| ObjectiveTo observe the effect of prescription for PingganJianpiHuatan on essentialhypertension(EH) patients' arterial blood pressure and discuss its mechanisms bymonitoring vascular endothelial function.Methods60 patients with grade1-2 EH patients were randomly divided into two groups according tothe numbers: the Chinese Medicine Board(A), Western Medicine group(B). A group ofpatients with prescription for PingganJianpiHuatan 1 package, orally twice a day; B grouppatients were treated with enalapril(Yangtze River Pharmacy Group production), 10mg,twice a day orally. The prescription for PingganJianpiHuatan by gastrodiae, rhynchophy,concha haliotidis, eucommia, leonuri, gardenia, achyranthis bidentata, loranthaceae,pachyma, citrus reticulate, magnol, atractylodes macrocephala koidz was produced intogranules each weighing 10g and 10g per gram granules equivalent Chinese medicine by999 Pharmacy Group production. 30 patients with grade1-2 EH patients as control group(C)with placebo 1 package, orally twice a day; placebo mainly by starch was produced by 999Pharmacy Group production. Treatment for 4 weeks. A and B patients in the day of thetreatment and the next day of the end of the treatment were conducted clinical symptomsand syndromes treatment efficacy assessment, A, B and C patients in the day of thetreatment and the next day of the end of the treatment were measured on blood pressure and flow-mediated brachial artery endothelium-dependent vasodilation(FMD) by ultrasonictesting. Changes in blood pressure and FMD before and after treatment were analyzed.Results(1) Clinical symptoms and syndromes treatment efficacy assessment: the difference wassignificant(P<0.01) on the overall syndrome effects between A and B group after treatment,A group was better than B group. Symptom scores of two groups were significantlyreduced, significant difference compared with those before treatment(P<0.01). Comparisonbetween A and B group after treatment on symptom score improvement A group was betterthan B group ,the difference was significant(P<0.01).(2) Changes in blood pressure: Both systolic and diastolic blood pressure of A and B groupwere decreased significantly after treatment, compared with C group, the difference wassignificant(P<0.01). Comparison between groups shows A and B group were better than Cgroup(P<0.05) after treatment. Diastolic blood pressure and blood pressure margin werecompared difference(P<0.05), diastolic pressure drop suggested that B group was betterthan A group.(3) Brachial artery by ultrasonic testing: the difference was significant(P<0.01) in FMD ofA and B group before and after treatment, comparison of the groups after treatment, A andB Group significantly higher(P<0.01) than C group in FMD. NMD was no statisticallysignificant difference(P>0.05) between the groups before and after treatment.(4) Correlation analysis: blood pressure and symptom scores before and after treatmentbetween A and B group were no correlation(P>0.05). Systolic blood pressure margin beforeand after treatment was positively correlated with FMD(P<0.01).Conclusion(1) Prescription for PingganJianpiHuatan can lower blood pressure of patients with EH,significantly lower systolic blood pressure.(2) Prescription for PingganJianpiHuatan can improve the clinical symptoms of patientswith EH, the effect of antihypertensive treatment was better than Western Medicinegroup(B).(3) Prescription for PingganJianpiHuatan can improve vascular endothelium-dependentvasodilation in patients with EH, protection of vascular endothelial function in bloodpressure may be one of the mechanisms play antihypertensive effect. (4) Upward attack of liver-yang and obstructing phlegm are the common syndromes ofpatients with EH in clinic. Prescription for PingganJianpiHuatan is another effectivemethod of treating hypertension. |
