,and raise the HDL.
